Buprenorphine is available in Singapore as substitution treatment for opioid dependence since 2002. This study surveys buprenorphine related deaths in Singapore between September 2003 and December 2004. The aims are to establish the autopsy prevalence of buprenorphine related deaths and the demographical and toxicological profile of the cases. Toxicological screening was performed for all unnatural deaths, deaths involving known drug addicts, as well as when autopsy revealed no obvious cause of death. Twenty-one cases had buprenorphine detected in post-mortem blood and/or urine samples. Eighteen were sudden deaths. There were two fatal falls from height and one death by hanging. All subjects were male. The age range was 24-48 years. Fourteen subjects were between 30 and 39 years of age. The mean age was 35 years. The majority (62%) were Chinese. Eleven (52%) were known drug abusers. For sudden deaths, two groups were identified. Six cases died from natural causes. Blood buprenorphine levels ranged from undetected (detected in urine) to 3.2 ng/mL (mean 1.4 ng/mL). Twelve cases were attributed directly and indirectly to mixed drug poisoning. Blood buprenorphine levels ranged from undetected (detected in urine) to 17 ng/mL (mean 3.2 ng/mL). Nineteen cases showed concurrent abuse of buprenorphine and benzodiazepine, diazepam being the most frequently detected, followed by nitrazepam and midazolam. The availability of buprenorphine as substitution therapy is associated with an increase in buprenorphine related deaths. The danger of co-abuse of buprenorphine and benzodiazepines is highlighted.  相似文献

Some scholars argue that the Afghan police are regularly engaged in bribery and drug-related corruption. Prevalent corruption in the Afghan police has demonstrably resulted in greater support for the Taliban which is a threat to security. This suggests that a robust anti-corruption strategy is needed to restore legitimacy in the Afghan state and police and to counter the insurgency. This article initially provides a discussion of police corruption in Afghanistan that reveals four interrelated explanations: (1) structural causes of corruption, patronage and nepotism, (2) low pay, (3) state capture, and (4) ethnic favouritism. The research methodology included 70 semi-structured interviews with elites conducted by the author in Kabul during May–June 2010 and 100 surveys conducted with patrolmen and lieutenants in various Afghan provinces during January–March 2012. The article finds that it can prove counterproductive to post poorly paid policemen in distant provinces in order to challenge patronage relations as it tends to exacerbate survival-based corruption. Moreover, improved pay reform could help reduce survival-based corruption in the Afghan police but systemic corruption, clientelism and state capture will remain if it is not implemented alongside other wider structural initiatives.  相似文献

Moral Foundations Theory posits five distinct foundations of morality: Harm/Care, Fairness/Reciprocity, In-group/Loyalty, Authority/Respect, and Purity/Sanctity. In combination, this should yield between four-to-six moral signatures—distinct combinations or patterns of support for these aspects of morality. We extend previous research by examining the replicability of these moral signatures in a New Zealand-based national sample (n = 3,635). Latent Profile Analysis identified four distinct moral signatures: Individuators, Moderates, Neutrals, and High Moralists. We integrate these moral signatures within the Dual Process Model (DPM) framework and show that Social Dominance Orientation predicts membership in the Neutral moral signature (moderate/lukewarm support for multiple moral foundations); whereas Right-Wing Authoritarianism predicts membership in the High Moralist signature (undifferentiated high support across moral foundations). These findings were observed controlling for Big-Six personality and various demographics. Thus, the authoritarian and dominance-based motives identified by the DPM independently predict categorical differences in the signatures people use to judge morality.  相似文献

Children living on ‘the edge-of-care’ are typically known to local safeguarding authorities and are considered likely to face risks to their safety. Many are subject to a child protection plan and/or involved in ‘pre-proceedings’ processes. A growing number of their parents face (un)diagnosed mental health difficulties as well as economic and social precarity. This article draws on a mixed methods evaluation of a pilot service in the East of England offering a therapeutically led attachment-based intervention for families. The service cross-cuts health and social care, allowing psychologists and psychotherapists to work alongside social workers and other practitioners. The evaluation examined psychological and safeguarding outcomes and explored practitioner perspectives. A key outcome was that 85.4% of families were enabled to remain, or reunite with their child, compared with an estimated 50% of ‘edge-of-care’ cases nationally. This supports the need for similarly oriented interventions that could help lower the incidence of child removals.  相似文献

Abstract. Devolution has been described as a key ‘global trend’ over recent decades as governments have decentralised power and responsibilities to subordinate regional institutions. UK devolution is characterised by its asymmetrical nature with different territories granted different institutional arrangements and powers. This paper seeks to examine the role of state personnel in mobilising the new institutional machinery and managing the process of devolution, focusing on transport policy. The research presented shows a clear contrast between London and Northern Ireland, on the one hand, and Scotland and Wales, on the other, in terms of the effectiveness of political leaders in creating clear policy priorities and momentum in transport.  相似文献
