Bile is, in certain cases, collected together with blood from different sites (heart, brain, femoral), urine and other organs or matrices. This study reports comparative results obtained from the analysis of blood and bile for different drugs found: acetaminophen, amphetamine and related compounds, several antidepressants, several benzodiazepines, cocaine and its metabolites, dextropropoxyphene and its metabolite, hydroxyzine, methadone and metabolite, morphine and codeine, levomepromazine, thioridazine, propranolol, tramadol and its metabolite. Several findings are presented: (1) There were no significant differences in the levels of the compounds among the samples of blood obtained from different sites. (2) Levels in bile are generally several fold higher than those in blood. The mean bile to blood ratios vary from about 1 (for acetaminophen, amphetamine) to about 2000 (for desmethylclobazam). (3) In certain cases (16 over 44), although the drug or its metabolite was not detected in blood from different sites, it was detected in bile. As other authors had advocated, it is very useful to ask the pathologist to take the gall bladder with its contents together with the other samples, in order that the sample of bile can be used in the comprehensive toxicological analysis and therefore be complementary to the other fluids or matrices. An additional advantage for using bile is that the concentrations of drugs or their metabolites are generally several fold higher than their blood concentrations.  相似文献

No consensus yet exists on how to handle incidental findings (IFs) in human subjects research. Yet empirical studies document IFs in a wide range of research studies, where IFs are findings beyond the aims of the study that are of potential health or reproductive importance to the individual research participant. This paper reports recommendations of a two-year project group funded by NIH to study how to manage IFs in genetic and genomic research, as well as imaging research. We conclude that researchers have an obligation to address the possibility of discovering IFs in their protocol and communications with the IRB, and in their consent forms and communications with research participants. Researchers should establish a pathway for handling IFs and communicate that to the IRB and research participants. We recommend a pathway and categorize IFs into those that must be disclosed to research participants, those that may be disclosed, and those that should not be disclosed.  相似文献

In this paper, results are introduced of an empirical study which investigated the practice of occupational health and safety regulation in two countries, Great Britain and West Germany. The analysis concentrates on the specific relationship that regulatory agencies and the concerned interest groups, employers and trade unions, develop in the two stages of policy formulation and implementation of health and safety regulation. Results show that interest groups actively participate in the regulation-making systems in both countries. Their policy pursued in this process is influenced and mediated by their actual resources as well as by individual views and assessments of each side's representatives. In Germany, a strong fragmentation with numerous private and quasi-governmental bodies entitled to formulate standards makes it difficult for the interest groups to concentrate on the competent and important committees. In contrast, we find a clearer responsibility of bodies and committees in the case of Britain. In both countries, enforcement policy prefers an advisory and persuasive style even though the legal backgrounds are quite different.  相似文献

This case report sets forth an authenticity examination of 35 encrypted, proprietary-format digital audio files containing recorded telephone conversations between two codefendants in a criminal matter. The codefendant who recorded the conversations did so on a recording system he developed; additionally, he was both a forensic audio authenticity examiner, who had published and presented in the field, and was the head of a professional audio society's writing group for authenticity standards. The authors conducted the examination of the recordings following nine laboratory steps of the peer-reviewed and published 11-step digital audio authenticity protocol. Based considerably on the codefendant's direct involvement with the development of the encrypted audio format, his experience in the field of forensic audio authenticity analysis, and the ease with which the audio files could be accessed, converted, edited in the gap areas, and reconstructed in such a way that the processes were undetected, the authors concluded that the recordings could not be scientifically authenticated through accepted forensic practices.  相似文献

This paper assesses the role of the European Union in the Libyan crisis (2011) and critically considers the implications for its evolution as an international security actor. Employing role theory, the paper reviews the historical development of the Union's security actorness and sheds specific light on the balance between self-conception and external expectations in the case of the Libyan crisis. Its central argument is that, despite external expectations and European narratives of a ‘comprehensive power role’, the Libyan crisis showed that the Union still acts in line with its traditional role as a civilian power. The inability to go beyond civilian power stemmed from internal dissonance on a potential hard power role and a corresponding lack of material capabilities. The growing gap between expectations about comprehensive actorness on the one hand and performance on the other is likely to damage the Union's future credibility as an international security actor.  相似文献

Transnational non‐state governance arrangements (NGAs) are increasingly common in areas such as labor standards and environmental sustainability, often presenting themselves as innovative means through which the lives of marginalized communities in developing countries can be improved. Yet in some cases, the policy interventions adopted by the managers of these NGAs appear not to be welcomed by their supposed beneficiaries. This article accounts for this predicament by examining the effects of different configurations of accountability within NGAs promoting labor rights. Most labor‐rights NGAs incorporate “proxy accountability” arrangements, in which consumers and activists hold decision makers accountable “on behalf” of the putative beneficiaries of the NGAs: workers and affected communities in poorer countries. The article shows how and why different combinations of proxy versus beneficiary accountability influence the choice of policy instruments used by NGAs, and applies the argument to three prominent non‐state initiatives in the domain of labor standards.  相似文献

The EU’s reaction is slow; the EU is divided; the EU is unable to deliver: time and time again, newspapers depict the image of an incoherent and uncoordinated EU foreign policy. This time, the topic under discussion was the EU’s response to the Libyan crisis. Many have compared the EU’s internal divisions over Libya with those over the Iraq war, an often used example to illustrate the limits of the Common Foreign and Security Policy (CFSP). This article aims to assess the coherence of the EU’s short- to medium-term response to the Libyan crisis. It distinguishes between the horizontal, inter-institutional, vertical and multilateral dimensions of EU coherence. The analysis shows that unilateral actions or inactions of the member states mainly account for the EU's incoherent response. The post-Lisbon institutional structure has done little to compensate for these internal divisions. While the EU cannot change the course of national foreign policies, it should increase its ‘leadership for coherence’, Europeanise its crisis response in the medium term and aim at preventing incoherence in the longer term.  相似文献

How might a global bioethics account for profound cultural difference in a world marked by porous borders? The authors endorse a critical, self-reflexive bioethics, suggesting that bioethics needs to change its fundamental orientation if it is going to remain relevant and intellectually vibrant throughout the twenty-first century. Bioethics must attend to issue of social justice and public health, while seriously considering the implications of social context for medical morality. Negotiating moral consensus across cultural boundaries will be difficult, but is is more likely to succeed if we critically engage with the cultural assumptions underlying bioethics itself.  相似文献

In this article we examine whether and how the policy‐oriented coalition building and the gridlock model of legislative choice explain changes in the allocation of German budgetary items between 1961 and 1994. The policy‐oriented coalition‐building approach suggests that only policy‐connected budgetary actors can make changes in the budgetary composition, while the location of the status quo is essential for budgetary modifications according to the gridlock model. We also draw the attention to the identification of the set of relevant budgetary veto players and their preferences by distinguishing between models of the government, the political parties of the Bundestag, and the two of them. We test the predictive power of these approaches and conceptions in a competitive manner and control for macroeconomic and other political factors that might influence changes in the budgetary allocation. Our findings show that the gridlock model better explains budgetary changes, and that the government is responsible for decreasing, but the political parties of the Bundestag majority are decisive for increasing the size of budgetary items. We conclude that identifying the relevant veto players and using these more sophisticated approaches on budgetary decision making provide significant insights into the political reasons for changing the allocation of the German budget.  相似文献
