正常人短音AEP反应阈与主观听阈之间关系的研究

摘    要:目的研究正常听力受试者的纯音主观听阈、短音主观听阈和短音AEP反应阈三者之间的关系,旨在达到通过短音AEP反应阈评估不同频率纯音听阈的目的。方法选择主观检查配合的正常听力受试者40名(80耳),依次测试1000Hz、500Hz、2000Hz和4000Hz四个频率的纯音主观听阈、短音主观听阈和短音AEP反应阈。结果用短音诱发AEP时,在四个测试频率上,均可见一个正相波,其潜伏期在7~15ms之间,平均潜伏期在10ms左右,我们命名为P10。在500Hz、1000Hz、2000Hz和4000Hz四个频率上,短音P10波反应阈与纯音主观听阈之差的均值和标准差分别为23.38±10.09、23.63±8.23、17.38±8.86、16.50±7.04dB。四个频率上的短音P10波反应阈与纯音主观听阈之差一般在20dB之内。因此,用短音P10波反应阈减去0~20dB,可以评估纯音听阈。结论短音诱发的P10波反应阈可以评估各个频率的纯音主观听阈,弥补短声诱发的ABR的不足。

An experimental study on prediction of behavioral thresholds using reaction thresholds of auditory-evoked potentials evoked by tone pips in normal- hearing subjects
Abstract:Objective In the present study we investigated the relationship among the behavioral pure tone thresholds,behavioral tone pips thresholds and AEP thresholds evoked by tone pips in order to predict the behavioral pure tone thresholds at different frequencies using AEP thresholds elicited by tone pips.Methods Forty healthy undergraduates (80 ears) studying at China Medical University were recruited in the study. All had normal hearing sensitivity at all frequencies from 500 to 8000 Hz (<25dBHL). After pure tone auditory thresholds had been obtained at 1000,500,2000 and 4000Hz,behavioral thresholds and AEP thresholds to tone pips were measured. ResultsA positive wave was obtained from all the subjects for each frequency. The latency of the component ranged from 7 to 15 ms,which was named positive response 10 (P_ 10 ) as was approximately 10ms in average. Mean differences and standard deviations between P_ 10 wave thresholds to tone pips and behavioral pure tone thresholds at 500,1000,2000 and 4000Hz were 23.38±10.09,23.63±8.23,17.38±8.86 and 16.50±7.04 dBnHL,respectively. Mean values of the differences between P_ 10 wave thresholds to tone pips and behavioral pure tone thresholds at 500,1000,2000 and 4000Hz were approximately 20 dBnHL. Therefore,when P_ 10 thresholds were used to predict behavioral pure tone thresholds,the value could be estimated with P_ 10 wave thresholds subtracting 0~20 dBnHL,indicating the predicted range of behavioral pure tone thresholds. Conclusion P_ 10 to tone pips can be used for predicting the auditory thresholds at each frequency,which may serve as a supplementary evidence for the clicks-evoked ABR.
