芪黄健脾滋肾颗粒治疗系统性红斑狼疮的疗效观察

目的 观察芪黄健脾滋肾颗粒治疗系统性红斑狼疮（systemic lupus erythematosus，SLE）的临床疗效及对患者生活质量的影响。方法 将60例SLE患者随机分成对照组和观察组，每组30例，对照组予以醋酸泼尼松片联合硫酸羟氯喹片口服，观察组在对照组治疗的基础上加芪黄健脾滋肾颗粒口服，疗程均为12周；观察两组患者治疗前后狼疮生活质量（lupus quality of life，Lupus QOL）量表评分、SLE疾病活动指数（SLE disease activity index，SLEDAI）和中医症状评分的变化，检测治疗前后中性粒细胞与淋巴细胞比值（neutrophil/lymphocyte ratio，NLR）、24 h尿蛋白（24 hour urine protein，24hUP）、免疫球蛋白（immunoglobulin，Ig）G、IgM、IgA及血清补体（complement，C）3、C4水平。结果 观察组疗效优于对照组（P＜0.05）。治疗后两组患者SLEDAI、中医症状评分较治疗前均显著减少（P＜0.05），NLR、24hUP、IgG、IgM、IgA水平较治疗前均显著降低（P＜0.05），且观察组减少或降低程度均较对照组更为显著（P＜0.05）；Lupus QOL量表各项评分较治疗前均显著增加（P＜0.05），C3、C4水平均较治疗前显著升高（P＜0.05），且观察组增加或升高程度均较对照组更为显著（P＜0.05）。结论 芪黄健脾滋肾颗粒治疗SLE临床疗效显著，具有免疫调节和抗炎作用，能显著改善患者生活质量。

Clinical Effect of Qihuang Jianpi Zishen Granule in Treatment of Systemic Lupus Erythematosus

Objective To investigate the clinical effect of Qihuang Jianpi Zishen Granule in the treatment of systemic lupus erythematosus (SLE) and its effect on the quality of life of patients. Methods A total of 60 SLE patients were randomly divided into control group and observation group,with 30 cases in each group.The patients in the control group were given oral administration of prednisone acetate tablets combined with chloroquine bisulfate tablets, and those in the observation group were given oral administration of Qihuang Jianpi Zishen Granule in addition to the treatment in the control group;the course of treatment was 12 weeks for both groups.The two groups were observed in terms of the changes in lupus quality of life (Lupus QOL),SLE disease activity index (SLEDAI),and traditional Chinese medicine (TCM) symptom score after treatment,and related laboratory markers were measured before and after treatment,including neutrophil to lymphocyte ratio (NLR),24 hour urinary protein (24hUP),immunoglobulin G (IgG),immunoglobulin M (IgM),immunoglobulin A (IgA),complement C3,and complement C4. Results The observation group had a significantly better clinical outcome than the control group (P<0.05).After treatment,both groups had significant reductions in SLEDAI and TCM symptom score (P<0.05) and the levels of NLR,24hUP,IgG,IgM,and IgA (P<0.05),and the observation group had significantly greater reductions than the control group (P<0.05). After treatment,both groups had significant increases in the scores of each dimension of Lupus QOL scale (P<0.05) and the levels of C3 and C4 (P<0.05),and the observation group had significantly greater increases than the control group (P<0.05). Conclusion Qihuang Jianpi Zishen Granule has a marked clinical effect in the treatment of SLE,with immunoregulatory and anti inflammatory effects,and it can significantly improve the quality of life of patients.
