香菇多糖联合顺铂治疗肺癌胸水的临床研究

摘    要:目的 评价香菇多糖联合顺铂胸腔灌注治疗肺癌胸水的疗效。方法 将60例肺癌合并胸水患者随机分为观察组和对照组,每组30例。对照组患者先放尽胸水,再胸腔灌注顺铂;观察组胸腔内联合注入香菇多糖和顺铂。观察两组临床疗效、中医证候疗效、卡氏评分(Karnofsky performance scale, KPS)疗效以及免疫功能变化。结果 观察组的临床疗效、中医证候疗效和KPS疗效均优于对照组,差异有统计学意义(P<0.05);观察组治疗后CD4+T细胞比例及CD4+/CD8+比值均较治疗前明显上升(P<0.05),而对照组上述指标反有轻微下降(P>0.05)。结论 香菇多糖与顺铂联合治疗肺癌胸水的疗效优于单用顺铂,安全性较好,可增强患者免疫功能。

Clinical Efficacy of Lentinan Combined with Cisplatin in Treatment of Lung Cancer with Pleural Effusion

Abstract:Objective To evaluate the clinical efficacy of lentinan combined with cisplatin given by pleural infusion in the treatment of lung cancer with pleural effusion. Methods Sixty lung cancer patients with pleural effusion were randomly divided into observation group (n=30 ) and control group (n=30). The control group underwent drainage of pleural effusion followed by pleural infusion of cisplatin; the observation group was given lentinan and cisplatin by pleural infusion. The treatment outcomes were assessed in terms of clinical symptoms, traditional Chinese medicine (TCM) syndromes, and Karnofsky Performance Scale (KPS) scores, and the improvements in immune function were evaluated. Results Compared with the control group, the observation group had significantly better treatment outcomes in terms of clinical symptoms, TCM syndromes, and KPS scores ( P<0.05). The observation group showed significant increases in percentage of CD4+ T cells and CD4+/CD8+ ratio after treatment (P<0.05), but the control group showed slight decreases in the two parameters (P>0.05). Conclusion Lentinan combined with cisplatin has better efficacy than cisplatin alone in the treatment of lung cancer with pleural effusion, and the combination therapy has high safety and can improve immune function.
