This study examines gap—traditional reciprocal associations common in Central Asia. Gap is an unofficial, regular get-together of people of similar age bound by socially acknowledged ties. The study argues that gap represents a communal type of civil society featuring many differences from analogous groups in Western liberal societies. Gap may effect social consciousness and facilitate mobilisation. The study also analyses modern forms of gap such as joint savings funds, female-only get-togethers, and internet-based gap. 相似文献

A qualitative study of primary health care teams in the UK provides insight into their co-ordination or learning-orientation and how this affects member commitment, conceptions of ‘core’ and periphery, group identification and willingness to integrate roles. Even if the ideal of a learning-oriented primary health care team is viewed as a myth, it can still perform useful functions despite an apparent withdrawal of its official authorization by the Cameron-led government. The perpetuation of this myth maintains a tension between reality and potentiality in the hopes that inspire its pursuit as an ideal of team formation. 相似文献

Since it was first advanced in the 1994 Human development report of the United Nations Development Programme [United Nations Development Project. (1994). Human development report. New York: UN], the concept of ‘human security’ has evolved as a holistic development-oriented acuity [Nef, J. (1999). Human security and mutual vulnerability: The global political economy of development and underdevelopment (2nd ed.). Ottawa: International Development Research Centre]. The human security concept reinforces the right to health, drawing on both the role of states and the global community's commitment to human rights. Yet, health and human security, long the purview of state power and responsibility, increasingly include alliances of state and non-state actors. This paper proceeds in three parts. The first looks at health and human security linkages, charting the trajectory of the health and human security relationship. The second deals with policy and operational implications. It explores the health–human security link, paying particular attention to the allocation of responsibility and accountability, including through private–public partnerships and rising powers such as China. The third provides a theoretical and technical analysis of the status of health and human security since 1994, taking into account its evolution vis-à-vis human rights’ development and development more broadly, also asking whether it represents but a wrinkle in time or a new sustainable development paradigm. 相似文献

Abstract This note presents wide evidence on the relationship between population and income for 125 countries for which data was available for the period 1950–2000. The main result is that there is a weak but negative relationship between population growth and per capita GDP, as income increases population expands at a slower rate. This relationship appears to be stronger for African countries and for Asian countries before 1970. 相似文献

AbstractThis paper critically reviews evidence from low and middle income countries that pensions are associated with better health outcomes for older people. It draws on new, nationally representative survey data from South Africa to provide a systematic analysis of pension effects on health and quality of life. It reports significant associations with the frequency of health service utilisation, as well as with awareness and treatment of hypertension. There is, however, no association with actual control of hypertension, self-reported health or quality of life. The paper calls for a more balanced and integrated approach to social protection for older people. 相似文献

AbstractProviding—and also not providing—public services to unlawful residents implies a certain cost for host societies, and both inclusion and exclusion involve localized renegotiations of fundamental rights, legitimate needs, and social membership. Based on original qualitative research data, this article compares how, why, and under which conditions irregular migrants are granted or denied access to healthcare services provided in London and Barcelona. From a multi-level perspective and by drawing on organization theory, I highlight key differences in how the responsible governments deal with the underlying contradictions and thereby either help or hinder effective policy implementation. 相似文献

New public management (NPM) provides an opportunity to analyze a reform trajectory that is influenced by country-specific institutional and social traditions. France adopted its own version of NPM, embracing some of its “traditional” elements such as quasi-markets and performance evaluation and rejecting others. NPM reforms in France pursued a re-centralization agenda rather than a disaggregation of public agencies. Outcomes were below expectations in core areas such as citizen participation and physician professional satisfaction. Subsequent health reforms indicate convergence toward a hybrid system rather than toward a post-NPM paradigm. 相似文献

The quantity-quality trade-off is one important motivation for the family planning policies in many developing countries. In this paper, we examine the effect of number of siblings on children’s health and education in China. We find evidence of quantity-quality trade-off in children’s health but not in children’s education. Our study has three contributions. First, we focus not only on children’s education but also on children’s health, which has received rather little attention in the literature. Secondly, we use a new source of exogenous variation in fertility – variations of the strictness of the One-Child policy across localities in China – to construct instrumental variable's (IV's). Finally, we empirically explore the underlying mechanism of the quantity-quality trade-off, and find supporting evidence for the resource dilution hypothesis. 相似文献

This paper studies the relationship between the use of formal and informal health care in a developing country setting by examining the introduction of a social health insurance scheme in Ghana. We estimate the effects of gaining coverage on changes in care seeking behaviour and show how these effects differ by age and wealth status. District-level differences in the implementation of the insurance scheme provide exogenous variation in access to insurance and allow us to address issues with selection into coverage. Results indicate that insurance access strongly increased use of formal care and reduced out-of-pocket expenditures on health services. 相似文献

This article uses Bosnia and Hercegovina as a prototype of an emerging subset of weak states, the passive-aggressive state. Appearing as the result of agreements ending civil strife, the general characteristics of the passive-aggressive state include complex power-sharing arrangements at the centre and local institutions that are designed for capture by parties to the conflict, which in turn creates a weak and segmented civil society. As a result, only the international community can devise and financially support policy reform, but it must rely on the state to implement the reform. Dependent on international support for its own survival, the centre rhetorically embraces reform while local institutions engage in passive resistance to block it. This article uses three examples of health care reform in Bosnia to illustrate the difficulties of reform in these types of states. The paper concludes with observations on how strengthening the powers of the central government and reorienting international aid towards civil society might alleviate some of the structural problems of passive-aggressive states.

The enormous success of local independent lists and independent mayors in Poland suggests it is important to examine their distinctive features, analyse the place they occupy in local politics, and explore the model of local government they encourage. This paper deals with these topics by investigating some extreme cases of non-partisanship at the local level. Research was conducted in three arbitrarily chosen Polish cities governed by independent mayors and dominated by independent lists in the local legislatures. Mixed quantitative and qualitative methods were used: a survey among city councillors; and individual key-informant interviews (IDI). Non-partisans appear to be a separate category of local politicians, different in terms of their views on politics. They explain their non-partisanship using anti-partisan, historical or tactical justifications. It seems that non-partisanship can be a means of institutionalising a particular manner of understanding and engaging in politics. Local lists organised by independent mayors are informal, concentrating on the leader and performing only some of the traditional functions of parties. This paper demonstrates that, due to the vague nature of the links between local politicians and their administration, independent councillors may serve only as a ‘democratic addition’ to a local administration headed by a skilful manager. 相似文献

Memory studies is simultaneously still in its charismatic phase, though it no longer has a right to be, as well as highly resistant to efforts to escape from it, though it clearly needs to do so. Anthropometric, mortality, and population data presented by Svedberg [1990] in this journal suggested a slight anti‐male bias in undernutrition in sub‐Saharan Africa. This article re‐analyses some of the same anthropometric, mortality, and population data and supplements them with more recent findings. In contrast to Svedberg's results, it finds evidence of a slight and rising anti‐female bias in sub‐Saharan Africa, which is particularly apparent in mortality and population indicators. 相似文献

This article presents an analysis of two post-Soviet states, Kazakhstan and Azerbaijan, which can be identified as post-Soviet rentier states. Both countries are characterised economically by enormous national resources of gas and oil and low economic diversification as well as politically by strong autocratic presidentialism with neopatrimonial structures. These two factors, combined with further post-Soviet legacies such as a low level of political interest in the respective societies and a basically hierarchical orientation of the population, lead to a specific post-Soviet variety of rentierism. From a political science perspective, this article reveals the impact of resource policies on these comparably new political systems and concludes with a summary of core features of these post-Soviet rentier states. 相似文献
