An impact incurred by the movable head may bring about a change in intracranial pressure and this change may play an important part in the occurrence of the cerebral contusion. We have carried out the following experiments to determine whether the intracranial pressure change was attributed to an accelerated motion of the head or to a skull deformation. In the blow experiment in which the head was accelerated, a positive peak in the intracranial pressure was recorded immediately after impact at the impact site and a negative one at a site opposite the impact. In the one in which the skull could be deformed, the intracranial pressure curves at both sites contained harmonics. The modal analysis revealed an inbending in the frontal and occipital regions of the skull and an outbending in the parietal and temporal regions immediately after impact, followed by a reverse deformation. Regarding the intracranial pressure change, positive pressures were recorded in the frontal and occipital regions immediately after impact, followed by a negative one. This study demonstrated that the positive and negative peaks were caused by the accelerated motion of the head, and that the curve of the intracranial pressure changes contained harmonics which were caused by the deformation of the skull.  相似文献

The 202 deaths connected with burns in 1976-1986 included 134 in which death occurred during the fire. In 49 cases the face was so badly charred that it was not possible to judge whether hemorrhage had occurred in the eyelids and/or conjuctivae. Such blood extravasation was seen in 14 of the remaining 85 cases, taking the form of petechiae in some cases and in others the form associated with more extensive extravasation, in some cases hemolytic. No other features known to cause such hemorrhages were present, so that they are attributed to the burning process itself. Signs of extensive pronounced burns were found in 13 of these cases, and it seemed that high-degree burns on the neck and trunk with less severe burns on the head were particularly likely to be associated with such hemorrhages. In most cases the flames had quickly come into direct contact with the body (clothing, bed or chair had caught fire), leading to rapid death, as documented by the fact that usually very little or no soot at all had been aspirated and by the low level of carbon monoxide intoxication (max. COHb 22%). One plausible explanation for the development of hemorrhage is the supposition first expressed over 20 years ago that when the circulation is maintained rapid burn-induced shrinkage of the skin of the neck has the effect of strangulating the victim; hemorrhage of eyelids and/or conjunctivae in such cases be could regarded as a vital reaction to the effect of fire. This is particularly significant, insofar as evidence of hemorrhage of this kind was found predominantly in bodies in which other vital signs indicative of the effects of burning were sparse and slight or even totally absent. In addition, specific examination of the laryngeal area revealed congestion-induced extravasation at various points, as well as petechial hemorrhage in the mucous membrane.  相似文献

The article describes the historical and legal basis, as well as the ballistic criteria of injuries caused by air-rifle shots to the head. Six of our own cases of craniocerebral air-gun pellet injuries are discussed and critically assessed, using the results communicated in other papers as the basis of the discussion. The different post-traumatic complications, including the histopathological reaction of the brain to lead-pellets, are considered. The literature is reviewed and the indications for stereotactic removal or craniotomy of intracranial air-gun pellets are discussed. Air rifles, while occasionally the cause of serious head injuries, are rarely thought of as lethal weapons. However, this article illustrates the potential penetrating power of the air-gun pellet, a fact not always appreciated by physicians.  相似文献

Missile head injury is predominantly caused by firearms; however, small blunt objects such as a stone can be propelled into the air and cause a low-velocity-type missile injury, as described in this case report. Since the cerebral damage in low-velocity missile injuries is usually focal, there might not be disturbance of consciousness until secondary brain damage occurs; therefore, adequate medical supervision is necessary to prevent and treat potentially fatal complications in a timely manner. This is a rare case of a penetrating missile wound of the head, most likely caused by the use of a stone propelled by a slingshot that penetrated the skull and lacerated the brain with the stone embedded within the substance of the brain. The deceased died at home 2 days later from focal suppurative meningitis with cerebral abscess formation and a subdural hemorrhage.  相似文献

Serious head and neck injuries are a common finding in fatalities associated with rollover crashes. In some fatal rollover crashes, particularly when ejection occurs, the determination of which occupant was driving at the time of the crash may be uncertain. In the present investigation, we describe the analysis of rollover crash data from the National Automotive Sampling System-Crashworthiness Data System for the years 1997 through 2007 in which we examined the relationship between a serious head and neck injury in an occupant and a specified degree of roof deformation at the occupant's seating position. We found 960 occupants who qualified for the analysis, with 142 deaths among the subjects. Using a ranked composite head and neck injury score (the HNISS) we found a strong relationship between HNISS and the degree of roof crush. As a result of the analysis, we arrived at a predictive model, in which each additional unit increase in HNISS equated to an increased odds of roof crush as follows: for ≥8cm of roof crush compared with <8cm by 4%, for ≥15cm of roof crush compared to <8cm by 6% and for ≥30cm of roof crush compared to <8cm by 11%. We describe two hypothetical scenarios in which the model could be applied to the real world investigation of occupant position in a rollover crash-related fatality.  相似文献

Direction, site and muzzle target distance can indicate suicide or homicide. This conclusion can be drawn from autopsies of 57 cases of suicide and 68 cases of homicide by handgun fired at close range to the head and neck together with going to the crimescene in some cases. This study was carried out in Bangkok during the period from January 1983 to January 1986. In order to determine whether it was suicide or homicide, the path of the bullet, the site, the muzzle target distance must be considered. The angle of the bullet would be either elevated (from below upward), horizontal or an angle of depression (from above downward). For suicide, the direction of the bullet should be at an angle of elevation in the majority of cases. The position of the handgun in relation to the head in suicide was most often in tight contact and near contact. For homicide, the direction of the bullet should be horizontal in most cases. The bullet was at close range in the majority of the cases. There are 8 common sites for suicide and homicide and 10 different sites in the case of homicide which are at neck, left cheek, left aural region, lip, left occipital area orbit, chin, left eyebrow, submental and nose.  相似文献

目的采用CT扫描动态观察新西兰白兔死亡后脑组织变化,建立利用CT值、脑组织/颅腔面积比,推断死亡时间(PM I)的研究方法。方法通过耳缘静脉注射空气,建立新西兰白兔死亡模型;于死亡后0~120h,运用CT进行头颅平描;选取颅顶面层测定脑组织CT值、脑组织/颅腔面积比。结果死亡后兔脑组织CT值呈双峰样上升、脑组织/颅腔面积比呈不断下降趋势;建立与死亡时间的回归方程:CT值=47.4780-0.1768T+0.0049T2(T/h,CT值/HU)、面积比=99.7368+0.3098T-0.0118T2/100(T/h)。结论CT扫描检查可准确显示兔脑的死后变化;脑组织消失之前,综合运用CT检查指标可用于准确推断死亡时间。  相似文献

No reliable data are available on cases of lethal child abuse (by active force) in the area of Federal Republic of Germany prior to reunification (the former West Germany). In a multicenter study we therefore examined the police and court records for such cases occurring in the period 1 January 1985 to 2 October 1990 in nearly the entire area of Federal Republic of Germany. RESULTS: The study center received information on 58 cases of lethal child abuse. Extrapolated to all institutes of legal medicine, this corresponds to 62 cases in all of West Germany in the period studied. An approximately equal number of unreported cases should be added to this figure. Including unreported cases, at least 20 cases of lethal child abuse occurred per year; thus only one in every two cases ever came to light. Almost two thirds of the victims were younger than one year old. At autopsy 59% exhibited signs of repeated abuse at autopsy. By far the most common cause of death was direct impact from a blunt object, usually to the head. Mostly, the male person to whom the victim relates most closely (father, stepfather, partner of the mother) has killed the child. Twenty-one of the 74 persons charged saw the charges against them dropped or were acquitted due to lack of evidence; 51 received sentences ranging from one year probation to life. In the remaining two cases the outcome of the trial was unknown. Signs of abuse were readily apparent at autopsy in almost all cases. The high number of unreported cases underscores the need to educate medical students and practicing physicians to be on the look-out for signs of abuse and argues for an increase in the rate of autopsy.  相似文献

In a case of murder by a single .22 caliber bullet fragmented in the head, determination of the trajectory was a key issue in identifying which of 2 windows, 1 of which was associated with 2 suspects, was the site of fire. For this purpose, we processed the computed tomography of the victim's head by using a radiation therapy planning system called Plato, which is routinely used in radiation oncology. This tool provided a 3-dimensional reconstruction of the cranio-cerebral lesions and a spatial location of bullet and bone fragments. In this article is described the more intuitive relationship with forensic and ballistic data obtained from the 3-dimensional visualization and the physical and mathematical analysis used to calculate the angle of shot.  相似文献

Forensic anthropology has traditionally relied on two-dimensional (2D) images, such as photographs and sketches, to perform analyses, and disseminate findings. However, as 3D imaging technology advances, it has become more widely implemented into forensic anthropology analysis and practice. Teaching and learning in forensic anthropology still often relies on 2D images, but increasingly three-dimensional (3D) models are available to be used by students training in anatomy and osteology. Additionally, 3D models have been found to be beneficial to comprehension in other contexts within forensic anthropology, such as in the courtroom. The use of these models in the teaching of forensic anthropology is not yet widely implemented and more importantly, the impact on learning is not yet understood. The use of 3D imaging and visualisation in other educational contexts has seen positive results, for example in medical training. To explore this further, a study was conducted using an online activity to compare the comprehension scores of students educated using 2D textbook style images or 3D models on Sketchfab. The results showed that the use of 3D images was not detrimental to comprehension. Students using the 3D models were more consistent in their performance and reported an increase in confidence regardless of prior experience. The results of this study are of particular importance when distance learning due to the COVID-19 pandemic which means that students cannot always learn in a laboratory environment.  相似文献

目的 观察头面部拳掌伤,及钝器打击伤致脑干损伤死的病理形态学特点,并探讨其成伤机理。方法 对23例拳击(A组)与30例钝器打击(B组)采用作者建立的脑干损伤取材方法,于脑干各颅神经根部作水平面切6块,HE染色光镜检查。结果 2组脑干损伤具有相同的病理形态学改变。A组水肿显著(71.6%),B组以组织撕裂、挫碎为多(76.6%);A组颅神经损伤多为单根(47.6%),B组2根以上占53.7%,最多可达4根;A组单纯脑实质挫伤多(50%),B组多伴发血肿或挫挤带。结论 拳、掌和钝器打击头面部致脑干损伤的病理形态学相同,但钝器打击比拳、掌致脑干损伤严重且广泛。提示脑干损伤致命与其受力部位、发病机理关系密切,而与受力强度关系不大。  相似文献

The collision speed is important in accident analysis, and needle marks can be helpful as the physical evidence. The deceleration impact system has been built to analyze the mechanics of the needle and the gauge plate. Two isolated groups were designed to record the speed values under the same sample labels from real crashes. The visualization platform was built for the first group to collect needle marks. The second group recorded the speed values by other methods. The collision deceleration, the gauge plate materials, and the collision directions determine the forming of the needle marks. There were eight positive results from the 23 effective samples (12 frontal, four side, and seven rear), with discernible tip and/or middle marks on gauge plates. Multiple marks have been distinguished effectively. The low- and high-speed impacts have no obvious differences for real needle marks. It is more accurate for frontal impacts.  相似文献
