An unusual case of double death

The report presents the case of an alleged double homicide or a homicide followed by suicide of a couple, in which the court-ordered autopsy of both victims revealed that the homicide had been committed by the husband immediately before he died himself from cardiac failure. The article gives a short summary of the pathophysiological relations between emotional tension (stress) and physical strain and the effects on the cardiovascular system. The case report describes a homicide by strangulation immediately followed by the perpetuator's death from a natural cause.     

Death related to aortic coarctation in a young female during sexual intercourse

Coarctation of the aorta (CA) is diagnosed mainly in pediatric patients, and therapy is conservative if asymptomatic, but surgical treatment is required if advanced arterial hypertension is present. Moderate to severe forms contraindicate any type of physical activity requiring cardiac effort. Here, we describe the first documented death of an apparently healthy 35-year-old woman because of cardiac tamponade by rupture of an aortic aneurysm, possibly related to congenital CA, prolonged use of oxymetazoline hydrochloride, and physical and/or emotional stress during sexual activity. Our patient was asymptomatic for classical CA symptoms. The patient's breathing difficulties likely in hindsight were due not so much to nasal congestion, but rather to an ineffective oxygenation of the blood from the abnormal heart. In an attempt to treat the "nasal disease," the patient ingested chronic and excessive doses of decongestants, aggravating her fatal disease. The danger of inhaling large doses of nasal decongestants without an appropriate medical indication is highlighted here.     

An unusual finding in a case of hypothermia

Suicide was initiated by several incisions in the skin; the resulting hypovolemia resulted in fatal hypothermia. The unusual autopsy findings in this case due to hypothermia were focal hemorrhages and necrosis of the pancreas and hemorrhagic infarction of the descendending colon. The pathophysiological relevance of the autopsy and histological findings are discussed.     

An unusual rubber stamp case

Given a questioned document and a questioned pre-inked rubber stamp, comparisons of each can be made with a microscope or similar magnification device. If sufficient evidence exists, these questioned items can be linked even if the stamp was not used as it was originally designed.     

An unusual case of railway suicide

A rare case of suicide in which the victim had been lying along the railway track in a supine position and with extended extremities is described. The wheels of a train caused longitudinal hemisection with complete evisceration. Epidemiological data on train suicides are given and the relationship between this method of suicide and mental illnesses and consumption of alcohol are discussed.     

An unusual case of corpse dismemberment

A case of corpse dismemberment into 850 fragments was investigated at the Cracow Department of Forensic Medicine. The perpetrator employed various tools to divide the body into fragments and subsequently boiled the pieces to reduce their volume. The thus-prepared body decreased its volume to 30 kg. Subsequently, the perpetrator placed the body fragments in 2 large pots, which were then positioned in the space under the stairs and walled in.     

Rectal fist insertion. An unusual form of sexual behavior

Rectal fist insertion (fist fucking) is an uncommon and potentially dangerous sexual practice. This is usually a homosexual activity, but can also be a heterosexual or an autoerotic practice. One known death has been reported associated with rectal fist insertion, in which the complications of anal and colonic tears and bleeding had occurred (see Editor's note). The possibility of drug overdose is also probable, as drugs and alcohol are commonly introduced into the rectum to promote sphincter relaxation and to ease the discomfort of anal dilatation.     

An unusual case of pseudo rape.

A case has been reported which adds a new and unusual dimension to false rape allegation and gives a heretofore unknown motive for a woman to claim a bogus rape. A female newspaper reporter entered a hospital, stated falsely that she had been sexually assaulted, allowed herself to be interviewed and then examined by the author, and accepted and took medication, all because she wanted to "find out how a rape victim is treated at the new Rape Center and I was pretending to be a rape victim." Rape center workers must be alert to spurious claims of unprincipled and deceitful women.     

An unusual case of cranial image recognition

We encountered an unusual practical case of craniofacial identification recently. The peculiarity of the case was that the skull itself was not available for examination unlike other such common cases. The supplied material exhibits were, a nearly front view photograph of a skeletonized face and a front view face photograph of the suspected victim. Further, the condition of the skull during taking its photograph was such that its lower and the upper jaws were not in a normal closed condition. The procedure involved in dealing with such a complicated craniofacial identification problem would be quite interesting from a forensic investigator's point of view, since standard methods of skull identification like photo/video superimposition techniques were not at all applicable here. As such, the present case report provides the details of the multiphase procedure adapted by us in dealing with this abnormal case. A solution to this unprecedented craniofacial identification problem was worked out by appropriate exploration of a newly introduced digital image processing technique that is based on craniofacial symmetry perception. The procedure leads to the reconstruction of a superimposable cranial image with upper and lower teeth in normal closed condition for establishment of its identity in usual way.     

An unusual suicide with a safety razor blade--a case report

A 29-year-old male was found dead lying in a pool of blood inside a community toilet locked from inside. The individual is alleged to have committed suicide by cutting his throat with a safety razor blade, which was found at the scene by the investigating police authority. The deceased, as per his relatives, was suffering from depression for the last few days.     

An unusual autoerotic accident: sexual pleasure from peritoneal pain

The paper describes a fatal autoerotic accident of a 23-year-old man who derived sexual pleasure from painful irritation of the peritoneum by sharp force. For this purpose he clamped two knife blades into a vice suspended from a rope-pulley construction so that he could lower it to pierce the abdominal skin with the tips of the knives. When one of the ropes broke, the vice with the knives fell onto the man's belly piercing the inferior vena cava and leading to death by exsanguination. This, to the authors' knowledge, has been the first ever report of a fatal autoerotic accident in which sexual pleasure was to be obtained from painful irritation of the peritoneum.     

An unusual case of anesthetic abuse by a full-face gas mask

The authors report a case of sniffing of halothane (Narcotan) by a 32-year-old man, master of pharmacy, through the military full-face gas mask. The liquid halothane had been applied on the scrubber of the gas mask and voluntarily inhaled. The sniffer was found dead in his flat, with the gas mask still fixed and sealed on his face. Because the authors have not encountered any report of such a case in the literature, they present and discuss this case in this article.     

An unusual fatal case of accidental asphyxia in a child.
