"This essay presents tentative findings for the Sundsvall area of Sweden, which is being studied in a recent project on the decline of infant and childhood mortality in the Nordic countries. The focus is on the complete reproductive histories of single mothers and the life expectancies among infants born to women who at least once in their reproductive life history experienced the birth of an illegitimate child. In Sundsvall, industrialization only temporarily affected the illegitimacy ratio, but its effect was obvious even in agrarian parishes. The number of illegitimate children per woman remained relatively stable over time, with the exception of the town of Sundsvall. It was more common in the urban environment to give birth to several illegitimate children. Mortality was also higher among these infants, but the negative effects can be seen equally among both legitimate and illegitimate children."  相似文献

This paper examines the existence of a mortality penalty for illegitimate abandoned infants in the Foundling Hospital of Madrid, La Inclusa, during the period 1890–1935, in the context of the mortality experience of the city. A rich dataset on the life histories of all infants abandoned in the city of Madrid (almost 60,000 children), nominally linked to births, has allowed the study of the determinants of mortality of foundlings, newborns in the city, and the study of the determinants of abandonment. Contrary to previous findings, our results for La Inclusa show no evidence of an illegitimacy penalty among foundlings and, in some cases, show even better prospects for them. However, this situation did not reflect the circumstances of the city, as illegitimate infants were both more likely to suffer a neonatal death and to be abandoned. The explanation proposed in this paper is that the health of infants born of wedded couples that ended up resorting to abandonment was possibly poorer than that of those born of single or migrant mothers, as the situation triggering abandonment meant that infants were under very hard conditions, possibly worse than those single women faced in the event of a pregnancy.  相似文献

The rise in illegitimate fertility in the late 18th and early 19th centuries has often been related to increasing economic and social vulnerability in the urban industrializing world. Many studies using macro-level data or analyzing individual characteristics of unwed mothers have found support for the vulnerability hypothesis. In this article, we investigate illegitimate childbearing in early 19th century Geneva in a longitudinal perspective. Relating events (illegitimate births) to the population at risk (single women), our multivariate analysis shows that the segment of the female population assumed to be most vulnerable – immigrants and maids – did not have a higher risk of illegitimacy. However, the substantially increased risk among women who already gave birth to illegitimate children indicates the existence of a small but highly vulnerable group of women.  相似文献

The essay examines the impact of socio-economic and demographic change on the living arrangements of different groups of elderly in Sundsvall, an industrial town in nineteenth-century Sweden. The proportion of old parents having children living nearby was stable throughout the century, although the proportion living in the same households as their children decreased over time, probably because the children had the economic resources to form households of their own earlier. The proportion of elderly not having relatives at hand increased, however, due to a higher proportion of unmarried old persons, many of whom had in-migrated to Sundsvall late in life.IntroductionChildren's responsibility for their old parents was deeply rooted in preindustrial Sweden. In medieval legislation, it was already stressed that the main responsibility for caring for the elderly lay with the family. The substantial increase in life expectancy over the past 150 years can largely be attributed to a drastic decline in infant mortality. In the continuing debate about the reasons for this development, recent research has focused attention on the role of public health. Since the 1870s in Germany, municipal and national public health strategies were launched. One major element of this policy was the fight against high infant mortality rates in urban areas. The establishment of municipal milk supplies and the creation of infant welfare centers attacked gastrointestinal disorders, the predominant cause of death among infants. This article investigates these developments in the largest German cities, particularly Düsseldorf, which is located in one of the most industrialized regions.  相似文献

We examined the sex ratio in sudden infant death syndrome (SIDS) cases in Hungary, in Tokyo and Japan between 1985 and 1996. From all the infant death cases in Hungary 395 (240 male, 155 female) were SIDS (odds ratio (OR)=1.179, with 95% confidence interval (CI)=0.961, 1.446), in Japan 4348 (2550 male, 1798 female) were SIDS (OR=1.145, with 95% CI=1.076, 1.218) and in Tokyo 307 (178 male, 129 female) were SIDS (OR=1.128, with 95% CI=0.894, 1.423). Male infants showed a significantly higher birth rate than females. The male infants are more vulnerable (p<0.005), however, higher mortality among male infants should not be considered a characteristic feature for SIDS.  相似文献

The discussion still continues among researchers about the causes for the decline in infant mortality in widespread parts of Europe at the turn of the 19th century. This article is based on sources such as unpublished statistical material from Prussia, parish registers, and lineage and village genealogies. As these sources are broken down by town and countryside and into various phases of infant mortality, they provide evidence for further reflection that may be fruitful. Thus, several causes for the decrease in infant mortality and ways of diffusion can be excluded and the impact of others delimited. The present study concludes that research should pay more attention to changes in infant care than it usually does.  相似文献

This study investigates differentials in the decline of cause-specific infant mortality by marital status of the mother in Stockholm (1878–1925) and factors contributing to the explanation of these differentials using computerized records of individual entries from the Roteman Archives. Included in the analysis were 120,094 children less than 1 year of age who lived in Södermalm during this period. Cause-specific mortality rates were calculated for three time periods. Cox's regression analysis was used to study the relationship between overall and cause-specific risk of infant death and of being born in and out of wedlock in relation to a set of variables. Infant mortality rates and mortality risks were higher among children born out of rather than in wedlock. The most pronounced differentials in cause-specific mortality rates between these groups of children were seen in cases of diarrhea. The socioeconomic status of the household head and number of children in the household were statistically significant with infant mortality, but explain only part of the excess mortality risk of children born out of wedlock. In Stockholm at the turn of the 19th century being born out of wedlock was strongly associated with poor health outcomes, particularly in diarrheal diseases, pneumonia/bronchitis, and immaturity/congenital causes.  相似文献

Estimates of intimate partner violence (IPV) during pregnancy vary by population being studied, measures, and other methodological limitations, hindering the ability to gauge the relationship between IPV and negative birth outcomes. The authors report aggregated data from a subsample (n = 148) of the first three waves of the Women's Employment Study. The authors compared groups of women who did and did not give birth to low birth weight infants on demographic, material deprivation, risk behavior, mental health, and IPV factors. The prevalence of domestic violence was more than twice as high for women with low birth weight infants as those women who had a normal weight infant. When considering additional risk factors, including food insufficiency, substance dependence, and depression and/or posttraumatic stress disorder, IPV remained a significant indicator, but it was most strongly associated with low birth weight among women also experiencing depression and/or posttraumatic stress disorder.  相似文献

In February 1929 the Bishop of Ossory commented on the fact that in Ireland illegitimate infants were often ‘done to death by father or relatives’ (Irish Catholic, 16 February 1929). There were many instances where family members of unmarried women who gave birth were the sole defendants or co-defendants in infanticide cases in post-independent Ireland. Although illegitimate infants were ‘done to death’ by their fathers in a number of cases that were tried at the Central Criminal Court in Dublin between 1922 and 1950, this article will focus on cases where infants were murdered or suspected of having been murdered by relatives of the birth mother both in the Twenty-Six Counties and in Northern Ireland (Irish Catholic, 16 February 1929). For the purposes of this article I have referred to the murder of illegitimate infants as ‘infanticide’ even though there was no separate charge of infanticide in the Irish Free State until 1949. The English infanticide acts of 1922 and 1938 also applied to Northern Ireland. This article discusses the motives of the relatives of single mothers who played a part in the deaths of illegitimate infants. Unmarried motherhood was severely frowned upon in Ireland and the relatives of single pregnant women assisted their female kin in destroying the evidence of extra-marital conception in order to protect the family's honour and moral reputation in the wider community. The records of infanticide trials provide a great deal of insight into the ways in which mainly working-class families dealt with the strain of pregnancy outside wedlock in Ireland between 1922 and 1950.  相似文献

ABSTRACT: It is essential that clinical physicians, medical personnel, medical examiners, and law enforcement agencies understand the types of injuries seen and demographics of children affected by intentional blunt force as this understanding can be crucial to the death and/or criminal investigations. An understanding of the injuries can also assist in drawing conclusions regarding how those injuries could have been sustained. This study discusses the types and patterns of injuries seen in blunt force homicides in children younger than 6 years. The study found that male infants are more often intentionally injured than are female infants and that fatal head injuries most frequently occur in the first year of life, whereas most fatal thoracoabdominal injuries occur in the first 3 years of life. In children with head injuries, subdural hemorrhage was the most common finding, followed by subarachnoid hemorrhage. In 2.5% of deaths due to head injury, concurrent neck injury was seen, a percentage far lower than previous literature would suggest if shaking was the primary mechanism of injury. Twelve legal confessions were also reviewed, none of which disclosed a pure mechanism of shaking the infant.  相似文献

At the end of the nineteenth century, infant mortality rates started to fall rapidly in the Netherlands. Unfortunately, not all regions benefited from this development. High infant death in the Roman Catholic provinces of North-Brabant and Limburg has often been ascribed to a growing reluctance of Catholic mothers to breastfeed their infants after 1870. This was supposedly caused by the combination of a strict, prudish Roman Catholic norm prohibiting women from baring their breasts and a refusal to accept new medical insights into healthy childcare. The food given to weaned children was generally of such poor quality that many infants succumbed to gastrointestinal diseases. Consequently, infant mortality rates caused by water- and food-borne infectious diseases would have been higher amongst weaned babies. By using recently digitised municipal cause-of-death registration statistics, it is possible to see if there are, indeed, indications of a shift in breastfeeding patterns after 1870. First, the authors look at infant deaths from all causes to see whether Roman Catholic municipalities underwent a rise in the mortality of children under the age of one. Second, the authors do the same for cause-specific infant mortality from typhus, typhoid fever, diarrhoea, dysentery, acute diseases of the digestive system and cholera. Based on the outcomes, there was no homogenous rise in infant mortality in all Roman Catholic municipalities. Furthermore, there is no indication that infant mortality due to digestive diseases increased uniformly in all Roman Catholic communities between 1875 and 1899. Either some communities were able to counteract the negative effects of a shift towards weaning or changes in breastfeeding patterns were not a specific Roman Catholic phenomenon at all.  相似文献

The epidemiology of sudden infant death syndrome in Finland in 1969-1980   总被引:3,自引:0,他引:3  
SIDS cases were defined by examining all death certificates, in which sudden deaths were expected to be found from the years 1969-80 from the Central Statistical Office of Finland. The age limits were 28-364 days. If the death certificate did not give enough information as to whether the cause of death was explained or unexplained, autopsy records and microscopic specimens were examined. If the death was sudden, but no autopsy was done, no microscopic specimens were taken, or there were some slight findings which could have partly explained the death were classified as borderline cases. The mean annual incidence of SIDS in Finland was 0.41/1000 livebirths in 1969-80. In 1969-74 and 1975-80 the incidences were 0.31 and 0.51, respectively. The increasing tendency of SIDS was partly due to more borderline cases in the first period and partly due to more twins, and infants with small birth weight, dying of SIDS in the second period. Deaths at weekends and sleeping with parents in the second period were more common than in the first study period. In the SIDs group the young maternal age, low social class, family type unmarried couple or single mother, maternal anemia during pregnancy were more common than in the control group. Mothers of SIDS infants had more previous children and fewer visits and later first visit to prenatal clinics than control mothers. The duration of gestation was shorter and the mean birth weight and length were smaller in the SIDS case than in the control group. Twins were more common among SIDS infants than in the common population. The most important risk factor of SIDS was maternal smoking during pregnancy. The epidemiological results conform with the hypoxia hypotheses.  相似文献

实践中对于非婚生育是否应当征收社会抚养费存在争议,而争议的背后是社会伦理道德、规范解读和理论适用等多方面因素共同作用的结果。非婚生育社会抚养费在行政收费原理下应围绕国家主体以概括量化的数字形式确定范围,避免婚姻状态对费用征收范围界定产生的影响。同时,在规范视角下,法律规定的“国家提倡一对夫妻生育两个子女”属于强制性义务,但不宜解读为禁止非婚生育行为。婚生与非婚生育本质上同属于生育行为,在同一体系内应当保证费用征收的同一性。因此,必须基于理论和规范的分析,方可界定非婚生育社会抚养费征收范围。  相似文献

"In the nineteenth century, the demographic development of the Meierij, a region in the south-east of the Netherlands, was different from that of the rest of modernizing northern Europe. Infant mortality remained high, while it dropped elsewhere. The article shows why the current explanation for high infant mortality, which links a sustained high infant mortality to a change in feeding habits is not valid. Increased fertility due, among other reasons, to a lower marital age offers a better explanation. Changes in economic options open to unmarried women provide the clue. With fewer premarital occupational possibilities, women would have been more inclined to marry, or there would have been less pressure on them to forestall a marriage in order to profit to the full from the occupational options. More and earlier marriages meant more children were born, and also a higher infant mortality rate."  相似文献

A qualitative secondary analysis explored stories of mothers (n = 49) who left violent relationships (VRs) through a lens of maternal identity. Constant comparative method identified a theory of Awakened Maternal Identity (AMI) and Leaving VR for the Infant/Children. Mothers described how the VR diminished their maternal identity (DMI). Partners controlled the VR though unrealistic infant care expectations, criticisms of infant care, harsh parenting, and control over mothering decisions. DMI lowered the mother’s capacity to provide emotionally nurturing infant care. Over time, mothers experienced AMI- as their ‘eyes were opened’ they experienced a stronger sense of mothering responsibility, focused more on the infants and children, and eventually prioritized their relationship with the infants and children over the partner. AMI seemed a turning point that led to leaving the VR for the infants/children. Recommendations offered for professionals to foster AMI as potential means to initiate the leaving VRs.  相似文献

The paper offers an analysis of the demography of the confessionally-divided and heavily industrialized northern Irish town of Lurgan in the early twentieth century. Its main source is the individual household forms of the Irish population census of 1911. This was the first Irish census to provide household-level data on the number of children born—and the number still surviving—to married couples co-resident at the time of the census. The paper begins by producing standard cross-tabulations of socio-economic status, as represented by literacy, numeracy, occupation, and the labour force participation of married women, by religion. It then presents econometric analyses of female labour force participation, marital fertility, and infant and child mortality. The greater poverty of Catholic households forced a higher proportion of mothers to work outside the home, mainly as unskilled workers in the town's linen mills. The analysis of the marital fertility suggests that confessional affiliation did not influence family size much at this juncture. The analysis of infant and child mortality suggests that religious affiliation was to a significant extent a cloak for living standards and socio-economic advantage.  相似文献

Six cases of cocaine-related deaths of infants have covered the spectrum of potentially devastating effects. They include an intrauterine death of a 35-week-old fetus following acute maternal cocaine abuse; anoxic encephalopathy at birth with 3 months' vegetative survival from a similar episode; traumatic compression asphyxia in a 4-month-old; infectious cardiomyopathy with heart failure in a twin at age 21 months following maternal cocaine abuse at birth; malnutrition and dehydration in a 7-week-old during continuing cocaine abuse by the parents; and a teenage sibling's cocaine lacing of a baby milk bottle ingested by his 6-week-old brother. All the cases had positive toxicological screening for cocaine or metabolites or both in the mother at delivery or in the infant at birth, or both. There were no instances of sudden infant death syndrome (SIDS, or "crib death"). Pathologic and toxicologic, as well as birth, developmental, and social data are presented. An integrated medical, public health, law enforcement, and educational policy to prevent or at least ameliorate these tragic cases, now approaching epidemic proportions, has yet to be developed. A careful obstetrical history and examination of the mother, indication on the birth certificate of maternal drug abuse, and notification of health authorities (by birth certificate checking, among other ways) may send an early warning message to providers for intercession. Active ingestion/injection and passive inhalation by older children and teenagers require more intensive monitoring and aggressive interaction by pediatricians, social workers, school authorities, and employers.(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献
