This article argues that for a number of reasons, the Russian state has had limited success in tackling the growing problem of HIV/AIDS. It begins with brief discussions of the health context of HIV/AIDS in the Russian Federation and the development of institutions in Russia post‐1991. It then discusses the attempts that have been made thus far to tackle the HIV/AIDS epidemic, both on the part of the federal government and with the aid of international donor funding. It is argued that a number of factors have contributed to Russia's lack of success in tackling HIV/AIDS. Firstly, the infection has been interpreted as a health problem rather than as a phenomenon which impacts on all aspects of public life. Secondly, there have been weak incentives at the federal level, which has resulted in a lack of a coordinated approach to HIV/AIDS at the national level. Thirdly, there have been regional variations in the autonomy and independence from the federal state impacting on the struggle against HIV in the Russian regions. Conclusions Australia and Thailand have made great progress in partnering with NGOs to respond to HIV/AIDS through the protection of human rights. Unquestionably, the Australian experience is more advanced. However, it is important to note that Australia’s political institutions and traditions were able to empower and accept an NGO movement of this nature almost from the start of disease identification.Thailand did not have this advantage, having only moved toward political institutions that are open to public opinion and civil society’s input within the last 15 years. In spite of their varied histories, both countries eventually traveled down similar paths. In Australia, HIV/AIDS NGOs moved rapidly forward from being standard-setting, fact-finding, and advocacy organizations to becoming capable of creating new HIV/AIDS umbrella organizations and influencing existing governmental organizations on HIV/AIDS human rights issues. Indeed, by the close of the 1980s, NGOs had established themselves firmly in all of these roles. The fact that Australia still struggles with addressing an emerging epidemic among indigenous people is more a sign that the movement has thus far been incomplete than that it is faltering or ineffective. Additionally, now that NGOs are actively working through AFAO on behalf of indigenous peoples, it is likely that there will finally be more movement on human rights and H1V/AIDS issues for this group. However, sex tourism and the illegal trafficking of women and children for purposes of prostitution continue to require ongoing proactive management by the partnership before they become a serious epidemic threat. Thailand’s situation is somewhat different from that of Australia. The HIV/AIDS NGO community has grown since the epidemic exploded in the 1990s, but the organizations themselves continue to have limited power, While they have been an important voice in human rights standard-setting, fact-finding and advocacy regarding HIV/AIDS, they remain unable to fully influence the governmental organizations that ultimately make and implement human rights policies in these areas. As the NGO experience in general is new to Thai politics, continuing human rights abuses are a sign of the miles left to travel on this road rather than an indicator that the road needs to be abandoned. Regardless of their differing experiences with creating HIV/AIDS partnerships, it is impossible to say that either effort has failed to use this mechanism successfully to at least begin seriously addressing HIV/AIDS human rights issues. What can be said is that each partnership can be placed at a differing point on a continuum of effectiveness that ultimately concludes with a fully integrated partnership capable of fundamentally influencing a country’s HIV/AIDS human rights policies on an ongoing basis.  相似文献

This article examines the political dimensions of Uganda's progress in bringing a generalised HIV/AIDS epidemic under control. The article documents the history of the political processes involved in Uganda's battle against HIV/AIDS and analyses the complexities of presidential action and the relation between action at the level of the state and that taken within societal organisations. By the mid‐1980s, Uganda was experiencing a full‐blown epidemic, the virulence of which was connected with social dislocation and insecurity related to economic crisis and war. Political authorities faced the same challenge as other regimes experiencing the onslaught of AIDS in Africa. The epidemiological characteristics of HIV and AIDS—transmission through heterosexual activities, with a long gestation period, affecting people in the prime of their productive life—meant that action required wide‐reaching changes in sexual behaviour, and the educational activities to achieve this, as well as relatively complex systems to monitor the virus and control medical practices (blood supplies, injection practices, mitigating drug delivery). The centralist character of the Museveni regime was crucial not only to mobilising state organisations and foreign aid resources, but also to ensuring significant involvement from non‐state associations and religious authorities. The Ugandan experience demonstrates that there is a tension between the requirements for systematic action that a strong public authority can deliver and the need to disseminate information requiring a degree of democratic openness. The President was able to forge a coalition behind an HIV/AIDS campaign in part because the virus largely ignored the privileges of wealth and political power. With the development of antiretroviral therapy and the access that the wealthy can gain to these drugs, this basis for the broadest possible coalition to fight HIV/AIDS may be weakened in the future. In most countries in Africa, the epidemiologic profile of HIV/AIDS is significantly different from that of the USA or Europe. Women in Africa are as likely to be HIV positive as men, while young women are significantly more likely to be HIV positive than young men. How can health research in Africa be made more responsive and relevant to women’s health needs? And how would a human rights perspective change the conduct of biomedical and social scientific research on gender and HIV/ AIDS in Africa? The application of a human rights framework to HIV/AIDS typically has focused on social justice issues employing national and international legal structures to legislate and advocate for HIV positive persons. This essay, however, offers some broad considerations of the links between the health of African women, biomedical and social scientific research, HIV/AIDS, research ethics, and the human rights movement.  相似文献

What impact has HIV/AIDS had on the structure of public administration and what further lessons do these changes hold for other policy sectors in Botswana? For long, Botswana has had the highest prevalence of HIV/AIDS, bringing many developmental challenges. An under‐researched facet of HIV/AIDS is the effect it had on the public administration in Botswana. Whereas classical approaches to public administration suggest that it is ‘civil’ service organisations that lead in health administration, HIV/AIDS spawned a particular type of organisation, the policy network. In mitigating HIV/AIDS, the policy environment became more fragmented with networks for treatment, prevention, advocacy and research emerging. These networks are made up of entities from the public, private for‐profit and not for‐profit sectors. They participate in the agenda setting, formulation, implementation and monitoring and evaluation of HIV/AIDS policy. Traditional public administration theories cease to hold sway; private actors become engaged in ‘public policy’ and the other way round. Policy is carried out in horizontal arrangements; linking government, business and non‐governmental organisations in mutual inter‐dependences. Health care professionals share policy spaces with the media, social scientists and politicians. The policy adoption is often seen as a function of internal determinants, external determinants, or a combination of the two. In the years since the start of the HIV/AIDS epidemic nearly every country in the world has adopted of some nature a program to combat the epidemic. This article demonstrates the mixed mechanisms influencing the adoption of AIDS programs, in a quantitative study of 90 countries. Prior studies have claimed that multilateral organizations were vital to global action against the HIV/AIDS epidemic. This study provides empirical evidence showing the actions taken by the multilateral organizations have both helped and hindered efforts to combat the HIV/AIDS epidemic. The spreading of HIV/AIDS in the African continent and related discrimination and stigmatisation towards people living with the virus have become a major issue to be addressed by communities, health organisations and governments in Africa. An effective way to successfully overcome these plagues necessitates the application of social marketing techniques and campaigns. However, when it comes to funding and supporting social marketing activities, African leaders deny the important role that the government could play in achieving the ‘HIV/AIDS free generation goal’ as well as making African societies a non‐discriminative and stigmatisation places for infected people. The present study intend to demonstrate through analysis of empirical data that social marketing is crucial in changing people's sexual behaviour and battling misconceptions that lead to discrimination and stigmatisation of infected people. In this light, African political authorities such as Congolese leaders need to understand the relevancy of this discipline in their HIV/AIDS policy.  相似文献

This paper provides an overview of the literature concerned with the impact of HIV on public administration, looking in particular at projections of the impact of HIV/AIDS on public expenditure, the impact of attrition on the health and education sectors and on the civil service as a whole. ‘Success stories’ such as Uganda and Senegal are discussed through their representation in the literature, and ‘political commitment’ is identified as a critical but under‐theorised factor in tackling HIV. Having taken as its starting point the existing work on HIV and governance, the paper then considers the impact of the epidemic in conditions of state vacuum—conflict situations. While both India and Brazil are seriously affected by the HIV/AIDS epidemic, each country has chosen a different approach to providing affordable pharmaceutical treatment. Whereas the Indian government has paved the way for market-driven solutions, Brazilian public authorities are strongly involved in the research and production of HIV/AIDS medication. Brazilian regulations permit comprehensive and free provision of HIV/AIDS drugs, whereas the majority of the affected population in India does not receive adequate pharmaceutical treatment. To explain the different policy outputs, we draw on the developmental state literature. Efficient decisionmaking structures, a devoted bureaucracy, and effective policy instruments enable public authorities to provide public goods even in the context of relative scarcity. We show that the assumptions of developmental state theory have to be complemented by the assessment of civil society actors' potential to trigger governmental interventions in the market.  相似文献

This paper examines the dynamics of condom policymaking in Malawi by analyzing debates, which took place in the Malawi National Assembly between the year 2000 and 2004. Using content analysis and key informant interviews, and situating the overall discussion within policy and science literature, we examine how scientific evidence is being applied in the policymaking process as it relates to the place of condoms within the context of HIV/AIDS prevention. The study not only shows the extent to which the policymaking process on condoms in Malawi largely embodies a tendency to blur the conventional divide between science and politics, but also demonstrates why controversy around condoms still persists at the highest level of government in a country with one of the highest HIV prevalence rates in the world. It was found that even when people are dying, consensus in HIV/AIDS policymaking can be difficult to achieve in a policy climate characterized by a binary perspective to social problems, and where different actors compete for control over the policy terrain. The paper makes recommendations that may be helpful in facilitating a more inclusive HIV/AIDS policymaking process in Malawi.  相似文献

The prevalence of HIV/AIDS in South Africa has drawn particular attention in recent years, not only because of the country's high rates of infection but also because of the highly contentious debates between the state, AIDS NGOs, and scientists over AIDS policy. The national AIDS lobby in South Africa, including groups such as the Treatment Action Campaign (TAC), has been forcefully articulating a rights-based discourse as a strategy to realize particular normative claims on the state. I examine these recent public debates and challenges to the human rights based response to AIDS that has gained consensus internationally among AIDS NGOs, governments, doctors and medical researchers. I explore the limitations of framing political demands in terms of the constitution and within a rights-based discourse in the present are of globalization and neo-liberalism, where the state’s capacity to respond to the social welfare demands of its citizens has declined, and where the protection of universal human rights requires that powerful transnational actors, in addition to the state, be held democratically accountable.  相似文献

Political advertising on Facebook is the latest in a long line of developments in campaign practice, and is a tool that has been mobilised extensively in elections around the world. In this article, we explore what we know about Facebook advertising at elections and ask what existing data from the UK Electoral Commission can reveal about current usage. Highlighting the principles behind Facebook advertising, we argue that existing metrics offer little insight into current campaign trends—posing analytical, methodological and normative challenges for academics and electoral regulators alike. Moreover, we argue that these challenges strike at the heart of debates about democratic responsibility and the degree to which governments should cede responsibility to commercial actors who may have differing understandings of fundamental democratic norms.  相似文献

This paper investigates responsibility/irresponsibility as an important regulatory strategy, in two instinct but interrelated social sites – people living with AIDS and HIV, and lesbians and gay men, especially within the Canadian province of Ontario. This responsibility/irresponsibility technique of governance is developing in response to the struggles of people with AIDS and community-based AIDS groups and lesbians and gay men and the alternative strategies of regulation articulated through these struggles. Practices of ‘resistance’ and activism alter the grounds of hegemonic regulation/governance and point towards new techniques of governance. I show how responsibilizing and normalizing strategies in the context of social struggles over AIDS and sexual regulation allows for those constructed as ‘responsible’ to be managed through forms of self-regulation and professional forms of goverance of their live. For those who continue to be constructed as ‘irresponsible’ forms of criminal law, policing and public health governance are called into action.  相似文献

Neuromarketing is an emerging field that bridges the study of consumer behavior with neuroscience. Controversial when it first emerged in 2002, the field is gaining rapid credibility and adoption among advertising and marketing professionals. Each year, over 400 billion dollars is invested in advertising campaigns. Yet, conventional methods for testing and predicting the effectiveness of those investments have generally failed because they depend on consumers’ willingness and competency to describe how they feel when they are exposed to an advertisement. Neuromarketing offers cutting edge methods for directly probing minds without requiring demanding cognitive or conscious participation. This paper discusses the promise of the burgeoning field of neuromarketing and suggests it has the potential to significantly improve the effectiveness of both commercial and cause-related advertising messages around the world.  相似文献

This article introduces the new Australian Alcohol Advertising Review Board (AARB) Code and assesses television advertising practices against its advertising content provisions. The Code is administered by independent experts to provide an alternative to the industry‐led Alcohol Beverages Advertising Code Scheme that has attracted substantial criticism. The new Code aims to balance the alcohol industry's right to promote a legal product against critical protections for young people and public health. To assess whether the new Code will require substantial changes to alcohol advertising practices, a content analysis was conducted of alcohol advertisements aired prior to its introduction on all four free‐to‐air commercial television channels over two months. A majority of the analysed advertisements (48 of 64) contained at least one element that could be construed as a breach of the AARB Code. The largest numbers of potential breaches were for the provisions relating to the association of alcohol with success and using appeals that are likely to be attractive to young people. The results demonstrate that the Australian alcohol industry will need to reassess current advertising practices to achieve compliance with the new Code. Government performance measurement is often faulted for focusing on outputs, while citizens are said to demand more information on outcomes to hold government accountable. To compare the influence of these measures, we randomized 774 participants to receive outcome or output information about a real HIV prevention program, with or without cost information, in a survey experiment. Citizens expressed less support for spending on the program when shown outcomes (infections prevented), rather than outputs (people served). Showing participants the high cost of treating HIV/AIDS boosted support for program spending, but did not make outcome information more persuasive. We interpret these results as partly a reaction to the high per-unit cost of an outcome in an HIV prevention program. But it may reflect a tendency of citizens simply to misinterpret less costly outputs, including serving more people, as if these were outcomes. This bias has implications for performance reporting and accountability.  相似文献

The author argues that to be successful companies need to adopt a stakeholder view of how they interact with society—in short their communications need to be integrated. Rather than focusing purely on their consumer markets, advertisers should be aware of the impact of their messages on public opinion, pressure groups, politicians and others that make up the ‘political market’. Equally, public affairs practitioners should be more involved in the commercial activities of their colleagues in advertising and marketing. Integrated communications, in the author's view, is not only an operational necessity for companies, but a strategic imperative for sustainable success in a global world with differing local expectations. The present article offers a critical appraisal of resemiotisation through the investigation of a controversial comment made by South African president, Jacob Zuma. The comment (that Jacob Zuma took a shower to minimise his risk of contracting HIV/AIDS) was made during a rape trial in 2006. Over a period of eight years, this comment has been continually recontextualised into different modes, and has served different functions. This article investigates newspaper reports, cartoons and YouTube clips which were collected from 2006 to 2014. All of these texts refer to the “Shower comment” in linguistic, visual or multimodal ways. Drawing on a multimodal analysis, focussing specifically on modes of representation, the article critically discusses the notions of mode, time and space in connection to resemiotisation. I argue that a Bakhtinian account of time, space and body allows for a non-binary view of semiotic phenomena that will lead to a deeper theoretical understanding of semiotic transformations in all their complexity.  相似文献
