Brain hydration was determined in death of strangulation mechanical asphyxia, acute alcoholic poisoning, acute poisoning with opiates, cancer of stage IV, coronary heart disease with acute heart failure, hemorrhagic stroke. Brainstem edema (BSE) occurred in 69.2% cases (upper BSE - 20.2%, lower BSE - 17.3%, BSE of the all parts - 31.7%). BSE was present in all cases of opiate poisoning; 52.9% cases of hemorrhagic strokes, 61.3% cases of coronary heart disease. The data obtained show prevalence of brain tanatogenesis in death of the above diseases. Hypohydration and hyperhydration of the brain were observed in short and slow agony, respectively.  相似文献

3130 cases of medicolegal postmortem in Shantou City between January 1977 and December 1996 were analyzed statistically. The results revealed that the ratio of male to female was 2.3 and most dead were young and middle age. The number of postmortem has increased in resent years. Of 3130 cases, 2829 (92.3%) were caused by mechanical injury, mechanical asphyxia, sudden death and poisoning. As to manners of the death, 1226 were related to accidents (39.2%), 776 homicide(29.8%), 658 suicide (21.0%). These results were compared with other studies reported in domestic and international literature.  相似文献

目的观察不同死亡原因大鼠尸体血液中红细胞溶血速度的变化规律,为死亡原因的法医学推断提供新思路。方法 40只SD大鼠随机分为4组,分别以断髓、置于99%CO的空间、高坠、勒颈方式处死后,取各组大鼠右心室血液,于死后即刻(0h)、8h、16h、24h、32h、40h、48h、56h、64h、72h,采用显微镜数码图像法进行全血红细胞计数(CBC),并对组内和组间数据进行统计学比较分析。结果 4组血液红细胞数量在死后即刻至72h期间,均随时间的延长因溶血而减少。其中0~16h,各组溶血速度无明显差异(P〉0.05);16~48h,速度加快,溶血速度以CO中毒组(88.50±25.99)%最快,其次为高坠(69.33±29.52)%和断髓组(48.78±3.17)%,机械性窒息组(41.90±9.61)%最慢,组间比较具有显著性差异(P〈0.05);56h后,溶血速度再次减慢,至72h机械性窒息组仍有少量红细胞存在。结论 4组不同死因大鼠死后不同时间红细胞计数均减少,各组间差异具有统计学意义,其变化特征可为死亡原因的推断提供新的思路。  相似文献

对汕头市1977年1月至1996年12月20年间3130例法医尸检资料进行统计分析,探讨其性别、年龄、季节、死因和案情性质构成,并与国内外文献比较。结果表明男性为女性的2.3倍;青壮年居多;尸检数逐年增加;损伤、窒息、粹死、中毒占2829例(92.3%),意外1226例(39.2%),他杀776例(29.8%),自杀658例(21.0%)。与广州、湖北部分地区资料相似,与日本东京、美国佛罗里达资料存在明显差异。  相似文献

Forensic medical diagnosis of death from coronary heart disease, acute ethanol poisoning, alcoholic cardiomyopathy, closed cardiac injuries, mechanical injuries incompatible with life which may be directly caused by acute cardiac failure, requires identification and evaluation of diagnostic complexes of acute myocardial changes. The diagnostic significance of such complexes of myocardial changes is characterized for the first time. 在本研究的13个案例中,怀疑原发性脑干伤死亡的5例,明确死因的8例,其中严重闭合性颅脑损伤2例;缢死1例,失血性休克1例,电击2例,海洛因中毒2例。用LSAB法对13例脑干的中脑、脑桥、延髓各部FN进行检测。结果显示5例怀疑为原发性脑干伤、2例严重颅脑损伤、2例海洛因中毒死者的脑千部位的神经元内均可见FN的异常沉积,其它各例未见FN的异常沉积。结果表明;LSAB-FN法是诊断原发性脑干伤早期死亡灵敏但非特异的形态学指标。  相似文献

限制性体位窒息是一种特殊类型的窒息,其死亡机理、过程复杂,体表损伤轻微,尸体表现缺乏特异性,鉴定难度大,目前还没有一个客观、准确、公认的鉴定标准,通过文献复习总结有关体位性窒息的研究成果,结合窒息死亡的组织病理改变和鉴定实践,提出限制性体位窒息的检验鉴定要点,在确定有长时间限定在某一影响呼吸的体位,且自己不能解脱;有明显的窒息尸体征象;排除损伤、疾病致死;常见毒物检测阴性;膈肌Fn免疫荧光检测阳性或透射电镜检查证明有膈肌损伤的可以诊断。如果合并有损伤或疾病,还应该有肺SP-A检查阳性或HIF1-α免疫组化染色核阳性表达。某些特定部位的损伤检查有助于分析体位关系。  相似文献

Crystallographic analysis of the cerebrospinal fluid (CSF) was carried out in 18 cases of death from coronary disease and 19 cases of death from ethanol poisoning. 目的 探讨火场尸体的法医学检验特点及其鉴定。方法 对86起火场中152例尸体的相关检验资料进行回顾性研究。结果 152例火场尸体,生前烧死109例,纵火焚尸38例,无明显高温作用的尸体5例。主要死因有“烧死综合征”100例,火场有毒气体中毒8例,机械性损伤32例(含高坠死4例),机械性窒息5例,电击、服毒各1例;不能确定死因5例。自杀5例,他杀40例,意外107例。在生前烧死尸体,87例呼吸道内有炭末沉着,71例皮肤局部烧伤边缘组织有红肿,46例检见水泡,17例有“闭眼反应”征象,大部分尸体血中HbCO浓度超过20%;纵火焚尸少见或不见上述改变。结论 检验火场尸体,根据烧死尸体征象和血中HbCO浓度检测,并结合火场勘验资料综合分析,其死因鉴定和死亡方式推断结论方能准确。  相似文献

目的探讨肥大细胞类胰蛋白酶、脑利钠肽(brain natriuretic peptide,BNP)在过敏性猝死和冠心病猝死鉴别诊断中的意义。方法选取山西医科大学法医病理学教研室2010—2015年尸检案例心肌标本共30例,分为颅脑损伤致死组、过敏性猝死组、冠心病猝死组,每组各10例。采用免疫荧光染色和Western印迹法分析各组心肌组织肥大细胞类胰蛋白酶和BNP的表达。结果过敏性猝死组、冠心病猝死组心肌组织内肥大细胞类胰蛋白酶免疫荧光染色均出现阳性染色;三组间两两比较,表达差异均具有统计学意义(P0.05)。冠心病猝死组心肌组织内BNP的表达量高于过敏性猝死组、颅脑损伤致死组(P0.05),过敏性猝死组与颅脑损伤致死组之间差异无统计学意义(P0.05)。结论联合检测心肌组织内肥大细胞类胰蛋白酶、BNP有望为过敏性猝死和冠心病猝死的法医学鉴别诊断提供帮助。  相似文献

To reassess the immunohistochemical distribution of pulmonary surfactant apoprotein A (SP-A) in relation to the causes of death, 282 forensic autopsy cases were reviewed. The most intense and dense granular immunostaining of intra-alveolar SP-A was observed in the hyaline membrane syndrome from various traumas, protracted death from drowning, and perinatal aspiration of amniotic fluid. Similar granular staining pattern was found in fatal poisoning by a muscle relaxant and organophosphate pesticides. An evident increase of intra-alveolar granular staining was noted in most fatalities from mechanical asphyxia and drowning, and some cases of fire death. SP-A staining was usually very weak or sparse in alcohol intoxication, poisoning by hypnotics and also carbon monoxide poisoning. These findings suggest that the amount of intra-alveolar granular SP-A staining may be a possible indicator of severity and duration of respiratory distress (agony) from peripheral (non-central nervous system) origin and alveolar damage.  相似文献

Catecholamines are major humoral factors and neurotransmitters that contribute to various stress responses. However, they have been considered unstable due to agony, terminal medical care and postmortem interference. The present study was a comprehensive investigation of postmortem serum levels of adrenaline (Adr), noradrenaline (Nad) and dopamine (DA) with regard to the cause of death in serial medicolegal autopsy cases (n=542) including fatalities from various traumas and diseases. There was a slight tendency toward postmortem increases of Nad and DA in cardiac blood as well as Adr and Nad in peripheral blood, a slight age-dependent decrease in Adr and DA in right heart blood, and a marked increase in serum DA due to administration during critical medical care. When these factors were taken into consideration, significantly higher cardiac blood levels were observed for Adr and Nad in injury and asphyxiation cases and for Adr in fatal methamphetamine (MA) abuse and other poisoning cases, whereas those levels were lower in fatal hypothermia. Drowning, fire fatality, acute cardiac death and cerebrovascular disease showed intermediate Adr and Nad levels. The DA level was elevated in cases of injury, hyperthermia, MA fatality and other poisoning. Topographical analyses suggested that the major sources of increased serum catecholamines in cases of injury was abdominal viscera including adrenal glands, and that in cases of asphyxiation, drowning, fire fatality, hyperthermia, MA fatality, other poisoning, acute cardiac death and cerebrovascular disease was the extremities in addition to abdominal viscera. However, there was in part a large case-to-case difference in each marker related to individual causes of death. These findings differed markedly from clinical observations and suggest that the postmortem serum catecholamine levels may reflect the magnitude of physical stress responses during the process of death in individual cases.  相似文献

This paper aims at updating terminology employed for the characteristic of selected forms of mechanical asphyxia. The medical terms "obturation", "aspiration", and "inhalation" are not infrequently used by forensic medical experts and pathological anatomists engaged in diagnostics of mechanical asphyxia, elucidation of the mechanism of the accompanying injury and causes underlying its fatal outcome. It is argued that the use of these terms for the purpose is sometimes either unjustified or incorrect or both. To begin with, they have different meanings in different situations. Second, the mechanism of death from mechanical asphyxia is variable and associated with specific morphological features that are directly dependent not only on the route by which a foreign body enters the respiratory tract but also on its size, weight, structure, shape, properties, and aggregated state. Third, it is necessary to differentiate between inhalation of a toxicant in case of poisoning and inhalation of a gaseous substance leading to mechanical asphyxia.  相似文献

Over 30 000 cases of acute poisoning with ethyl alcohol and its surrogates are recorded annually in this country. Differential diagnostics between fatal poisoning and death from coronary heart disease encounters serious difficulties. The authors report a comprehensive forensic chemical, morphometric, and pathomorphological study of the activity of ethanol-oxidizing enzyme systems in the internal organs. The results of histochemical examination provide a basis for the extension of diagnostic potential of the available methods and the enhancement of the objective value of expert reports.  相似文献

Pathologists frequently examine victims of sudden cardiac death. In some cases, a firm diagnosis of cardiac-related death can be made based on conclusive gross and histologic findings. In many other cases, we find evidence supportive of, but not diagnostic of, cardiac death (e.g., atherosclerotic coronary artery disease, cardiomegaly, myocardial scarring). A final cohort consists of cases of sudden death with minimal to mild cardiac disease, no other significant pathology, and negative toxicologic studies. This prospective study compared 38 cardiac-related deaths with 52 control cases with respect to concentrations of pericardial cardiac troponin I (cTnI), heart weight, evidence of old and/or recent myocardial injury, and presence of significant coronary artery disease. The influence of documented chest trauma and/or perimortem cardiopulmonary resuscitation (CPR) on levels of cTnI was also analyzed. Even though median cTnI levels were significantly higher in cardiac deaths than in controls (p = .003), cTnI was not found to be a significant predictor of cardiac deaths, as determined by discriminant analysis (p = .52). Heart weight >500 g, evidence of old and recent myocardial injury, and significant coronary artery disease were seen statistically more often in cardiac deaths than in controls (p < or = .005 in each case), and median age was significantly higher in cardiac deaths than in controls (p = .001). Based on a stepwise logistic regression model, significant coronary artery disease, old and recent myocardial injury, and heart weight >500 g were found to contribute significantly to the prediction of cardiac death. Finally, neither chest injury nor CPR significantly affected concentrations of cTnI in pericardial fluid. These data confirm that the presence of acute and remote myocardial injury, significant coronary artery disease, and cardiomegaly (heart weight >500 g) strongly supports the diagnosis of a cardiac-related death. In contrast to a recently published report, we do not find that elevated concentrations of cTnI in pericardial fluid are strong indicators of cardiac-related deaths using our methodology.  相似文献

Two cases of self-inflicted injury, associated with suicide by ingestion of barbiturates, are presented. The head injury was so severe in one of the deceased that the death was at first thought to be due to homicide. 目的检测cathepsin-L(CTSL)在大鼠早期缺血心肌中的表达变化,并探讨其法医学意义。方法建立大鼠早期心肌缺血模型,设置早期缺血组(early ischemic myocardium,EIM)、非缺血组(non-ischemic myocardium,NIM)、假手术组以及空白对照组,采用real-time PCR方法检测大鼠缺血后15min,30min,1h和2h CTSL mRNA的表达量,并进行组内和组间比较。收集心源性猝死者缺血心肌和机械性损伤致死者正常心肌样本,采用免疫组化染色方法观察心肌组织中CTSL蛋白表达。结果 EIM组CTSL mRNA表达量与NIM组、假手术组及空白组相比,在15min时无统计学意义(P>0.05),在30min、1h和2h时分别升高1.4、3.1和4.5倍(P<0.05);其余3组间差异无统计学意义(P>0.05)。观察10例冠脉狭窄程度Ⅲ~Ⅳ级的心源性猝死(SCD)者缺血心肌,CTSL表达增强。结论大鼠心肌缺血后早期即可检测到CTSL mRNA的升高,并在SCD冠脉狭窄死者缺血心肌中高表达,提示CTSL可作为心肌缺血与SCD的参考指标。  相似文献

Unprecedented poisoning terrorism by use of sarin took place in Japan in 1994 and 1995. On July 25, 1998, a curry poisoning incident in Wakayama occurred resulting in the death of four people and injury of 63 people. Since then, more than 30 imitative poisoning cases have been reported by mass communication within 1 year. In this brief review, we present some details of a series of poisoning cases and measures taken by the Japanese Government and non-government groups. In addition some research projects on analytical toxicology being conducted in our laboratories are also introduced; gas chromatography (GC) with cryogenic oven trapping for volatile organic compounds, and GC/surface ionization organic mass spectrometry for tertiary amino compounds are described especially as new techniques for biological samples.  相似文献
