The purpose of this study was to examine the relationship between neonatal behavior and prenatal and perinatal risk factors in infants of teenage and older mothers in Puerto Rico and Mainland United States. The sample included approximately 300 newborn infants;half were examined in Puerto Rico, the other half in Florida, using the Brazelton Neonatal Behavioral Assessment Scale. Comparisons by maternal age and the number of obstetrical complications showed that for the Puerto Rican group, infants with fewer obstetrical complications were better able to regulate their crying and active states. Infants of teenage mothers with fewer complications had a higher level of arousal than infants of older mothers with fewer complications. Infant behavior in the Mainland sample did not vary by maternal age or complications. Multiple regression analysis indicated that the combination of biomedical variables significantly predicted neonatal behavior in both cultures. Mother's age was not separately correlated with neonatal behavior, but was repeatedly combined with other variables in the significant regressions. The findings suggest that infants of teenage mothers may differ from infants of older mothers, particularly in the organization of state behavior, and that the effects of maternal age on neonatal behavior are increased in the presence of biological outcome factors.This article is based on a final report to the NICHD Center for Population Research, Contract N01-HD-7283.Received Ph.D. from Michigan State University. Early puberty is associated with stressful family environments, early sexual intercourse, and teenage pregnancy. We examined pubertal timing and sexual debut among the 14-year-old offspring of teenage mothers. Mothers (71% Black, 29% White) were recruited as pregnant teenagers (12–18 years old). Data were collected during pregnancy and when offspring were 6, 10 and 14 years old (n = 318). Adolescents (50% male) compared the timing of their pubertal maturation to same-sex peers. There was a significant 3-way interaction effect of race, sex, and pubertal timing on sexual debut (n = 305). This effect remained significant in a model controlling for maternal age at first intercourse, substance use, exposure to trauma, authoritative parenting, and peer sexual activity (n = 255). Early maturation was associated with early sex in daughters, and may be one pathway for the inter-generational transfer of risk for teenage pregnancy among daughters of teenage mothers.  相似文献

The program assesses parenting capabilities and help the mother make an early and informed decision regarding permanency for her baby. This project assessed the influence of religiosity and spirituality on the socioemotional and behavioral adjustment of 110 adolescent mothers and their teenage offspring at age 14. Maternal religiosity, measured prenatally and when children were 3, 5, and 8 years of age, was defined as involvement in church as well as contact with and dependence on church officials and members. Levels of spirituality, defined as religious practices and beliefs, were assessed for both mothers and their children at 14 years postpartum. Hierarchical regression analyses suggested that maternal religiosity was a strong predictor of maternal and child adjustment; children’s own spirituality served as a predictor of their socioemotional adjustment as well. Furthermore, child spirituality mediated the relationship between maternal religiosity and children’s externalizing behavior. Implications for designing intervention programs with high risk families are discussed.  相似文献

Sixteen white intact families with unwed teenage daughters completed Olson, Portner, and Bell's Family Adaptability and Cohesion Scales III of 1985 and Cervera's Family Alliance Questionnaire of 1989 at the sixth and eight months of the pregnancy and one month after the baby's birth. In the first interview, subjects also responded retrospectively to the questionnaires. The results suggest that on average the families became more cohesive and had improved family functioning over the pregnancy. However, individual families showed different patterns. Discernible trends in the largely inconclusive survey-oriented literature on adolescent pregnancy and abortion suggest that demographic idiosyncracies or neurotic predispositions are the significant correlates of illegitimacy. An in-depth study of a homogeneous population of 13 unwed pregnant adolescents suggests a strong developmental determinant. The experience of pregnancy and abortion is heavily determined by the stage-specific conflicts of early, middle, and late adolescence. From an analysis of interview and projective materials, three clinically and statistically significant patterns of the experience and motivation for pregnancy emerge, one for each of the three adolescent substages. It is suggested that similar developmental paradigms could be applied to a wide range of adolescent issues.This paper is based on a dissertation written in 1972 at the University of Michigan.Received Ph.D. from the University of Michigan. Using data front 2522 young men who were first surveyed as 7th-grade students in Houston, Texas in 1971, we examined the psychological consequences in early adulthood of having a girlfriend become pregnant in adolescence. By age 21, 15% of the young men were involved in a nonmarital pregnancy. Rates were higher for blacks (24%) than for whites (12%) or Hispanics (16%). Among whites, most adolescent pregnancies were ended by abortion (58%). Adolescent pregnancies to blacks most often resulted in single parenthood (56%). Hispanics tended to have the child, and marry or live together (55%). Consistent with the life course perspective, young men involved in adolescent pregnancies were more psychologically distressed as young adults than those who did not have a girlfriend become pregnant in adolescence. The greater distress in adulthood is not simply a function of accelerated role transitions, because men whose girlfriends had abortions are also distressed, and those who let their girlfriends assume major parenting responsibility are no less distressed than those who became fathers and married or lived with their girlfriends. Subgroup comparisons revealed that psychological distress levels of young black men were not influenced by adolescent pregnancy.This paper was presented at the North Central Sociological Association meetings, Pittsburgh, Pennsylvania, April 1988, and reflects equal authorship. The assumed etiology of adolescent illegitimate pregnancy is reviewed, and seven central concepts are indicated. The prognostic value of these, concepts, expressed as antecedent attitudes, is evaluated by secondary analysis of data drawn from a lengthy, survey of seventh graders. A community-based case-control study to determine the risk factors associated with adolescent pregnancy was conducted in the rural community of Gbongan in southwestern Nigeria. One hundred and thirty-two pregnant girls aged 20 years or less were compared with 131 nonpregnant girls of similar age. Information on their sociodemographic characteristics and those of their parents, their knowledge of reproductive health, and practice of contraception were obtained by household confidential interviews and by focus group discussions with parents and adolescents. Univariate analyses revealed a large number of potential risk factors, but after adjustment by logistic regression, only two factors remained significantly associated with pregnancy. These were being married (OR=9.8) and doing an income job as opposed to being an apprentice (OR=4.7). In turn, doing an income job was the only significant factor that predicted marriage in the logistic regression model (OR=1.5). Both pregnant and nonpregnant adolescents had poor knowledge of, and negative attitude toward, contraception, and only a small percentage of them had ever used contraceptives. We conclude that pregnancy among adolescents in this community is mostly associated with completion of formal education at an early age by the girls and to their lack of knowledge of reproductive health. Measures that could reduce the high rate of pregnancies among adolescents in rural Nigeria include (1) a program to encourage adolescents to continue formal and informal education, (2) reproductive health education and services for adolescents, and (3) appropriate legislation to discourage early marriage and pregnancy in the community.This paper was written during the author's one year leave at the Harvard School of Public Health as a Takemi Fellow in International Health with funds provided by the Carnegie Corporation.  相似文献

Previous research suggests that maternal smoking during pregnancy remains prevalent, particularly among adolescents. However, little is known about the factors related to smoking during adolescent pregnancy. The goal of the present study is to identify intrapersonal, familial, and peer factors that are related to smoking during adolescent pregnancy, and to determine the relative degree to which they affect this behavior. Interviews were conducted with 241 unmarried pregnant adolescents who planned to carry their pregnancies to term. Consistent with previous studies, 27% of the respondents reported daily smoking during pregnancy, and whites reported higher rates of use than members of other racial groups. Smoking during adolescent pregnancy was related to intrapersonal, familial, and peer factors. The results of a regression analysis suggest that perceived parental disapproval of smoking during pregnancy, friends' cigarette use, and race play a particularly important role in this behavior. This paper examines the association between adolescent pregnancy and socioenvironmental, physical, and mental health problems in 1590 inner-city females aged 13–18 who use health clinics. Adolescents who have become pregnant, those who are sexually active but never have been pregnant, and those who are sexually inactive are compared. The sexually active youngsters come from more psychosocially disadvantaged backgrounds than their sexually inactive peers; the sexually active girls who become pregnant come from more psychosocially disadvantaged backgrounds than those who have never been pregnant. Despite this, the youths who have become pregnant do not have more current relationship problems, more stressful life events, or worse physical health than the never-pregnant sexually active youths. Although sexually inactive youths have the lowest rates of mental health problems, adolescents who have been pregnant have lower rates of anxiety and conduct disorder symptoms than those who are sexually active but never pregnant.Supported by the Robert Wood Johnson Foundation. Perception of pregnancy risk, fertility knowledge, and probability-based teaching examples of risk were assessed in 104 primiparous urban adolescents 13–18 years of age in their second and third trimester of pregnancy. Perception of risk was not associated with age, actual frequency of intercourse, or level of fertility knowledge. Sexually active adolescents were surprised at subsequent conceptions.
