The paper examines the conception, implementation and abolition of the GP fundholding scheme, all within 10 years, for evidence of a changed style of health care policy-making. A health care policy community, in which the interests of the medical profession were prominent, existed between 1948 and the mid-1980s. The paper highlights the important factors in the breakdown of the policy community and traces the effect through to the negotiation, implementation and abolition of the GP fundholding scheme. In particular, the role of evidence in health care policy-making has changed significantly. A consequence of the collapse of the policy community has been that a 'folk psychology' rather than evaluative evidence has guided some aspects of health care policy since the 1990s.  相似文献

The South Korean intergovernmental environment has undergone drastic changes after the reintroduction of local autonomy in 1995. This study investigates how and why the governance has changed in terms of its structure and processes after devolution using the case of dam politics in South Korea. It also addresses the accountability implications of the changed governance. This study analyses two cases of dam construction initiative, which are the Youngwol dam project (1990–2000) and the Hantangang dam project (1998–2004). The analysis of these cases reveals that the governance structure concerning water resource supply has changed from a form of policy community, which can be characterized by its closed membership and stability in relationships, to a form of issue network, where the membership is enlarged to include various participants. More specifically, there are three important findings. First, the cancellation of the Youngwol dam project served as the moment to establish and empower a network of opposition against dam building, which had long been advocated and led by the policy community comprising the Ministry of Construction and Transportation, Korea Water Resources Corporation, engineering experts, and some private companies related to dam construction. Second, as the central government plans to build 12 more dams including Hantangang dam, the network of opposition tends to be further vitalized with heated participation from local activists, some water resource management experts, and environmental activists. Third, as the pro-dam policy community and the network of opposition initiate dialogue among themselves, they are forming an issue network which works as a governance mechanism in the area of water resource supply management.  相似文献

This article explores the contention that the conventions of public expenditure accounting conceal, rather than reveal, the real nature and implications of resource trends. A benchmark — a constant level of service output — is established as a basis for examining the relationship between trends in expenditure inputs and service outputs. Changes in unit costs are identified as the major source of deviation between expenditure and output trends. The impact of resource constraint on policy and policy-implementation is then examined in relation to one, essentially stable, area of policy in the personal social services: community care.
The meaning of the term'policy' is far from straightforward and community care is best understood as the interaction of relatively independent streams of policy, towards service outputs and resource inputs, extant in both central and local government. Mechanisms by which policy streams could be reconciled are of particular interest and an innovative example — joint finance — is examined in some detail.  相似文献

Studies of policy-making and management in health and related fields have generally been confined to England even when professing to embrace Britain or the United Kingdom. The assumption of a unitary state in which the policy lead is given by Westminster/Whitehall is shown to be grossly misleading in the case of community care policy. Considerable variations exist not only in policy means but also in policy ends. The paper demonstrates the limitations of studying health and personal social services adopting a unitary state perspective. It argues for more intra-Britain comparative work aimed at evaluating the costs and benefits of service variations in different policy fields.  相似文献

Abstract

The Labour government in England is pressing for further individual consumer choice in local authority services and, in the name of localism, exploring possibilities for devolution of powers to councils. Little work has been done on how the two agendas might interact. Are they in inevitable tension, because choice promotes exit and devolution promotes voice? Might devolution be offered as a reward for good performance in choice? What might be the consequences for equity? This article argues that the more significant interaction issues are likely to arise from the dynamics created by their combined effects on incentives on households at the margin to relocate. The article offers conceptual and deductive analysis, because before we know the design of devolution and choice schemes we cannot measure the size of the interaction effects. Although major policy tension is not inevitable, the article concludes that policy makers may have to decide how much gains in voice and from choice are worth the possible losses in equity.  相似文献

Housing and living environment options are both one of the most pressing problem are as for the elderly and an unavoidable policy subject for public administrators. This article analyzes the topic of housing and the elderly so as to high light selected dimensions that might have implications for decision - makers in resource considerations and allocations. An opening section gives the status of how and where the elderly are now living. Six events, or environmental and policy changes, that affect the housing options of the elderly are analyzed. These events are: high mortgage rates; rent controls; condominium conversions; tax relief measures; weatherization programs; and reverse annuity mortgages. The two dominant trends that appear to shape the future housing options of the elderly are examined. The first trend is the provision of a range of alternatives in living arrangements. The creation of alternatives, also known as a continuum of living environments, includes congregate housing, share - a - home, life care or continuing care, geriatric foster care, and single room occupancy hotels. The second major trend is the increasing provision of services in the home setting. The final portion draws together the evident implications about the housing problems of the elderly for policy makers in the public management field. Implications that have community-wide ramifications are clustered as they relate to: increasing the housing supply; regulatory mechanisms; and budgetary references.  相似文献

This article examines, through a case study of housing, the extent to which national housing policies under the Thatcher government were modified or deflected at the N. Ireland regional level. Three aspects of housing policy - housing expenditure, public sector rents and the 'right-to-buy' legislation are considered and the roles of key actors within the territorial community described. Although similarities with GB housing policy emerge, there is evidence of a distinctive regional approach. Factors which influenced a N. Ireland-specific policy approach are considered and the potential for variance in the future outlined.  相似文献

Through their ambitious devolution programme, the Conservatives show a new assertiveness in relation to restructuring the local state, in contrast to the laissez-faire approach of the previous Coalition government. Although the Coalition piloted devolution, its signature policies of ‘localism’ and the ‘Big Society’ focused on non-state actors, providing rights and opportunities for communities to challenge local government and establish their own services. The Conservatives are promoting devolution as a strategy to stimulate economic growth based on greater sub-regional autonomy and increased competitiveness across and between English localities. ‘Combined authorities’ have the opportunity to champion local identities and acquire new economic development powers from Whitehall. But devolution could be a strategy to decentralise austerity, shifting responsibility to the local level for deeper cuts (56% by 2020) and inevitable service reductions. Local government confronts ‘super-austerity’, where new cuts come on top of previous ones, compounding original impacts and creating dangerous (and unevenly spread) multiplier effects. The Conservatives’ ‘smarter state’ policies, aimed at delivering ‘more for less’, amount to little more than a recycling of new public management diktats. The emerging patchwork of ‘devolution deals’ challenges the redistributive assumptions of the grant regime and could leave disadvantaged areas at particular risk of failure. ‘Metro mayors’ are intended to provide visible and accountable leadership; but roles for locally elected councillors, and prospects for community and citizen engagement, remain unclear. The public has yet to be adequately engaged in what is in danger of becoming a technocratic transfer of power.  相似文献

The devolution of authority from central to regional and local governments is a widespread trend in many countries. Differences in the outcomes of devolution reforms are often significant, between countries as well as within a country. The work reported in this paper assumes that the dynamics of the implementation process and the way it is affected by the national tradition of governance and by the features of the politico‐administrative system is important in explaining such differentiation. The paper investigates devolution in Italy and proposes explanations for the substantial differentiation of outcomes that can be observed. The case of devolution in agriculture in Lombardy, investigated in depth in the article, is striking for the magnitude and rapidity of change as well as for the way the reallocation of the workforce to the lower levels of government has occurred. This case study provides the basis for some theorizations about the dynamics of devolution processes in countries that have a legalistic administrative tradition, especially those nations which have a ‘Napoleonic’ administrative tradition.  相似文献

Critiques of policy networks have highlighted particularly the inability of concepts such as policy communities to explain policy change. The established construction of policy community places it chiefly as a metaphor for a relatively stable network within the policy process, which emphasizes the resource dependencies between key stakeholders. Typically, a process of bargaining brings about accommodation and a state of negotiated order.
However, a key problem arises in explaining major policy change where an established policy community persists. One solution here is to appreciate that, over time, dominant ideas and associated policy meanings may shift appreciably within an otherwise durable policy community. Thus, even a seemingly insulated policy community, under certain conditions, may not be immune to idea mutation and new policy meanings. Given the central importance of policy communities, these shifts may induce significant policy change.
A case study of this type is provided by the Oxford Transport Strategy (OTS), where a dual process of change took place. On one level of analysis, a challenge to the policy community produced a typical bargaining strategy, with an emphasis on negotiated order. On another level of analysis, however, the terms of the policy debate shifted markedly, and produced a new meaning for the key concept of integrated transport within the policy community. In turn, this process induced significant policy change. The article concludes that, ironically, the survival of a policy community depends on its ability to re-create itself by visualizing a new future.  相似文献

This article examines the impact of international conflicts involving the Muslim world upon Australia's multicultural society by studying the Howard government's responses to tensions between Islam and the West since September 11. Specifically, it surveys the Howard government's participation in the ‘War on Terror’, other aspects of Australia's foreign and security policies, including the relationship with the United States, the emergence of immigration and refugee flows as national security issues, and the subsequent impact of all this on multiculturalism and in particular on Australia's Muslim community. At a broader level, the article challenges the argument that foreign policy is inconsequential for social policy, intercommunal, inter-ethnic and inter-religious relations. The internalization of the ‘War on Terror’ has reinforced a negative portrayal of Islam and Muslims in Australia by blurring the dividing line between ‘Islam’ on the one hand and ‘extremism’ and ‘terrorism’ on the other. The evidence suggests that government statements and policies, more generally had provoked apprehension amongst a wide cross-section of the Australian Muslim community. A major finding of this article is that unless care is taken, it is not inconceivable that the hitherto harmonious relationship between Muslims and other sections of Australia's community could be unnecessarily endangered.  相似文献

Abstract

In the Kenyan context of new resource discoveries and an ambitious devolution programme, and what is argued to be a shrinking of civic space globally, the role of civil society organisations (CSOs) working on natural resource governance is critical. The resilience, space and capacity of civil society to engage in the policy process, from community-based organisations to national non-governmental organisations, all shape outcomes in terms of legislation, policy and management of scarce resources. Drawing on interviews with CSOs from across Kenya, following the new 2010 constitution and devolution programme, this article explores how new negotiated spaces of participation around resource governance have emerged in Kenya. Using multidimensional frameworks to analyse power relations, it explores how Kenyan CSOs are cautiously redefining roles, offering expertise when devolved governments struggle, and standing up to powerful interests of corporate lobbies with varying degrees of success.  相似文献

Inspired by New Public Management, many countries have changed their central public apparatus from an integrated to a more segregated structural model. A central element in this process is structural devolution and the establishment of new or reorganized state-owned companies with increased business autonomy and new formal control systems. This paper focuses on how this development, as exemplified by the case of Norway, is affecting the role of central executive political and administrative leaders. The study, based on elite interviews, shows that corporatization has made the role of central leaders more complex and ambiguous and undermined traditional political control. We interpret this development from a transformative perspective, underlining how structural devolution is filtered through the dynamic context of environmental pressure and internal structural and cultural factors; in addition, experiences from New Zealand are used to contrast the Norwegian case.  相似文献

The last two decades have witnessed a significant turn towards community participation in public policy around the globe, raising concerns that states are resorting to ‘government through community’, shifting responsibilities onto communities. In order to unpack the ambiguous rhetoric of policy statements, this article employs ideas from evaluation methodology to develop a generic theory of change for community participation policy. The model is then utilised to analyse and compare the UK Coalition Government’s Big Society/Localism agenda and the Scottish Government’s Community Empowerment approach, demonstrating the ways in which these represent a clear example of policy divergence, and potentially significant alternatives to state–community relations in the context of austerity. The article also demonstrates the potential wider applicability of ‘theories of change’ methodology for policy analysis.  相似文献

Recently, some European social scientists have claimed that the old legal-bureau-cratic model of administration has been replaced by a new paradigm in public administration, characterized by a strong emphasis on collaboration in local inter-organizational networks. The proponents of this policy network approach take a clearly voluntaristic view on policy implementation; network actors build consensus in negotiation processes, and the role of central government is restricted to that of goal-setter, facilitator and mediator. Thereby, phenomena like power and steering are overlooked. This paper gives an account of a major Swedish reform in the area of old-age care, whereby the boundaries between regional and local areas of responsibility for care of the elderly were displaced. By using a variety of control methods, central government was able to structure and steer the old-age implementation networks. The consequences of this central steering were different on different administrative levels: for the county councils, the reform has resulted in a specialization for the core areas of primary health care and hospital treatment, whereas the municipalities have had to diversify their areas of activity. Thus, to understand the effects of the reform, implementation networks must be viewed as both hierarchical and horizontal power structures, where national government, from a hierarchically superior position, can affect formally horizontal relations between actors by creating patterns of interdependence. Central government's steering has taken on less direct forms than the traditional ones, but indirect forms of steering can certainly be efficient, especially when several mutually reinforcing control methods are combined.  相似文献

For several years, local governments have been tackling new challenges related to the regeneration of their cities within the physical sphere, as well as in their economic and social aspects. This task is developed in accordance with what has been called the Integrated Urban Development Model, understanding the integrated nature of this approach not only in terms of the spheres of public policy involved, but also in relation to their management. In this paper, this management is analysed in terms of the development of mechanisms for New Local Governance and New Public Management (NPM). Using Spanish municipalities as a case study, this paper develops an analytical tool for the analysis of this model of urban intervention within the European context, paying close attention to the development of mechanisms for New Local Governance from a multilevel perspective, as well as the development of mechanisms for NPM.  相似文献

The article presents the crisis intervention model devised by Andreoli (Geneva) which is currently being developed in most crisis units and emergency services in the French-speaking countries of Europe. Two clinical examples are presented: the Short Therapy Centre (Geneva, Switzerland) and the crisis unit of the Saint-Luc Clinic (Brussels, Belgium). The following aspects of these approaches are discussed: (a) the need for crisis intervention rather than a simple answer to emergency, (b) the need for crisis intervention in all acute psychiatric disorders and not only in psychosocial problems, (c) the need to integrate psychiatric hospitalization into a coherent mental health policy, (d) the need for well-trained and round-the-clock teams, (e) the need for continuity of care.  相似文献

Policy networks are advanced as an alternative to the Westminster model of the UK polity but the theory lacks an internal dynamic and has typological problems. This article applies Peter Hall's (1993) concept of 'social learning' to policy networks and maps the networks found in two case studies of British economic policy making: Hall's own study of the shift from Keynesianism to monetarism in the 1970s and the author's research on the advent of 'Keynesian-plus' in the early 1960s. The article advances three main propositions. Firstly, that integrating the concept of social learning can dynamize the policy network model. Secondly, the case studies suggest that different network configurations are associated with different orders of policy change but that Hall's definition of 'third order change' may be too restrictive. Thirdly, policy networks can be much more complex and fluid then is generally claimed, sometimes becoming so extensive that they might be termed a 'meta-network'.  相似文献

This article examines the concept of Eurasianism in relation to Russia's East Asian policy from Yeltsin to Putin. It argues that there are three main interpretations of Russia's Eurasianist identity in the foreign policy discourse regarding East Asia: Pragmatic Eurasianism, Neo-Eurasianism, and Intercivilisational Eurasianism. Each interpretation emphasises a different aspect of Eurasianism with different policy implications. However, they all share an instrumentalist nature, of being used to justify Russia's Great Power status and a greater role for it in East Asia. Moreover, it is the pragmatic and geoeconomic aspects of Russia's Eurasianist identity that are being stressed most by the Putin administration, especially on energy and transport links.  相似文献
