Medical decision-making for incapacitated elders: A "therapeutic interests" standard

Some older individuals lack sufficient present cognitive and/or emotional ability to make and express autonomous decisions personally. In those situations, health-care providers routinely turn to available formal or informal surrogates who often must apply the best interests standard in making decisions for the incapacitated person. This article contends that defining the best interests standard of surrogate decision-making for older adults in terms of optimal or ideal choices (truly the patient's "best" interests) frequently sets out an unrealizable goal for surrogates to satisfy. Instead, a decision-making standard based on the incapacitated person's "therapeutic" interests is more realistic and hence more honest to adopt and apply from legal, ethical, and medical perspectives.     

Building from the ground up: exploring forensic mental health staff’s relationships with patients

Recovery-orientated care is becoming generally accepted as the best practice, and continued development in the ways it is practiced is necessary to ensure improvement of ongoing care. Forensic patients often experience double stigmatization (the dual stigma of mental illness and offending behaviour) and during admission to hospital may lose touch with their community supports. While working through their personal recovery, patients develop therapeutic relationships with their multidisciplinary team members. When positive, these relationships can enhance a patient’s recovery. Clinical staff members participated in 88 in-depth interviews, which were transcribed, reviewed, and analysed using thematic analysis. From analysing the data, main themes and subthemes emerged related to staff’s perceptions of therapeutic relationships. When developing relationships, staff need to overcome receptiveness issues by increasing trust through understanding their preconceptions, reducing stigma, sharing innocuous stories, and giving patients the time they need. The key pillar underpinning all traits ascribed to patients and staff is collaboration and approaching treatment protocols with a social approach is essential to enhancing recovery. Staff shared a holistic view of recovery that incorporated the benefits of positive relationships and the need to create a sense of home within the institution.     

Hope in the treatment of sexual offenders: the potential application of hope theory

Abstract

The present paper attempts to demonstrate how hope theory is conceptually consistent with current positive approaches to sexual offender treatment. Hope theory and its components of goals, pathways thinking, and agentic thinking is reviewed as defined by Snyder and his colleagues. A theoretical relationship exists between the role of hope and many aspects of sexual offending and treatment. Developmental course, attachment style, and the formation and maintenance of coping styles are reviewed as they relate to hope and sexual offending. A framework by which to apply hope theory to pre-treatment, treatment, and more specifically, self-management or relapse prevention for sexual offenders is proposed. Emphasis is placed on the use of techniques that target hope components, particularly in pre-treatment programs.     

Generating data with sexual offenders using qualitative material: A paradigm to complement not compete with quantitative methodology

This paper proposes triangulation as a research strategy in which the domain of sexual offending may be assessed using both qualitative and quantitative data. Examples of studies that have employed this approach with sexual offenders are outlined, showing the benefit that can be gained from utilizing multiple methods of assessment. The paper also outlines a range of quality criteria when conducting qualitative research, that Gaskell and Bauer (2000) argue are functional equivalents of reliability and validity.     

The new international financial architecture and Caribbean ofcs: Confronting financial stability discourse

This article examines the role and force of international finance in contemporary capitalism and the norms being established by international financial governance structures. More than the matter of exclusion, the article argues that financial stability discourse girding issues related to the nature of capital market liberalisation, terrorist financing, money laundering and tax evasion attempts to suture over, erase and/or render illegitimate ideational resistance—particularly by offshore financial centres (ofcs) of the global South. The ongoing experience of the English-speaking Caribbean ofcs is discussed as local operatives have long been simultaneously framing as well as resisting Anglo-American insistence on a new configuration of market ethics, norms and sociality in the virtual world of ‘shores’ and high finance. This adds to the emerging body of work seeking to discursively unpack financialisation.     

Control versus consent in community-based long-term care services: the role of economic empowerment

Professor Kapp responds to the article in this issue by Safewright and McAuley. He discusses the use of economic empowerment as a public policy strategy that is essential to a meaningful consideration of enhancing consumer autonomy and choice within the context of publicly-funded home and community-based long term care programs.

Heterosexuality and Race in the Australian Same-Sex Marriage Postal Survey

ABSTRACT

This article argues that race and class are central aspects of sexual citizenship in a Australia. It does so by investigating representations of heterosexuality that were produced and circulated during the 2017 same-sex marriage postal survey. Engaging with feminist and critical race theorists, we position same-sex marriage as not exceptional but part of a wider distribution of sexual citizenship within Australia's ongoing settler colonial history. We do so by introducing a number of illustrative examples of representations of heterosexuality produced during the survey. These representations reveal how same-sex marriage perpetuated heterosexual authority by asserting claims to authenticity and the occupation of space. We observe how heterosexuality in the survey material reproduced fantasies linking these three themes, for example, in an authentic white heterosexual family who speaks from their suburban backyard. It reveals that ceding to a bifurcated view of either progressive or conservative voices forestalls rather than advances other visions which may exceed the limited imaginings of sexual citizenship offered by the white liberal settler colonial state.     

Formal and Informal Seed Systems in Kenya: Supporting Indigenous Vegetable Seed Quality

Indigenous vegetables play an important role in Kenyan food security, but production is limited by poor seed quality. Traditionally, seeds have been traded through informal networks, but a new formal seed sector is emerging. This study assessed the relative potential for formal or informal seed systems to meet the need for high-quality indigenous vegetable seed. By evaluating determinants of farmers’ seed purchasing behaviour, we conclude that informal seed systems have greater potential to meet this need and should be strengthened. This study suggests that policy-makers should use context-specific data to guide decisions on seed policy.
