New health professions after a decade and a half: delegation, productivity and costs in primary care

Though knowledge about physician's assistants and nurse practitioners is far from conclusive, these new health practitioners (NHPs) appear to perform a large percentage of primary care services at a high level of quality and productivity. Moreover, the gap between the physician/NHP substitution ratio and the NHP/physician cost ratio seems wide enough to assure cost savings when NHPs are used well.     

Comment: Does benefit cost analysis stand alone? rights and standing

The issue of standing in benefit cost analysis is not different from the issue of rights. Benefit cost analysis contributes to legal analysis and also rests upon legal analysis. Debates about standing issues can be reinterpreted as questions of the role of benefit cost analysis when rights are uncertain at the margin. This perspective illumines such questions as whether gains to the criminal count and what weight should be given to expert opinion, to irrational fears, and to gains or losses by foreigners. This perspective is also consistent with a rights-based interpretation of 1) the willingness-to-pay approach, 2) an approach that considers distributional consequences, and 3) an approach that ignores distributional consequences when the costs of determining them are likely to be greater than the benefits.     

Issues in Program Budgeting

Funding arrangements for human service programs are often complex, particularly when intergovernmental financing is involved. There are two financial arrangements used by human services administrators to assure that money budgeted is spent as intended. Reallocation of unspent funds among jurisdictions and carryover to the following year of unspent balances are examined in this article based on examples drawn from two programs in Minnesota.     

Information transfer implications of development research administration in Nigeria

The study of research utilization and administration has been a growing field, but the emphasis in this field has been on industrialized countries. This article presents the results of a pioneering study of the use of research by senior officials in the Nigerian Federal Government. The article focuses first on Federal Government-sponsored research, it reviews how such research was initiated and conducted, how results were presented and what affected the use of the results. The article then describes expatriate-sponsored research, casts light on the conventional assumption about the amount of such research and points to the fact that there was comparatively less awareness of the results. The article not only provides significant information about the administration of research in Nigeria but emphasizes the need to give attention to this aspect of administration.
