The azygos vein ascends along the thoracic spine through the mediastinum and drains into the superior vena cava at the level of the fourth thoracic vertebra. Fracture-dislocation of the mid-thoracic spine, as a result of blunt thoracic trauma, can tear the azygos vein. Four such fatal cases (three motor vehicle accidents and one fall) were studied, only one of which was recognized prior to death. The vein can also be torn, in the absence of skeletal injuries, by horizontal acceleration/deceleration forces. The pathologist must consider azygos vein laceration as a possible cause of either hemothorax or hemomediastinum or both in a victim of a blunt chest trauma, if that individual had persistent hypotension during the few hours before death and no identifiable source of hemorrhage can be found postmortem in sites such as the heart, great vessels, lung, and chest wall. A fracture-dislocation of the thoracic spine may not necessarily be present. Azygos vein laceration seems to be an uncommon cause of hemothorax and hemomediastinum; however, this injury is probably more frequent than is implied by the few cases described in the medical literature.  相似文献

In this article no attempt is made to defend the Reagan administration's six-year-old policy of constructive engagement toward the Republic of South Africa, for to all but a few diehards the policy is indefensible. Instead, my response to Beres contains three components. First, I take issue with his dismissal of constructive engagement as no more than a ruse by which "to make apartheid palatable" to the American people, and argue that the purpose of the policy was to enable the Reagan administration to take credit for achieving a Namibia independence settlement. Second, I show that Beres' plea for popular opposition to an unjust policy comes too late, for a large number of Americans, during the past two years in particular, have been doing his bidding. Finally, I conclude with an assessment of the nature of the policy debate over South Africa that such popularity has prompted, in which direction U.S. South Africa policy now appears headed, and what issues, if any, remain to be resolved.  相似文献

A meta-analysis investigating the relationship between marital satisfaction/discord and intimate partner violence (IPV) in heterosexual relationships was conducted with 32 articles. Overall, a small-to-moderate effect size (r = −0.27) indicated a significant and negative relationship existed between marital satisfaction/discord and IPV. Moderator analyses found no differences between effect size based on construct examined (discord or satisfaction). However, the magnitudes of observed effect sizes were influenced by other moderator variables, including the use of standardized versus non-standardized measures, gender of the offender and victim, role in the violence (perpetrator versus victim), and sample type (clinical versus community). The data suggests that gender is an especially important moderator variable in understanding the relationship between marital satisfaction/discord and IPV.  相似文献

The global infrastructure sector is thriving. But community opposition to major projects is also rising. Australian examples demonstrate the policy backflips, reputational pitfalls, and financial costs of project delays and cancellations. Failures to engage communities are surprising, given the widespread adoption of community engagement (CE) principles and the increasing professionalization of CE roles. If acceptance of the need for CE in infrastructure is more widespread than ever, why are we not seeing smoother project delivery, reduced protest, and cost savings? This paradox is the driving force behind the Next Generation Engagement project. This article offers a practitioners’ perspective to introduce the project and present key findings from its 12‐month pilot study aiming to establish a transdisciplinary, industry‐led research agenda for CE in Australia's infrastructure sector. The article contributes to our understanding of CE literature and research codesign. It maps out the top five priority themes for future research to support infrastructure selection, planning and delivery. The research agenda provides guidance for policy, and practice, offering consolidated, research‐based insights for policymakers and practitioners.  相似文献
