Skin lesions suspected to have been caused by a burning cigarette require thorough diagnostic evaluation as to the mode of infliction. Accidental cigarette burns must be differentiated from injuries due to self-infliction or maltreatment. The typical categories are presented on the basis of the literature and exemplary cases from the authors' own study material. An intentional infliction must be taken into consideration when a body region is involved which does not normally come into contact with a cigarette by chance. Full thickness burns from glowing cigarettes require an exposure time of more than 1s. One should also keep in mind the possibility of confusion with local skin infections or thermal effects by traditional medical practices (e.g. moxibustion). In unclear cases, repeated inspection of the lesion is recommended in order to facilitate its classification as to causation and age. The courses of healing in first- to third-degree cigarette burns are demonstrated by means of continuous photographic documentation. The discussion deals with different kinds of accidental and intentional cigarette burns, e.g. in drug addicts, psychiatric patients, victims of child abuse, maltreatment and torture, but also in persons feigning a criminal offence.  相似文献

Travel-related poisoning is an emerging social and public health emergency in Bangladesh but its cause and significance have not been determined. To investigate this syndrome we performed a prospective clinical study and retrospective analysis of hospital records in a general medicine unit of a public tertiary care teaching hospital in Dhaka, Bangladesh, using toxicological analysis by fluorescence polarization immunoassay (FPIA) and liquid chromatography coupled to time-of-flight mass spectrometry (LC-TOF MS). The participants of the prospective study were 130 consecutive patients aged 16-80 years who were admitted with central nervous system depression (Glasgow Coma Score 3-14) after using public transportation, in the absence of other abnormalities, from January through June 2004, and a convenience sample of 15 such patients admitted during 3 days in May 2006. In 2004-2006, travel-related poisoning increased from 6.1 to 9.5% of all admissions (210-309 of 3266-3843 per year), representing 46.6-55.7% of all admitted poisoning cases. Incidents were associated with bus (76%), taxi, train, and air travel, or local markets; 98% of patients remembered buying or accepting food or drinks before losing consciousness. Direct financial damage (missing property) was diverse and frequently existential. Among 94 urine samples analyzed by FPIA, 74% tested positive for benzodiazepines. Among 15 urine samples analyzed by LC-TOF MS, lorazepam was detected in all; five also contained diazepam or metabolites; nitrazepam was present in three. FPIA results obtained for these 15 samples were below the recommended cut-off in eight (53%; lorazepam only). Our findings show that the massive medicosocial emergency of travel-related poisoning in Bangladesh is the result of drug-facilitated organized crime and that benzodiazepine drugs are used to commit these crimes, suggesting modifications to the local emergency management of the victims of this type of poisoning. They also highlight the need for more research in the neglected field of acute poisoning in Bangladesh, and for criminal investigations of the use of benzodiazepine drugs in this country.  相似文献

Placed within EU Cohesion policy and its objective of European territorial cooperation, macro-regional strategies of the European Union (EU) aim to improve functional cooperation and coherence across policy sectors at different levels of governance, involving both member and partner states, as well as public and private actors from the subnational level and civil society in a given ‘macro-region’. In forging a ‘macro-regional’ approach, the EU commits to only using existing legislative frameworks, financial programmes and institutions. By applying the analytical lens of multi-level and experimentalist governance (EG), and using the EU Strategy for the Danube Region as a case, this article shows that ‘macro-regional’ actors have been activated at various scales and locked in a recursive process of EG. In order to make the macro-regional experiment sustainable, it will be important to ensure that monitoring and comparative review of implementation experience functions effectively and that partner countries, subnational authorities and civil societies have a voice in what is, by and large, an intergovernmental strategy.  相似文献

This paper provides an overview of the operational implementation of negative interest rates in Europe and Japan. Drawing attention to the fact that there is precedent for negative policy rates and negative money market rates, the paper addresses conceptual issues and summarizes measures which define negative interest rate policy. Based on detailed institutional analysis and an examination of the interaction of interest rate policies with balance sheet policies, it is argued that there exists substantial heterogeneity in the purpose, design, and operational specificities of negative interest rate policies across economies, with significant consequences for effective market rates, private sector funding conditions, and expectations. Summarizing transmission channels of negative rates, benefits, and risks of the policy, the paper calls attention to potential adverse effects resulting from the interaction of negative interest rates with tighter liquidity and capital standards adopted since the Global Financial Crisis.  相似文献

Green accounts of environmental citizenship typically seek to promote environmental sustainability and justice. However, some green theorists have argued that liberal freedoms are incompatible with preserving a planetary environment capable of meeting basic human needs and must be wound back. More recently, ‘ecomodernists’ have proposed that liberalism might be reconciled with environmental challenges through state-directed innovation focused on the provision of global public goods. Yet, they have not articulated an account of ecomodernist citizenship. This article seeks to advance the normative theory of ecomodernism by specifying an account of ecomodernist citizenship and subjecting the theory’s core claims to sympathetic critique. We argue that state-directed innovation has the potential to reconcile ambitious mitigation with liberal freedoms. However, full implementation of ecomodernist ideals would require widespread embrace of ecophilic values, high-trust societies and acceptance of thick political obligations within both national and global communities. Ecomodernism’s wider commitments to cosmopolitan egalitarianism and separation from nature thus amount to a non-liberal comprehensive public conception of the good. Furthermore, ecomodernism currently lacks an adequate account of how a society that successfully ‘separates’ from nature can nurture green values, or how vulnerable people’s substantive freedoms will be protected during an era of worsening climate harms.  相似文献

The relationship between managers’ use of rewards and employees’ motivation is controversial, but recent research suggests that employee perceptions of governance initiatives can help us understand the association. Experimental evidence further emphasizes the difference between verbal and material rewards, and this study therefore analyzes real-world public managers’ use of these different rewards and their employees’ perceptions of a governance initiative. This enables us to shed light on a mechanism through which rewards can be connected to motivation. Can rewards contribute to a supportive perception of governance initiatives and potentially crowd-in motivation? This is analyzed in a multi-level dataset with 82 Danish school principals and their 1,273 employees. Managers’ use of contingent verbal rewards is positively associated with employees’ perceptions of the relevant governance initiative, while the corresponding association between material rewards and employee perceptions is weak and statistically insignificant when controlling for other types of managerial behavior. Although the findings need to be tested in an experimental design, they suggest that verbal rewards are a promising tool for public managers.  相似文献

Public opinion on immigration is increasingly relevant for political behaviour. However, little is known about the way in which citizens’ political allegiances in turn shape their attitudes to immigration. Abundant existing evidence suggests that voters often take cues from the parties they support. Using panel data from the Netherlands and Sweden, this article investigates the dynamic relation between attitudes to immigration and party preferences. The longitudinal nature of the data allows for making stronger claims about causal mechanisms than previous cross-sectional studies. The analysis shows that voters who change their preference to the Radical Right become stricter on immigration, whereas voters changing to the Greens become less strict on immigration over time. This confirms that citizens’ support for anti- and pro-immigration parties results in a ‘radicalisation’ of their views on immigration along party lines. A similar ‘spiral’ of radicalisation can be found around the issue of European integration.  相似文献

This article examines women's polling and eligibility in the municipal elections in Swedish cities and towns in the decades round the 1900 turn of the century. The aim is to present the patterns of suffrage, voting behaviours and representation that emerge from statistics produced by the women's movement and to discuss how these relate to the women's movement's strategies for women's political citizenship and national suffrage. The results are furthermore analysed in comparison with eighteenth-century conditions, when legally competent women who paid taxes could vote in some elections.  相似文献

‘Results’, ‘value for money’, ‘effectiveness’ and similar buzzwords have become commonplace in development cooperation and peace building. The use of technical instruments such as project cycle management and evaluations is hardly questioned anymore: these are presented as a minor shift of focus to make current practice more effective. This paper argues that there is far more to this shift: a machinery of practices and institutions has been installed that removes political questions on development or peace from the political realm and places them under the rule of technical experts. Drawing on a Foucauldian understanding of discourse analysis, the paper analyses how this machinery prioritises gradual reform, subjugates other approaches to societal change and reproduces power/knowledge networks in both the global South and North. Based on ethnographic field research in Myanmar, it also explores discursive strategies of local actors and assesses how they are aiming to create spaces to challenge this machinery.  相似文献
