Abstract: Controversy has surrounded the use of antidepressants and suicidal behaviors in youth. This study reviewed the Medical Examiner’s Office records of 253 persons aged 24 years or younger classified as suicides in Miami‐Dade County, Florida, from 1990 to 2007. Information was collected regarding demographic information, suicidal characteristics, psychiatric and psychosocial factors, and toxicology results. Eighty‐five percent of the sample was men, and 53.4% of the subjects were Hispanic. Consistent with previous literature, the existence of antidepressants at autopsy was rare; present in only 6% (n = 15) of the victims. The occurrence of antidepressants was not significantly different between Hispanics (n = 7) and non‐Hispanics (n = 8). The incidence of antidepressants was weakly correlated with a tendency to be men and no history of psychiatric illness. The presence of antidepressants in Hispanic youth suicide victims was similar to non‐Hispanics. 相似文献

This article provides a systematic evaluation of the options for incremental health insurance reforms aimed at older Americans nearing age sixty-five. It presents three basic arguments for giving special consideration to this age group: (1) early retirement and its effect on access to employer insurance; (2) changes in health and health care expenses associated with increasing age; (3) the vulnerability to unexpected economic or health "shocks" that will affect people throughout their retirement. The analysis of policy options begins by specifying criteria for evaluating alternative approaches to reform. The proposed criteria emphasize that reforms for this age group should be designed to fit with other financial plans and decisions made during such a transitional stage of life. Policy options should be judged according to fundamental goals such as equity and efficiency, not simply ranked according to the number of uninsured who will gain coverage. After offering a comprehensive catalog and evaluation of available options, the analysis identifies and discusses a preferred approach-which preserves choices while offering universal and subsidized access to Medicare before age sixty-five. 相似文献

In the early 1980s, the leadership of the antiabortion movement became involved in a campaign to establish legal rights to extraordinary medical care for seriously handicapped newborns. Armed with political contacts in the Reagan administration and Congress, and allied with advocates for the disabled, the antiabortion movement searched for a test case to guide through the courts. Antiabortion advocate Lawrence Washburn found such a case in Baby Jane Doe, who was being treated at Stony Brook Medical Center. The movement went on to amend the Child Abuse Act to include protections for handicapped newborns. Activists in the movement chose the issue of Baby Jane Doe because they believed it would attract welcome publicity, give them the appearance of supporting civil rights, and enhance their argument as to the legal rights of the fetus and thus strengthen the case against abortion.
