Unexpected death due to infectious mononucleosis

A 14-year-old boy with infectious mononucleosis died unexpectedly in hospital. The most significant finding at autopsy was the presence of marked bilateral tonsillar enlargement with considerable narrowing of the upper airway. There were no other underlying organic diseases that could have caused or contributed to death. Narcotic analgesia had been administered less than 2 h before death and may have contributed to respiratory compromise. The blood morphine level was 0.08 mg/L. Toxicological evaluation of individuals with obstructive lesions of the upper airway may, therefore, be a useful adjunct to the autopsy assessment of such cases as it may reveal factors exacerbating mechanical blockage.     

Data registers in respiratory medicine: a pilot project evaluating compliance with privacy laws and the National Statement on Ethical Conduct in Research Involving Humans

This article reports on data from a small pilot survey evaluating the compliance of voluntary databases in respiratory medicine with privacy laws and the National Health and Medical Research Council's National Statement on Ethical Conduct in Research Involving Humans. The increasing complexity of privacy law, including the recent private sector amendments, creates many challenges for database administrators. The impact of privacy laws upon voluntary or non-statutory databases, and upon doctors reporting patient data to such databases, is far from straightforward. The article suggests way in which the law might be adapted in order to better facilitate the role of voluntary data registers in health research and public health surveillance, while still protecting the privacy of patient information. The article also briefly considers how database administrators might "future-proof" their existing data holdings to ensure compliance with legal and ethical standards.     

Serving those Who Served: Outcomes from the San Diego Veterans Treatment Review Calendar (SDVTRC) Pilot Program

Veterans Treatment Courts (VTCs) are a type of specialty treatment, problem-solving, criminal court. Though the number of VTCs has increased over the past decade, few research studies have examined their effectiveness. This paper examines the data collected concerning a particular VTC experience, the first 82 Veterans enrolled in the Veterans Treatment Review Calendar Pilot Program conducted by the California Superior Court of the county of San Diego from February 2011 until July 2014 (SDVTRC.) The evidence presented herein concerns the nature of this cohort’s population, the SDVTRC program structure in which these Veterans participated, and the outcomes of participation. SDVTRC participants showed a significant decrease in symptoms on 11 of 12 clinical measures from baseline to 12 months. Particular outcomes of the SDVTRC program were related to factors of military service, such as length of service, number of awards, and discharge status. There were also significant relationships between symptom decrease and court process factors, such as length of time in program and sanctions imparted. This court has a 0% recidivism rate, which is believed to be related to its systematic data collection process that was used to inform individualized treatment plans for participants. Implications for other VTCs are provided.     

“The Sore That would Never HeaL”: The Genesis of the Polish Corridor

This paper consists of an examination of the historic and ethnographic origins of the Polish Corridor question and of the various suggestions proposed for its equitable solution in the aftermath of World War One. It outlines the negotiations and memoranda devoted to the issue at the Versailles Conference and, in particular, the role played by the British Prime Minister, David Lloyd George. It concludes that the result of these deliberations was as much the product of international tensions and a collective failure of vision as of any coherent planning or policy.     

The Greater Menace to the Peace of Nations? The 1877 Mills Mission and Direct Canadian–American Diplomatic Relations

In 1877, employing the unwanted presence of armed Sioux in western Canada as a useful mechanism, Canada’s Cabinet, represented by Minister of the Interior David Mills, unhappy with perceived British indifference to Canada’s concerns, sought to initiate direct diplomatic relations with the United States. That effort failed, and British opposition to this endeavour was so sharp that Canada made no similar initiative for half a century. Although he failed, Mills’ effort marked the birth of the Functional notion that Canada’s voice should matter more in Imperial foreign policy formulation when its direct interests were at stake, especially when dealing with the United States.     

Mediation in disputes between municipal governments

Emerging forms of intergovernmental management include roles for neutrals in a range of problem solving and conflict management situations. Third party intergovernmental mediation has been used to settle adversarial municipal boundary disputes in Virginia since 1980. Settlements fashioned through mediated negotiations are responsive to local officialst concerns. This paper reports on this innovation.
