Sudden death in right ventricular dysplasia with minimal gross abnormalities

Arrhythmogenic right ventricular cardiomyopathy is emerging as a relatively common cause of exercise-induced sudden death in the young. The diagnostic criteria at autopsy are, however, not fully established, leading to both over- and underdiagnosis. We report a young man and a young woman dying suddenly of right ventricular dysplasia during exercise, in whom the gross autopsy findings in the right ventricle were minimal or even absent. However, the histologic features in both right and left ventricles were typical of the disease, and consisted of fibrofatty infiltrates with typical myocyte degeneration of the right ventricle and subepicardial regions of the left ventricle. These cases illustrate that microscopic findings are diagnostic and may be present in the absence of gross findings. Marked fat replacement is not essential for the diagnosis of right ventricular dysplasia, and the right ventricle should be extensively sampled histologically in all cases of sudden unexpected death, especially those that are exercise related.     

Detection of azide in forensic samples by capillary electrophoresis

Azide salts are highly toxic compounds that have been difficult to detect in forensic samples. Here, anion analysis by capillary electrophoresis with indirect spectrophotometric detection was applied to detect azide in forensic specimens from two suicide victims. Gastric specimens from the victims were shown to have high azide concentrations; azide represented one of the major anionic components and no corresponding component occurred in normal gastric juice. Samples of blood and bile had low concentrations of azide near the limits of detection. The method described for azide analysis used simple steps for sample preparation and analysis time was less than 10 min per sample. It offers a simple and reliable method for detecting azide in biological fluids.     

Unexplained sudden death and the likelihood of drug abuse

The common history of drug abuse in adults with an undetermined cause of death has led us to hypothesize that chronic drug abuse increases the risk of sudden death. To begin evaluating this hypothesis, we conducted a retrospective case-control study of 61 decedents whose cause of death remained undetermined following autopsy matched one to one to a control group of pedestrians or passengers killed in motor vehicle collisions. In 21 pairs, the case subject had evidence of drug abuse but the control did not, and in 5 cases the reverse was true. Analysis showed that individuals with an undetermined cause of death are 4.2 times more likely to have evidence of drug abuse than are victims of a motor vehicle collision.

Acupuncture in drug treatment: Exploring its role and impact on participant behavior in the drug court setting

The originators of the Miami drug court incorporated acupuncture into the substance abuse treatment regimen that has been widely imitated in hundreds of drug courts since 1989. Although there is some evidence to suggest that acupuncture may be an effective adjunct to treatment more generally, research has not yet examined its role and impact in the drug court setting. This paper describes an effort to study the impact of acupuncture on offender behavior and progress in treatment in the Clark County, (Las Vegas) Nevada Drug Court using a prospective modified experiment, where 336 new participants were randomly assigned to acupuncture and no-acupuncture conditions. However, significant treatment contamination hindered straightforward analysis, as nearly 40% of the control group received at least some acupuncture. To compensate for the treatment compliance problem, two-stage least-squares (2SLS) regression is employed with original group assignment as an instrumental variable and acupuncture exposure as a predictor. Results indicate no significant difference along a range of criminal justice and treatment outcomes, with the exception of one measure of treatment progress. The paper concludes with a discussion of the implications of the findings and the need to design studies that are better able to separate the effects of acupuncture from other treatment and court interventions.     

The development management task and reform of 'public' social services

With its emphasis on target-setting and performance measures, the New Public Management (NPM) appears to offer a coherent and 'no-nonsense' approach to public sector reform and the public management task. This article suggests that three questions require further thought: 'Management of what?', 'Management by whom?', and 'How to manage?' It considers these questions using the case of Community Based Health Care (CBHC) and its promotion by NGOs in Tanzania. The article argues that the task of public management is one of managing an arena of public action which includes (and excludes) a range of actors and agendas. Once this is taken into account, it becomes clear that the challenge to all development managers is how to manage more effective interdependence.
