EEO issues in the health-care field: a roundup of recent developments.

The rapidly expanding health-care field, like other service areas, has experienced its share of controversy over employment discrimination issues. Recent cases fall into two categories: claims involving traditional EEO questions and claims that reflect the unique nature of the health-care institution. A survey of cases in an array of key areas offers guidance for health-care employers in handling common problems.     

Unusual hanging deaths

Four unrelated hanging deaths involving young white males, all in enlisted status in the United States military, are presented. Taken in a military context, one scene served as a suicide note equivalent with the unmistakable message of extreme defiance toward military regulations. In two cases, the elaborate attire of the decedents could have lead to the misclassification of the manner of death if investigative information had not been considered. Extensive background investigation and review of medical records in the fourth case disclosed that the victim had an eight- to ten-year history of compressing his neck to cause unconsciousness. He left an extensive suicide note in which he concluded that "hanging would be good." Each case has certain unique features, as a group, the cases all demonstrate a continuum from obvious suicide to cases where the manner of death is, at first, equivocal.     

Distribution of morphine in body fluids and tissues in fatal overdose

Two cases of fatal overdose with morphine are presented. Large amounts of the drug were involved in both cases, one by oral ingestion, the other by intravenous injection. Morphine concentrations in various body fluids and tissues are compared to those in the literature.     

Sudden unexpected natural death in young persons

In a series of 1920 medico-legal autopsies, 78 cases fulfilled the criteria of sudden unexpected natural death in the age group 2-30 years. In 39 (50%) of these cases a health hazardous condition was known. A large group included epileptics, alcoholics, obstetric/gynecologic conditions, and drug addicts. For prophylactic reasons the medical intervention should focus on earlier detection, if possible, of acute myocarditis, better instruction to epileptics as to thorough medication and information of parents. Physicians should be reminded about the danger of inflammatory processes in the upper respiratory tract, especially acute tracheobronchitis and acute epiglottitis.     

Identification of fentanyl derivatives

An interpretative approach to the identification of fentanyl homologs and analogs is presented. The techniques employed are liquid/liquid extractions; capillary gas chromatography; and infrared, mass, and nuclear magnetic resonance spectral characterization. Spectral data are presented for eight fentanyl derivatives of clandestine origin.
