Most of the criteria for competence in current use emphasize cognitive rather than affective dimensions. Our clinical experience indicates that affective disorders may impair competence in a detectable and identifiable way. In particular, patients with major affective disorders can retain the cognitive capacity to understand the risks and benefits of a medication, yet fail to appreciate its benefits. A case study of a pathologic grief reaction is introduced to illustrate how cognitive and affective impairments may coexist and require separate remedial strategies for restoration. Further empirical work on the role of affective disorder in impairing competence is warranted and planned.  相似文献

A leading British political scientist argues that Gorbachev's responsiveness to different opinions and institutional interests has been a necessary part of the Soviet transition. He stresses the importance of Gorbachev's combination of tactical flexibility and movement in a basically liberal direction. One step back has often been followed by two steps forward. The author maintains that Gorbachev's political acumen and international prestige remain significant assets, suggesting that it would be an illusion to imagine that the task of transforming the Soviet system would proceed more smoothly without him. As General Secretary, Gorbachev evolved from a Communist reformer to a socialist of the social democratic type. The most important programmatic documents of the CPSU in 1990–1991, on which Gorbachev had the predominant influence, were essentially social democratic. (Many in the Party apparatus had no intention of implementing them.) From 1988, Gorbachev was advocating fundamental change of the Soviet system, although there were zig-zags in his public pronouncements under the pressure of events. Ill-understood both at home and abroad, Gorbachev's social democratic ideas were ‘outflanked’ by the market fundamentalism and political impatience of his radical opponents and frustrated by the implacable enmity of conservative defenders of the vanishing Communist party-state.  相似文献
