Excited delirium denotes a life-threatening medical condition characterized by the acute onset of agitated and violent behavior that often results in a sudden and unexplained death. Cocaine-induced excited delirium refers to fatal cocaine intoxication with the following symptoms occurring sequentially: hyperthermia, delirium with agitation, respiratory arrest, and death. We present a case of cocaine-induced excited delirium in a cocaine “body packer” or a “mule”, specifically an individual who attempts to smuggle cocaine within the body. Investigators at the scene initially suspected homicide due to the victim's sharp and blunt force injuries. Three rubber packets containing cocaine were removed from the victim's rectum. Blood toxicological analysis revealed an alcohol concentration of 0.016 g/100 and cocaine >1 mg/L. The forensic pathologist should consider cocaine-induced excited delirium when an individual exhibits aggressive behavior, unexpected strength, and resistance to pain who dies suddenly. Further analysis should be performed during the scene investigation and autopsy for evidence of body packing. 相似文献

Debates about US empire have subsided somewhat in the aftermath of the George W Bush presidency but the issues underlying such debates have not gone away. In arguing that the history of the United States is an imperial one, this article proposes that US empire is the expression of an intersectional totality, one shaped by various vectors of power but reducible to none. To make this case, the article presents a sketch of US imperial history in order to show how this intersectional totality has evolved over time. Such an exercise can give useful context to the foreign policy initiatives of the Barack Obama administration, one that differs from that of its immediate predecessor but is not outside the structure of imperial history’s longer duration. 相似文献

This article examines the impact of structural reforms and industrial relations changes on the employment security and decision behaviors of middle-level managers in the Australian Taxation Office (ATO). This article is based on an empirical study using focus group and survey data that investigated how structural change and public-sector reforms substantially altered the employment environment. This research reveals that the ATO environment can be characterized by low morale, risk aversion, fear, and distrust brought about, in part, by employment insecurity. Where middle managers perceived organizational threats to their employment security, they engaged in self-protective “survivor” behaviors even when no such threats to their employment security existed. The article concludes that a substantial number of middle-level managers, survivors of years of restructuring, downsizing, and organizational change, were unlikely to display high-quality decision-making behaviors. 相似文献

Is it possible to imagine New Public Management without marketization? In Denmark the present liberal-conservative Government has, throughout its 10 years in power, designed and implemented more than 15 major management reforms in the public sector. Although most of the reforms are rhetorically firmly rooted in neo-liberal ideologies they have, in practice, promoted tools and mechanisms of the “traditional,” or Old, Public Management. Based on an empirical study of the reforms, we suggest that the notion of “pragmatic” New Public Management is introduced to enhance the current understanding of New Public Management in the Western industrialized societies. 相似文献

The public participation agenda is a significant element of public service reform in Europe and beyond. This article examines how citizen participation in local public services was conce ived and enacted under successive New Labor governments in the UK during the period 1997–2010. It is suggested that the emphasis on public participation was central to the ethos of New Labor. This accounts for the persistence of the participation agenda even in the face of scant empirical evidence that specific engagement and empowerment initiatives were successful. Significantly, there was no single New Labor participation narrative. There were instead several distinct strands, drawing variously from the Party's received traditions of State welfarism, corporatist central-local relations, municipal socialism, Blairite managerialism, and, lastly, mutual cooperativism. The article explores the bidding process for the European Capital of Culture (ECOC) award, an aspect of local regeneration policy reliant upon a specific conception of culture. The process is examined in terms of changes in urban layout, manifestations of cultural and community identity, media representations, and the spectacle of culture, gender, and locality. The process is viewed as an urban managerialist project, driven by private and public sector elites in pursuit of economic rather than cultural goals. A narrow and particular view of culture was employed in the bidding process to achieve essentially managerial goals, and cut adrift from significant issues of gender, identity, and class. “Culture,” as conceived within the ECOC process, is viewed as a policy product of local government, regeneration partnerships, government agencies, and business interests, in contrast to culture as a way of life or lived urban experience. Communicating with citizens, stakeholders, or service clients is challenging under normal circumstances. Reaching government audiences who are hard to reach because of language or culture differences, lifestyle unpredictability, mistrust, isolation, or other reasons compounds the difficulty. This article examines who the hard to reach are, addresses reasons why it is important for governments to reach them, explores research and experience, suggests effective approaches for reaching these audiences—drawing upon a social constructionist approach—and proposes lessons and guidelines for public sector communicators. Communication practice and research indicate that more effective strategies include: utilizing knowledge about target audiences; forming partnerships with agencies and individuals that interact with targeted populations; utilizing children to reach parents and older relatives; using ethnic media that effectively reach immigrant and ethnic minority households; and simplifying communication and using feedback techniques. 相似文献

Producers in small-scale rural markets often receive unfavourable prices for their goods as a result of more powerful market participants. This study uses a combination of price analysis and interview data to assess the position of snake hunters in the aquatic snake market from Tonle Sap Lake in Cambodia. Despite the hunters' dependence on intermediary traders for market access and credit, the evidence implies that they are not powerless participants. Intermediary traders operate under high competition as a result of the increasing scarcity of snakes and therefore, despite interlocked credit and snake markets, offer relatively high prices to hunters. 相似文献

Little is known about the labour market impact of immigration to developing countries, because most immigration to developing countries is poorly measured. We use an unusual dataset from a campaign to register irregular migrants to study how immigration has affected wages, employment, and internal migration in Thailand. We allow for endogenous migration, whereby immigrants are disproportionately attracted to areas with higher wages. Our results suggest that immigration sufficient to increase Thailand's total labour force by one per cent would reduce Thai wages by approximately half a per cent. This effect is stronger than is generally found in developed countries. We find no evidence that immigration has reduced Thai employment rates or has affected internal migration by Thais. 相似文献
