Social Health Insurance and the Use of Formal and Informal Care in Developing Countries: Evidence from Ghana’s National Health Insurance Scheme

This paper studies the relationship between the use of formal and informal health care in a developing country setting by examining the introduction of a social health insurance scheme in Ghana. We estimate the effects of gaining coverage on changes in care seeking behaviour and show how these effects differ by age and wealth status. District-level differences in the implementation of the insurance scheme provide exogenous variation in access to insurance and allow us to address issues with selection into coverage. Results indicate that insurance access strongly increased use of formal care and reduced out-of-pocket expenditures on health services.     

Counter-Intelligence for counter-revolutionary warfare: The South African police security branch 1979–1990

In counter-revolutionary warfare strategy, political action forms the overwhelming part; however, also central as an off-shoot of the tenets of counter-revolutionary warfare is the elimination of insurgent structures – generally a euphemism for assassination. In reality, assassination is a subset of covert paramilitary action, implemented as a consequence of counter-intelligence or even counter-terrorism. South Africa's Security Branch presents one of the best recent examples of the use of counter-intelligence techniques for counter-revolutionary warfare. While politico-constitutional intelligence was gathered by the National Intelligence Service to support constitutional efforts to achieve a settlement of the conflict and bring about a new political dispensation in South Africa, the apartheid government relied on the covert operational capabilities of the security forces (especially the Security Branch) to not only halt the ‘Revolutionary Onslaught’ of the liberation movements, but to eliminate them as a viable political and revolutionary force. In attempting to support an unwinnable political objective, the ultimate corruption of the intelligence process and the reliance of individuals overseeing it far and away on the covert operational intelligence capabilities of the state, the apartheid government brought about its own downfall. The Security Branch was at the heart of these efforts.

he meaning of the term 'community service obligation' (CSO) has been a point of conjecture in recent years. This paper highlights the importance of CSOs within the context of the accountability of public sector organisations. In particular the paper advocates the need to formulate an agreed definition of a CSO. The paper seeks to distil such a definition by using academics and GBE representatives as authoritative opinions.     

The Anglo-American “Special Relationship”: The Lazarus of International Relations

Scholars have variously queried the existence of the Anglo-American “special relationship,” consigned it to history as “special no more,” or demanded that Britain choose between its European and American relationships. These critiques have become increasingly prevalent since the Cold War. Yet the current British government, like many before it, continues to portray a choice between America and Europe as a “false choice,” and the “special relationship” has arguably deepened in the wake of the September 11 terrorist attacks. This article contends that international diplomatic history can contribute much to understanding the “Lazarus-like” quality of the “special relationship.” Specifically it argues that a number of critical continuities in post-World War II British foreign policy survived the end of the Cold War and have since contributed heavily to the determination of the British foreign policymaking elite to maintain the “special relationship” at the same time that Britain pursues a leadership role within Europe.     

Understanding Judicial Hierarchy: Reversals and the Behavior of Intermediate Appellate Judges

One of the central controversies in the judicial behavior literature is the extent to which judges' ability to act according to their ideological preferences is affected by their location in the judicial hierarchy. Judges on intermediate appellate courts have different decisionmaking environments than high court judges. As a result, the goals of lower appellate court judges may differ from those of their superiors: the quest for legal accuracy may compete with the desire to pursue policy preferences. Analysis of the reversal rate of the U.S. circuit courts of appeals offers insight into the extent to which these judges balance the pressures of their own policy preferences with the desire to achieve the legally accurate result in cases they decide.     

Devolution and Development: Territorial Justice and the North-South Divide

One of the unintended consequences of devolution under New Labouris that it might make it more difficult to secure territorialjustice between the nations and regions of the United Kingdom.Centralized regional policy, the aim of which was to promoteeconomic equity between the regions, has been replaced by adevolved regional policy that aims to promote an economic dividendwithin each region. In the unlikely event that devolution yieldsa uniform economic dividend in each region, the result doesnothing to redress the territorial inequalities at the heartof the North-South divide. Treating unequal regions equallyis not a recipe for territorial justice.     

Spectacular Security: Mega-Events and the Security Complex

In Discipline and Punish Foucault famously declares that "our society is one not of spectacle, but of surveillance." Our theoretical aim in this paper is to problematize Foucault's strict demarcation between spectacle and surveillance through an analysis of urban mega-events. In the process, we detail emerging features of contemporary mega-events that shape and are shaped by shifts in the field of security and surveillance more broadly. Three dynamics in particular warrant consideration: the move toward a precautionary logic among security planners, a "semiotic shift" wherein security iconography is integrally bound up with the production of contemporary urban spectacles, and various forms of security and surveillance legacies that circulate beyond the spatial and temporal frame of the event itself. While mega-events support Foucault's assertion of the dispersal of discipline across the social field, they also suggest that this dispersal occurs in concert with, not in spite of, the power of the spectacle in contemporary society.
