A number of factors, including cultural, social, economic, political, and historical, influence policy. United States and Canada's health care systems are the conscious and subconscious outcomes of formal political structures and informal political processes. The Canadian parliamentary political system encourages centralized, organized, planned policy in health care. However, this is accomplished at the risk of leaving some individuals—physicians in this case—quite frustrated. American constitutional features, the presidential system, weak political parties, and the tireless participation of interest groups in the political process all function to discourage the formation of highly rational and efficient policy. While few special interests are ever completely satisfied with the legislation produced, seldom is any organized group completely thwarted. Therefore, it is no surprise that a tightly integrated national program of universal health insurance was not adopted by Congress in 1993–1994. Political structures and processes discourage effective, comprehensive health care reform in the United States.     

Counterterrorism research and development

An effective de-radicalization process in prisons is intended to facilitate the renouncement of violence and terrorism by those that have carried out such offenses. A key lesson that can be drawn from Israel's de-radicalization efforts is that it is possible, indeed recommended, to treat inmates—regardless of their level of radicalization—in a dignified and humane manner. However, Israel's ability to significantly de-radicalize security prisoners is limited if it is at all existent in its current form. Security prisoners with the potential for positive change should be placed in a different, perhaps foreign setting. This article provides an overview of Israel's prison system, the challenges it faces, its efforts to de-radicalize security inmates and suggests additional courses of action.     

Caring for the Disabled? New Boundaries in Disability Discrimination

The scope of the disability-related less favourable treatment provisions of the Disability Discrimination Act 1995 has been reduced by the House of Lords in a housing case, London Borough of Lewisham v Malcolm , while the European Court of Justice has taken a broad approach to disability discrimination and carers within the EC Framework Directive in Coleman v Attridge Law . In discussing both cases, this note suggests that the pre- Malcolm approach to identifying the comparator in disability-related discrimination claims should prevail in the employment context, in view of the obligations under the EC Framework Directive.
