Actions Necessary to Prevent Childhood Obesity: Creating the Climate for Change

Childhood obesity has become a public health epidemic, and currently a battle exists over how to frame and address this problem. This paper explores how public policy approaches can be employed to address obesity. We present the argument that obesity should be viewed as the consequence of a "toxic environment" rather than the result of the population failing to take enough "personal responsibility." In order to make progress in decreasing the prevalence of obesity, we must shift our view of obesity away from the medical model (which focuses on the individual) to a public health model (which focuses on the population). At the same time, we must be sensitive to the problem of weight bias. Potential obstacles to taking a public policy approach are identified, as well as suggestions on how to overcome them.     

THE DIVIDED WORLD OF THE CHILD: DIVORCE AND LONG‐TERM PSYCHOSOCIAL ADJUSTMENT

This study evaluated the extent to which divorce creates the “divided world of the child,” as well as consequences of this “divided world” for long‐term adjustment. An ethnically diverse sample of 1,375 young‐adult university students completed retrospective measures of parental nurturance and involvement, and current measures of psychosocial adjustment and troubled ruminations about parents. Results indicated that reports of maternal and paternal nurturance and involvement were closely related in intact families but uncorrelated in divorced families. Across family forms, the total amount of nurturance or involvement received was positively associated with self‐esteem, purpose in life, life satisfaction, friendship quality and satisfaction, and academic performance; and negatively related to distress, romantic relationship problems, and troubled ruminations about parents. Mother‐father differences in nurturance and involvement showed a largely opposite set of relationships. Implications for family court practices are discussed.     

Ensuring safety, implementation and scientific integrity of clinical trials: lessons from the Criminal Justice–Drug Abuse Treatment Studies Data and Safety Monitoring Board

Data and safety monitoring boards (DSMBs) provide independent oversight to bio-medical clinical trials, ensuring safe and ethical treatment of research participants, data quality, and credibility of study findings. Recently, the type of research monitored by DSMBs has been expanded to include randomized clinical trials of behavioral and psychosocial interventions in community and justice based settings. This paper focuses on the development and role of a DSMB created by the National Institute on Drug Abuse (NIDA) to monitor six multi-site clinical trials conducted within the Criminal Justice–Drug Abuse Treatment Studies (CJ-DATS). We believe this is one of the first such applications of formal DSMBs in justice settings. Special attention is given to developing processes for measuring and monitoring a range of implementation issues for research conducted within criminal justice settings. Lessons learned and recommendations to enhance future DSMB work within this area are discussed.     

A River,a Dam and A Hawthorn Tree

The Three Gorges area on the middle reaches of the Yangtze River is undoubtedly one of the most beautiful and exciting places in China,and the perfect destination to admire China's natural and manmade marvels.In May,I followed my inner siren call and visited the area on a sunny weekend with my boyfriend.We took a cruise on the Yangtze.The ship was swarming with domestic tourists and several foreigners,many of us leaning over the railings in awe.I had booked seats on the top deck,and we were not disappointed as the panorama of the Xiling Gorge passed us by along with the lush vegetation on the hills and teal waters of the Yangtze.     

Assessing civil competence in the elderly.

This study was designed to establish reliability of the Community Competence Scale-Revised (CCS-R) and provide evidence for its validity in making discriminations relevant to civil competence in the elderly. The CCS-R is an individually administered structured interview of 17 subscales. Criterion groups were formed by drawing a sample of research participants from a retirement complex with various levels of care and with nurses having extensive knowledge of the residents' level of functioning. The study demonstrated high reliability and found converging evidence for the effectiveness of the CCS-R in making discrimination about competence in the elderly. The study had added to the growing evidence that it is possible to standardize the assessment of civil competence, thereby making the adjudication process a more accurate one.     

Fatal complications related to diagnostic barium enema

Thousands of barium enemas are performed in the United States each year without incident. Occasionally complications arise, only a few of which are fatal. Perforation is the most frequent serious complication of this procedure. Two instances of fatal perforation, one colonic and the other vaginal, are described, and the relevant literature is reviewed.
