Two unusual cases of suicidal overdose of acetaminophen (paracetamol) without the usual extensive centrilobular necrosis of the liver are reported. Both cases were subjected to comprehensive drug screening by immunoassay, and a combination of gas chromatography with mass spectrometry, nitrogen detection, and electron capture detection. Acetaminophen was detected in both cases. No other drugs were detected in case #1, and only a small amount of olanzapine (<0.1 mg/L) was detected in case #2. No anatomical cause of death was identified in either case. If untreated, the normal outcome of a large acetaminophen overdose would be massive hepatic necrosis with delayed death and low blood and tissue acetaminophen concentrations. In contrast, particularly high postmortem acetaminophen concentrations were measured in both our cases with little hepatic tissue damage. For case #1, femoral blood acetaminophen 1280 mg/L, vitreous 878 mg/L, and liver 729 mg/kg; in case #2, cardiac blood 1220 mg/L, vitreous 779 mg/L, liver 3260 mg/kg, and gastric 11,500 mg/500 g. Acetaminophen was measured using high performance liquid chromatography with UV detection (254 nm) using 3-hydroxyacetanilide as the internal standard. The very high concentrations of acetaminophen is these cases but relatively little hepatic damage suggests an alternative, possibly cardiac, mechanism of death.  相似文献

Network neutrality issues have been vigorously debated worldwideover the past few years. One major aim of network neutralityproponents is to prevent high-speed Internet service providersfrom charging content providers for priority delivery. Recently,proponents have turned their attention to the regulation ofwireless networks, such as those for cellular phones, whichprovide increasing numbers of consumers with access to Internetservices. Some application providers have relied on a recentacademic paper to support greater regulation of wireless operators.Although the proposals to regulate these networks use the phrase"net neutrality," the regulations they seek to impose on wirelessoperators have little in common with those being sought forother Internet service providers. In this article, we providea framework for determining whether certain kinds of regulationsshould be imposed on the owners of wireless networks. We alsoconsider the benefits and costs of specific proposals for theregulation of these networks. Our principal conclusion is thatthe costs of most of these proposals are likely to exceed thebenefits.  相似文献

Despite the promising advances in the construction and use of social indicators, there has been little application to the formulation, monitoring, or evaluation of foreign policy. In the formulation stage—our concern here—predictor or early warning indicators could be very useful. The annual state of the world message contains many such predictive indicators of war, but in a purely verbal and intuitive form. Three of these (prior war, relative capabilities, and alliance levels) are converted into operational language and then put to the empirical test. In general, the indicators do not predict war (over the past 150 years) as postulated by the Administration. These tests are, however, very incomplete, and our objective is not to evaluate the Administration's arguments, but to suggest one way in which indicators could improve the quality of foreign policy formulation.This is a revised and abbreviated version of the paper originally prepared for the 1972 meetings of the American Political Science Association held in Washington, D.C. Skeptics argue that statements about right and wrong are merely expressions of preferences. They are mistaken; values are not the same as mere preferences. When we assert preferences that affect others, we justify our actions by giving reasons that we believe others should accept. When we evaluate those reasons, we typically reject certain preferences as illegitimate. Values are different from preferences because they entail demands we feel entitled to make of each other, after critical reflection. But this does not require us to become moral realists who base value judgments on the existence of moral facts. Critical normativity adopts the paradoxical stance that value judgments are based on human will but that they can be right or wrong. Human beings cannot live without strong normative claims but we are obligated to be careful about them. This article illustrates this stance by telling three parables of justice.

Certain governments have been faster than others in relaxing their restrictions on the cross‐border movement of capital. How can we explain the timing and extent of financial liberalization across countries since the 1970s? We argue that IMF stabilization programs provide a window of opportunity for governments to initiate financial reforms, but that policy makers are more likely to seize this opportunity when welfare expenditures are high. Large loans from the IMF shield policy makers from the costs of financial reform, while welfare expenditures provide credibility to the government's ex ante promises of compensation to individuals who are harmed by the reforms. We test this hypothesis on data for 87 countries from 1975 to 2002. We employ a spatial autoregressive error sample selection model which accounts for the nonrandom participation of countries in IMF programs as well as the processes of international policy diffusion. The results provide strong support for the interactive effect of IMF programs and domestic welfare expenditures on financial liberalization.  相似文献

This paper is a review of the literature that examines obesity in China. Multiple factors that contribute to this phenomenon are investigated, such as globalization, urbanization and cultural changes. Most of the researches reviewed in this paper concern urban, coastal populations of China. Obesity is defined differently for Asian populations; thus, diagnosis has been underestimated in the past. As China's economy develops, the country may now be gaining western problems, such as obesity. Obesity is related to economical causes as China participates in international business and trade. Furthermore, the role of the globalized food industry is reviewed. More western food companies and restaurants have infiltrated Chinese cities, causing mass changes in the traditional Chinese diet. Likewise, employment is a significant factor examined since more occupations in Chinese cities have transitioned to the service sector. Cultural attitudes differ from those of the West in which the Chinese view heavier bodies as healthier than thinner figures. Interventions that have been attempted are reviewed.  相似文献
