This paper is the first research report of a comprehensive study of AID as a medical treatment in the U.K. It examines the policies that determine who is able to receive AID when it is administered by doctors in medical clinics. Findings from the survey upon which this report is based indicate that AID treatment is administered under highly discriminatory policies. Women's access to treatment is restricted according to marital status, sexual preference, disability and costs. By operating under restrictive policies, doctor's discretion and medical jurisdiction extends beyond the immediate concerns of the health of patients and into the realm of social control.  相似文献

Mixed drug reactions are frequently encountered in emergency department overdose cases and also in fatal intoxications. Assessment of the relative contribution of each drug in producing adverse effects is often compounded by lack of case history and the paucity of cases reported in the literature. This report describes a fatal intoxication with three common over-the-counter medications: guaifenesin, diphenhydramine, and chlorpheniramine. A 48-year-old woman was found dead in the attic bedroom of her residence. Specimens obtained at autopsy for toxicologic analysis included heart blood, urine, bile, gastric contents, vitreous humor, and cerebrospinal fluid. The over-the-counter drugs were identified and quantitated by acid/neutral or basic liquid-liquid extraction followed by gas chromatographic analysis with nitrogen phosphorus detection. Concentrations of guaifenesin, diphenhydramine, and chlorpheniramine detected in the heart blood were 27.4, 8.8, and 0.2 mg/L, respectively. The cause of death was determined to be acute intoxication by the combined effects of guaifenesin, diphenhydramine, and chlorpheniramine, and the manner of death was determined to be suicide. To our knowledge, the blood guaifenesin concentration in this case is the highest reported concentration to date associated with an acute intoxication.  相似文献

In policy debates concerning large scale R&D efforts, the achievements of the Manhattan and Apollo projects are frequently cited as evidence of Yankee ingenuity and the ability to overcome technical obstacles. In this article, the factors which contributed to the success and failure of large scale crash development projects are analyzed systematically. Successes are distinguished from failures according to two criteria. First, while the successes are marked by parallel development of technological components which began only after the basic scientific and technical obstacles had been overcome and the basic feasibility had been demonstrated, in the failures, parallel development began much earlier. In addition, the successful crash projects, such as the atomic bomb effort and the moon program, were designed to meet static technical goals and did not depend on overcoming countermeasures. The unsuccessful projects, such as the Safeguard ballistic missile defense (BMD) system, failed in the face of changes in Soviet military technology.Using these criteria to analyze the Reagan administration's space-based ballistic missile defense program (SPBMD), the author concludes that despite the claims made by supporters, this R&D effort is not similar to the Manhattan or Apollo projects. Rather, like the Nuclear Airplane and Skybolt missile, parallel developments have begun prior to proof of feasibility, and like the Safeguard BMD, the SPBMD must adapt to countermeasures.  相似文献

This article deals with the role of government in encouraging the decline of radical movements. The question posed is: “Which story can the government tell to encourage the decline of radical groups and the disengagement of their members?” The article makes use of the survey of factors promoting decline and disengagement drawn up by Demant, Slootman, Buijs (? ?Deceased. ) and Tillie in 2008, as well as the factor “official policy strategies” based on concepts taken from discourse analysis, adapted to counterterrorism and deradicalization strategies by De Graaf in 2009. The article will therefore not address the different practical measures in this field, but focus instead on the perception of these official measures by the radicals. It will illustrate this with two case studies: the deradicalization of South Moluccan youths in the 1970s and of jihadist radicals after 2001, both in the Netherlands.  相似文献
