National Association of Medical Examiners position paper on the certification of cocaine-related deaths

The National Association of Medical Examiners Committee on Cocaine-related Deaths recommends that the following guidelines be applied in the process of documenting, interpreting, and certifying potential cocaine-related fatalities. The committee cautions that the investigation of any drug-related death requires a complete investigation of the circumstances of death, the death scene, and past medical history. It is also necessary to have the results of the forensic toxicological analysis and those of a complete forensic autopsy examination prior to formulating an opinion as to the cause and manner of death. Cocaine should be considered the underlying cause of the death when 1 or more of the following is true: (1). the circumstances surrounding the death can be associated with an acute cocaine exposure and there are no supervening causes of death; (2). the immediate cause of death is directly due to a readily identifiable mechanism or disease such as a gunshot wound or a stroke, yet the acute use of cocaine was the direct underlying cause of the trauma or the disease process; and (3). chronic cocaine use leads to a disease that results in an ultimately fatal pathologic process leading to organ injury and death. The committee further cautions that reported drug levels may not directly relate to the toxic or lethal effects of the drug upon the patient. These guidelines are intended for use by practicing medical examiners and physicians who certify drug deaths, as well as providing education tools for students.     

Use of liquid nitrogen to remove duct tape from a homicide victim.

Removal of duct tape or similar adhesive products from a homicide victim may be facilitated by rapidly chilling the tape surface with liquid nitrogen. Physical separation of tape layers can be performed using the same technique. Cyanoacrylate glue (i.e., "super-glue") may be used to preserve fingerprints on the outer surface of the tape for recovery, or other techniques may be used to recover fingerprints from the outer surface prior to tape removal.     

Changing development models in small economies: The case of Jamaica from the 1950s to the 1990s

By examining in detail the successes and failures of different development models in one developing country over a four-decade period, this article sketches a development model for small economies in the 1990s as an alternative to the neoliberal model pushed by the International Monetary Fund. It reviews the experience of Jamaica with various development models from the 1950s to the 1990s, with special attention focused on the experience of the Seaga government of the 1980s. It also draws lessons from the successful development experience of small European countries and of the East Asian Newly Industrialized countries. In normative terms, the alternative development model attempts to combine growth with equity and democracy. In analytical terms, it takes account of the constellation of domestic forces and appropriate political strategies, as well as of international economic and political conditions. The main features are a strong role for the state in economic interactions with transnational corporations, in identification of export markets and promotion of export production, in selective protection of domestic industry with an export potential, in promotion of agriculture linked to industrial development, in improvement of human resources and promotion of regional economic integration. Within these parameters, a crucial role is assigned to the domestic private sector and a complementary one to foreign investment. Distribution is to be addressed primarily through distribution of productive assets and access to health care and education. Rectal impaction following enema with concrete mix

This article describes an unusual rectal foreign body resulting from homosexual anal erotic activities. The patient had used an enema containing a concrete mix which became impacted and required surgical removal. The use, abuse, and complications of enemas are reviewed.     

Vitreous humor chemistry: the use of potassium concentration for the prediction of the postmortem interval

Upon review of the literature, extensive disagreement was found as to the usefulness of vitreous humor potassium concentration as a predictor of the postmortem interval (PMI). A pilot study of 1427 cases was performed to address this problem. The requisite statistical analysis for the prediction of PMI is inverse prediction. The 95% inverse prediction interval was found to be approximately +/- 20 h. The linear regression equation for the data was y = 0.238 chi + 6.342, with a coefficient of determination (r2) of 0.374. This r2 value means that 62.6% of the variation of potassium is unaccounted for by the variation in PMI. Further studies are required to attribute this unaccounted variation to quantifiable factors. This would narrow the inverse prediction interval and enable vitreous potassium to be a useful aid in the prediction of PMI.
