Obesity has attained epidemic proportions in the United States, with more than 50% of adults classified as overweight or obese. If untreated, morbidly obese patients have a 1 in 7 chance of reaching normal life expectancy. The surgical treatment of obesity has emerged as the most effective treatment modality in long-term weight control and has become increasingly popular, with attendant postoperative complications and death. We performed a cross-sectional, coroner based, 2-year retrospective review of archival case records for decedents who died following bariatric surgery for the treatment of obesity to identify underlying causes of death and forensic characteristics of this cohort. Fifteen (0.5%) out of 3097 archival cases died following bariatric surgery, with approximately 73% of decedents dying within 6 months of surgery. The underlying causes of death in 80% of decedents were natural comorbidities of obesity, with cardiovascular diseases (33%) being the most frequent causes of death, followed by gastrointestinal diseases (20%), acute pulmonary thromboembolism (13%), and acute bacterial pneumonia (13%). The majority of decedents were white females who remained morbidly obese after bariatric surgery. Only 2 decedents died of direct inadvertent/accidental surgical complications.  相似文献

The analysis of politics in antiquity presents new opportunities for political science and international relations, particularly the ancient New World ( c . 2000 B.C. to A.D. 1521). Governance through leadership and institutions, collective action, war and peace, alliance dynamics, regional hegemonies, interstate rivalries, and other universal patterns of world politicsexisted in Mesoamerica, antedating the modern state system. We report findings from a study to record systematically the rise and fall of Maya polities in the Mesoamerican political system, using sources from archaeology and epigraphy. Based on tests of competing hypotheses and new distribution statistics and hazard rates (survival analysis) for 72 Maya polities, our findingssupport a model of Maya political dynamics based on Preclassic origins, punctuated phases of development, multiple cycles of system expansion and collapse, and weaker political stability for increasingly complex polities. We draw two main implications: (a) a new theory of the Maya political collapse(s), based on their failure to politically integrate; and (b) confirmation for a newperiodization of Maya political evolution, different from the traditional cultural periodization, based on several cycles of rise-and-fall, not just one. Our findings may also make possible future investigations in areas such as the war-polarity and war-alliances hypotheses.  相似文献

The events of September 11, 2001, have raised troubling questions regarding the reliability and security of American commercial air travel. This article applies the concepts and logic of high–reliability organizations to airport security operations. Contemporary decision theory is built on the logic of limited or buffered rationability and is based on the study of error–tolerant organizations. The concept of high–reliability organizations is based on the study of nearly error–free operations. For commercial air travel to be highly secure, there must be very high levels of technical competence and sustained performance; regular training; structure redundancy; collegial, decentralized authority patterns; processes that reward error discovery and correction; adequate and reliable funding; high mission valence; reliable and timely information; and protection from external interference in operations. These concepts are used to inform early–stage issues being faced by both local airports and the newly established Transportation Security Administration.  相似文献

This study examines the effects of 14 estimator variables (e.g., disguise of robber, exposure time, weapon visibility) and system variables (e.g., lineup instructions, exposure to mugshots) on a number of measures of eyewitness performance: identification accuracy, choosing rates, confidence in lineup choice, relation between confidence and identification accuracy, memory for peripheral details, memory for physical characteristics of target, and time estimates. Subjects viewed a videotaped reenactment of an armed robbery and later attempted an identification. Characteristics of the videotape and lineup task were manipulated. Prominent findings were as follows: identification accuracy was affected by both estimator and system variables including disguise of robber, weapon visibility, elaboration instructions, and lineup instructions. Memory for peripheral details was positively correlated with choosing on the identification task but negatively correlated with identification accuracy.  相似文献

This article studies the development of informal bargaining tables to mitigate postelectoral conflicts in some 15 percent of Mexico's local elections between 1989 and 2000, even as formally autonomous electoral commissions and courts were being constituted. By documenting the dual institutions that resulted, the study qualifies theories of institutional design that take actor consent for granted. It argues that in the Mexican case and perhaps others, elections, particularly subnational elections, are focal points for informal bargaining over rules that are the true motors of protracted transitions. It finds electoral institutions to be critical to democratization, but for reasons beyond those given by most institutionalists.  相似文献

A general theory of interpersonal exchange is developed from a public goods, public choice point of view and within an analytical context that assesses both the benefits and the costs of social interaction. Social transaction benefits and costs are specified as a composite function of interpersonal bindingness, which itself depends upon five parameters. The model provides for the determination of an optimal personal degree of bindingness in a two-person relationship. Extensions encompass the more interesting reaction case, where two individuals each select their own optimal personal bindingness levels. Although some community violence research has examined the context of community violence, including the social, economic, and structural organization of neighborhoods, more needs to be learned about family, school, and community-level factors that may promote and lessen the incidence and prevalence of community violence. In addition, further research is needed on various social, environmental, and contextual factors hypothesized to protect youth from exposure. This article (a) reviews and examines the relation between neighborhood context and risk of violence exposure, (b) reviews current literature on predictors of community violence and mental health and behavioral consequences for children and families adversely affected by community violence, (c) examines sources of resilience and community strengths that extend beyond the individual, (d) discusses the contributions and limitations of current conceptualizations of risk and resilience, and (e) highlights directions for future research. Information from this review can inform community and government efforts to lessen community violence through prevention and treatment.  相似文献

This article seeks to explain how science has emerged as political brand. While science and politics have intersected for centuries, more recent social, cultural, and political events led to increased attention to the role of science in everyday life and how science is used in policy decision-making. This led to a tipping point in 2017 when the March for Science was formed, following what many in the U.S. and countries around the world viewed as anti-science stances by political leaders. The political spectacle of the March for Science not only brought increased attention to the scientific community, but also emerged to define the brand of science in society. Drawing on research from the role of brands in consumer culture – including political marketing, brand resonance, and brand community – I describe the implications of the science brand for the scientific enterprise, and the ways in which the scientific community consider the strategic communication of their brand within the political marketplace.  相似文献

This paper presents a self-contained survey of club theory with an emphasis on the contributions of the last fifteen years. Club goods are contrasted with pure public goods, and equilibrium and optimality notions are contrasted. After presenting three basic representations of club theory, we focus on recent advances and generalizations that include heterogeneous memberships, transaction costs, uncertain utilization, asymmetric information, and noncompetitive influences. An agenda for research is indicated that includes further analysis of asymmetric information, institutional structure, and applications.  相似文献

While over 60% of police agencies in the United States are either implementing or planning for community policing (Annan, 1995), actual implementation of community policing practices is disjointed and has produced more problems than solutions. One such problem is the resistance among officers to the philosophy itself. In a study of the orientations of noncommunity policing officers in the largest public safety department in the US, it is determined that elements of the organizational culture of the police agency are not significant predictors of support for the community policing philosophy. Rather, individual orientations toward citizen involvement in crime prevention and job involvement are predictors of officer support. Implications for police managers are discussed.  相似文献
