Health care reform became a premier issue on the U.S. policy agenda in the 1990s. While the comprehensive proposal put forth by President Clinton failed, states and the federal government successfully pursued a variety of lesser initiatives. This article focuses on a set of reforms intended to make private health insurance more accessible and affordable to individuals and workers in small firms. It outlines the key arguments made by experts to justify stronger regulation of health insurance and the options and difficult tradeoffs that must be considered in policy design. It then examines the scope and strength of legislation adopted by 45 states and the federal government from 1990 to 1996. The substantial variation in state policies demonstrates that even though insurance market reform was the one issue that commanded nearly universal support in the health care debate, few design features were universally accepted by those who crafted the reforms. The article concludes by assessing the pattern of state and federal action. The reforms represent some progress on nominal access to insurance but little progress on the affordability of insurance for individuals and small groups. Few of the reforms present a serious challenge to existing practices and interests of the insurance industry. This pattern of policy design reflects the logical and political constraints of incrementalism. In a system where insurance coverage is voluntary, changes to increase access for one group tend to increase costs and thereby decrease access for another segment of the population. In addition, because incremental reforms will not attract sustained attention and support from the general public, it is politically difficult to impose substantial new regulation on a powerful industry. 相似文献

Considerable research on ego identity has appeared over the past 15 years, indicating the need for an overall review. Part I commences by considering the complexity of Erikson's concept and suggesting several different theoretical contexts in which it has been used. The validity of investigators' attempts to operationalize the concept ego identity is assumed to depend in part upon their interpretations of its meaning. The subsequent review of empirical literature is organized according to different procedures which have been used to measure ego identity. A first section summarizes and evaluates research utilizing Q-sort and self-report questionnaire measures, while a second considers the large number of investigations that have employed James Marcia's Identity Status Interview. The latter group of studies are ordered according to the type of dependent variable (e.g., cognitive, behavioral, personality, developmental) examined in relation to ego identity status (i.e., achievement, moratorium, foreclosure, diffusion). Recent years have witnessed growing research interest in citizen perceptions of the police. This was likely the result of the community policing movement, which sought to strengthen ties between citizens and the police, as well as to concerns about the legitimacy of the police. Most of this research focused on police agencies in the United States, and little research had been conducted on citizen perceptions of the police in Asian countries. This study sought to fill this void through an analysis of citizen perceptions of the police in South Korea. To address these issues, telephone interview survey data gathered from 11,500 randomly selected citizens were obtained in the rapidly industrializing country, South Korea. Several sets of multiple-regression models were estimated to assess the effect of urbanization, demographics, evaluations of police services, and perceived corruption on citizen satisfaction with police. The results suggested that citizens in small to mid-sized cities and large urban cities were less satisfied with police than were residents of rural areas. In addition, age, evaluations of police service, and perceptions of corruption were related to citizen satisfaction. The results were discussed in comparison to previous research in Asia and the United States and other Western countries. 相似文献

Since 2016, the UK government has outlined plans for ‘Global Britain’ as a framework for post‐Brexit foreign policy. Some criticise the idea as a vision of ‘Empire 2.0’, but it is rarely made clear exactly what form it takes or what its wider political implications are. This article argues that Global Britain constitutes not just an idea or a slogan, but a foreign policy narrative and, more specifically, the narrative of empire. Indeed, to appear reasonable its grand ambitions require pre‐existing knowledges of past imperial ‘successes’ and accepting images of empire among the British public. Yet Global Britain lacks efficacy: as a domestic rather than an international narrative, by being inherently regressive in its worldview, and for contradicting the preferences of international partners on which the UK heavily relies.
