This study suggests that the prevalence of "silent" myocarditis may be higher in the pediatric population than is generally suspected and may contribute to a significant number of sudden and unexpected deaths in children, particularly those older than one year of age. The incidence of histologic myocarditis in children dying a violent death is similar to that reported as an incidental finding in adults.  相似文献

Can termination occur withboth a bang and a whimper? This article examines the termination of Oklahoma's State Training Schools for dependent and neglected children and the continuation of the policy of institutionalization and detention through the use of admission to psychiatric hospitals and treatment centers. This case study is discussed in light of Peter deLeon's model of termination and Herbert Kaufman's theory of organizational evolution, and demonstrates the resiliency of public policy, even in the face of organizational termination.  相似文献

A dating violence and sexual assault prevention program was presented to 396, predominately African American, middle schoolers in two inner city schools in the United States. In one school the program was offered with a same-gender group composition; in the other school, the same program was offered with mixed-gender group composition. A quasi-experimental design was used to assess how gender composition in programming influenced youths’ attitudes about dating violence. Study findings suggest that boys benefit most from participation in a same-gender program and girls benefit most from a mixed-gender program. Implications for prevention programming are discussed.  相似文献

In the United States, the Safe Drinking Water Act (SDWA) regulates most groundwater used for drinking water. The Act covers most urban areas but because it does not cover small water systems, it implicitly exempts nearly half of those living in rural America. In large measure, monitoring required by the SDWA has illustrated the prevalence of naturally occurring arsenic in groundwater in concentrated areas throughout the country. Even though many in Congress seem aware of this threat and have, indeed, supported more stringent arsenic standards, Congress, on the whole, has failed to update the SDWA to cover those water systems left unprotected by the Act. Conventional political science theory suggests that effective congressional oversight depends on Congress creating both active (e.g., hearings and commissioned studies) and passive oversight mechanisms (i.e., citizen suits and opportunities for constituent feedback). In this case, Congress had, in fact, created sufficient tools to detect a serious problem but, having identified it, nevertheless failed to respond. Why? In exploring Congress’s inaction, we find something unexpected: the structure of the SDWA has created perverse incentives not only for unregulated water systems but also for regulated systems to push to keep exempted water systems unregulated. The outcome is that those outside of the SDWA’s protections remain outside and continue to drink contaminated water by the glass full. So, while Congress created a loophole, it may have inadvertently tied a noose.  相似文献

Psychiatric disorders of 142 parents of adolescents who were documented as physically abused during adolescence and 168 parents of non-abused comparison adolescents were assessed. Fathers and mothers of physically abused adolescents exhibited higher rates of Axis I diagnoses and co-occurring disorders, and had more impaired GAS scores than comparison parents. Fathers of abused adolescents had greater lifetime incidence of Conduct Disorder and Substance Abuse/Dependence than comparison fathers. Mothers of abused adolescents had more unipolar depressive disorders than comparison mothers. This study highlights the importance of mental health assessments and interventions for parents of physically abused adolescents. Since onsets of parental psychiatric disorders antedated the abuse, early recognition and treatment of parental disorders may contribute to prevention of the onset and/or reoccurrence of adolescent physical abuse.

Since the 1980s, the USA has fought cocaine in the Andes with carrots and sticks: interdiction and crop eradication wield the sticks, while Alternative Development (AD), which offers economic assistance to farmers who voluntarily abandon illicit cultivation, provides the carrots. Yet cocaine continues to permeate US streets, and rural Andean communities remain isolated from the legitimate economy. Many critics blame US belligerence for compounding the Andean drug war. The underlying problem with the existing strategy, however, might not be the aggressiveness of its military sticks, but the flimsiness of its development carrots. The inability of AD to persuade farmers to abandon coca cultivation may be causing US policy makers to over-apply military solutions – often inflaming rural communities and exacerbating regional instability in so doing. Few legal crops can match the earning power of coca. The article therefore suggests that the US carrot could be made more attractive by adopting a Venture Development model which helps rural farmers to process their legal produce into high-quality finished goods that command premium prices. Such a strategy could conceivably choke the cocaine engine by applying market-based forces to address market-based realities.  相似文献
