Mr. Spaeth writes about the relationship between hospital administration and the physician, and how that relationship affects the quality of medical care delivered to the patient. The article focuses on the differences between the employment structure in an academic teaching hospital, and the open, independent contractor medical staff typical of a traditionally smaller community hospitals. The individual traits and nuances of these structures and how they can be distinguished from one another create dynamic differences in the approach for quality care. Peer review, credentialing, and management of adverse outcomes are just a few of the ways in which hospitals continue to strive to provide higher quality care, but the way these methods are implemented and performed in different hospital structures are dramatically different, yielding distinct results. Mr. Spaeth argues that the challenges faced by community hospitals in their effort to provide higher quality care and eliminate medical error are exacerbated because of their unique structure and the particular relationship physicians share with the hospital and its administration. 相似文献

This case study charts the classic transformation of a small business organisation from being a vehicle of protest that attracted a reasonable but transient membership into a much larger group with a more stable membership and a group with an effective insider policy style. The paper asserts that the change in style and the change in recruiting success are not causally linked, and, indeed, it claims that an insider style may harm recruiting. In the case of the Federation of Small Businesses (FSB), however, any potential damage through adopting an insider style was more than offset by the separate decision to market the group door to door with a package of selective material incentives (Olson 1965). The paper describes the predominant insider politics style of political representation and finds that while the FSB has moved in that direction, it does not fully fit the stereotype. This article notes the systemic lack of conceptual clarity in the social sciences and attempts to illustrate the adverse consequences by closer examination of the particular example of the interest group field. It indicates the significant ambiguities implicit in the term. Not all policy-influencing organisations are interest groups as normally understood, but because there is a lack of an appropriate label the term interest group is used by default. The article seeks to distinguish between interest groups and other policy relevant bodies—often corporations or institutions. It finds disadvantages in adopting a functional interpretation of the interest group term (i.e. any organisation trying to influence public policy). While the wider range of organisations are crucial in understanding the making of public policy, it is confusing to assume that this wider population are all interest groups. The article instead advances the complementary notions of pressure participant, policy participant and interest group. This slightly expanded repertoire of terms avoids conflating important distinctions, and, in Sartori's term permits 'disambiguation'. Any democratic society requires mechanisms for citizens to have effective political voice. Clearly, political parties provide a key channel for expressing views and preferences. However, organised interests provide another important mechanism for such representation. A crucial question in this regard is whether the interest group system is capable of ensuring the representation of a variety of public and private interests. Resolving these debates requires data that map the terrain and also are attentive to organisational diversity. This article takes up this challenge through exploring the composition and diversity of the Australian system of organised interests, using a new data set based on the Directory of Australian Associations. This system‐level approach delivers important insights into the nature of the Australian interest group system, as well as provides a framework for subsequent work interpreting and contextualising advocacy activities of particular groups, or lobbying dynamics in specific policy domains. 相似文献

AbstractThis article examines the relationship between neighborhood quality, residential instability, employment access, location affordability, and work outcomes among individuals relocated as part of the Boulevard Homes HOPE VI redevelopment in Charlotte, North Carolina. We found that, contrary to expectations, relocation to private-market units with vouchers, as compared with public housing, did not always result in better neighborhood outcomes. Whereas voucher holders relocated to better quality neighborhoods, relocatees who moved to other public housing lived in neighborhoods with better employment access and lower costs. This paper is concerned with the end of 71 years of single party government in Mexico. The paper explores the ways in which the opposition harnessed modern campaign techniques to the opportunities presented by democratisation to secure regime transition. A number of campaign features, such as the stress on personality and negative campaigning, have attracted considerable criticism, but the author argues that there are grounds for believing the 2000 election to have been a unique election, and that competition should be more clearly structured around parties and issues in the future. A fundamental challenge facing applied time-series analysts is how to draw inferences about long-run relationships (LRR) when we are uncertain whether the data contain unit roots. Unit root tests are notoriously unreliable and often leave analysts uncertain, but popular extant methods hinge on correct classification. Webb, Linn, and Lebo (WLL; 2019) develop a framework for inference based on critical value bounds for hypothesis tests on the long-run multiplier (LRM) that eschews unit root tests and incorporates the uncertainty inherent in identifying the dynamic properties of the data into inferences about LRRs. We show how the WLL bounds procedure can be applied to any fully specified regression model to solve this fundamental challenge, extend the results of WLL by presenting a general set of critical value bounds to be used in applied work, and demonstrate the empirical relevance of the LRM bounds procedure in two applications. 相似文献

AbstractThe critical race theory has been predictive of how minority youth are treated in the juvenile and criminal justice systems in the United States. However, the theory has not been applied in explaining the existence of wrongful convictions among juveniles. Using secondary data derived from the National Exoneration Registry, the purpose of this study is to identify specific factors (e.g., DNA evidence, etc.) related to the wrongful convictions of Black youth who have been exonerated. Compared to other racial categories, the results reveal that Black youth are more likely to experience wrongful convictions as a result of false confessions, faulty eyewitness identification, perjury, and official misconduct. Limitations, policy implications, and areas of further investigation are offered. 相似文献
