Fatal epiglottic abscess after radiotherapy for laryngeal carcinoma

ABSTRACT: Acute epiglottitis is a rare condition, especially in adults, largely owing to the widespread vaccination against Haemophilus influenzae. Traumatic epiglottitis as a result of thermal or caustic insults is documented. Epiglottic abscess formation is described as a sequela of epiglottitis in some cases. The development of epiglottic abscess from epiglottitis secondary to radiotherapy has previously been described in the literature; however, there are no reports of fatalities due to epiglottic abscess after radiotherapy. We present an unusual case of an adult sudden death due to epiglottic abscess formation subsequent to radiotherapy for laryngeal squamous cell carcinoma.     

Co-producing a post-trafficking agenda: collaborating on transforming citizenship in Nepal

This article discusses how a new agenda on post-trafficking is gaining momentum through academic and activist anti-trafficking collaborations focused on co-producing knowledge with women who have returned from trafficking situations. Co-production of this nature is important as the issues raised by post-trafficking scenarios are largely ignored in anti-trafficking strategies, and the stigmatisation and poverty which women in these circumstances encounter means they rarely have a voice in policy-making. Drawing on research in Nepal, we present four types of co-produced data around transforming citizenship post-trafficking, and reflect on the strategies for generating and using them for advocacy purposes.     

Writing the regulations for health

The development of regulations in health is much more than a purely technical procedure. Rather, it involves decisionmaking and bargaining processes that engage a wide range of individuals and organizations with diverse interests and varied resources. Attempts at statutory precision and goal clarification may improve, but will not fundamentally alter the regulation writing experience. The dynamics of regulations development are revealed in three cases. The primary case, The Health Maintenance Organization Act of 1973, suggests that despite the efforts of the regulations writers themselves, there are limits to the extent to which basic interest conflicts and questions of health policy can be resolved in the process. Other cases examined are the Professional Standards Review Organization Act of 1972 and the National Health Planning and Resources Development Act of 1974.
