The right to health in public health: is this a new approach?

The international human rights community has frequently reaffirmed the importance of full respect for the right to health. However, there remains a large gap between the standards set out in Art 12 of the International Covenant on Economic, Social and Cultural Rights and the situation prevailing in most countries. While there has been a substantial amount of work done to expand upon the content of the right to health, there has been little consideration of what a right to health means for public health programming generally. This article addresses this issue by considering the compatibility of the essential elements of contemporary public health programming practice with the principles contained in General Comment No 14, para 43(f) (a minimum core obligation of the right to health). The article considers that while both contemporary public health practice and para 43(f) do contain compatible elements, the right to health brings with it new elements of monitoring (through right to health indicators) and accountability, both of which will require new tools to be adopted by the public health sector.     

Deep finance: sovereign debt crises and the secondary market ‘fix’

Links between ballooning national debts and official aid conditionalities are well understood. Private sovereign debt markets, in contrast, have been largely overlooked outside mainstream economics. Yet, since the 1980s, a ‘secondary market regime’ in sovereign bonds has transformed financial geographies and legal practices, facilitating the build-up of riskier sovereign debts and strengthening specialized distressed debt investors. This secondary market ‘fix’ has enabled the international financial system to muddle through successive crises. Using the recently concluded Argentine debt saga as an example, I show how the secondary market regime’s effects on debtors are intertwined with, but distinct from, those of structural adjustment programmes; in cases of resistance to neoliberalism, ‘deep’ secondary market debt structures serve to push countries (back) into the neoliberal fold.     

Experience in the transfer of handpump technology to less developed countries

Many of the world's nations have neither adequate water supplies nor sufficient sanitation facilities. The United Nations, therefore, designated 1981–1990 as the International Drinking Water Supply and Sanitation Decade with the explicit goal of providing the world's poor with access to adequate water supplies and proper sanitations. Over the past eight years, the International Division of the Engineering Experiment Station at Georgia Tech has been helping the world's poorer countries assess their need for handoperated water pumps, develop effective hand pump programs, and manufacture, install, maintain and monitor the pumps. The purpose of this paper is to provide a methodology for technology transfer that is applicable to hand pumps as well as to numerous other hardware items.
