Medicaid waivers play an important role in the health policy process. Various health care policy initiatives have encouraged states to experiment ith their Medicaid programs by expanding coverage to populations not traditionally covered by Medicaid, or by delivering Medicaid services in non traditional ways. Lessons learned by individual states' Medicaid waivers could be an important part of the health care reform debate. This paper presents an evaluation of a Medicaid waiver operating in South I Carolina. The South Carolina Community Long-term Care Waiver for the Elderly and Disabled is evaluated for cost effectiveness, quality of care, and client satisfaction. This evaluation shows that the waiver is highly effective in offering individuals who are medically and financially eligible for Medicaid supported nursing home care a viable, cost effective, high quality alternate to institutionalization.  相似文献

Employing evidence from the history of the comparable worth movement, I argue that (1) the State-citizen shaping process is one in which States shape citizens and movements and citizens and movements shape States; (2) States are not always successful in shaping movements because movemnts require a certain level of both stakes and consciousness on the part of potential members; (3) States are more effective at influencing whether a movement will win than they are at actually producing social movements; and (4) the costs and benefits associated with the various institutional channels impact heavily upon a movement, particularly upon its organization and its alliances. These theoretical propositions provide clues as to why the comparable worth movement has been able to persist at the state and local level despite Federal opposition.  相似文献

The work of a small and unusual activist group in the north Indian state of Rajasthan has raised a series of practical and theoretical issues concerning the best means for combating specific instances of corruption, and for promoting accountability more generally. The Mazdoor Kisan Shakti Sangathan (MKSS)-literally-:Workers' and Farmers' Power Organisation-has waged a campaign to secure the right of ordinary people to gain access to information held by government officials. In the process of experimenting with methods of compiling, sharing and verifying expenditure data at very local levels-thus far, in the absence of a statutory entitlement to such informationthe MKSS has developed a radical interpretation of the notion that citizens have a right both to know how they are governed and to participate actively in the process of auditing their representatives. This article examines the process by which this campaign emerged and the means by which it pursues its goals. It then analyses the implications of the MKSS experience, and the larger movement it has spawned, for contemporary debates in three areas: human rights, participatory development and, of course, anti-corruption.  相似文献
