Death certificates are the source for mortality statistics and are used to set public health goals. Accurate death certificates are vital in tracking outcomes of cancer. Deaths may be certified by physicians or other medical professionals, coroners, or medical examiners. Idaho is one of 3 states that participated in a Centers for Disease Control and Prevention-funded study to assess the concordance between cancer-specific causes of death and primary cancer site among linked cancer registry/death certificate data. We investigated variability in the accuracy of cancer death certificates by characteristics of death certifiers, including certifier type (physician vs coroner), physician specialty, years of experience as death certifier, and number of deaths certified. This study showed significant differences by certifier type/physician specialty in the accuracy of cancer mortality measured by death certificates. Nonphysician coroners had lower accuracy rates compared with physicians. Although nonphysician coroners certified less than 5% of cancer deaths in Idaho, they were significantly less likely to match the primary site from the cancer registry. Results from this study may be useful in the future training of death certifiers to improve the accuracy of death certificates and cancer mortality statistics.  相似文献

The current study used a random sample of 502 men and women to investigate the intergenerational transmission of violence and assess the impact of family-of-origin violence on later adult intimate partner violence (IPV) perpetration and victimization while controlling for a host of theoretically-relevant factors and demographic characteristics. Six multivariate logistic regression analyses were modeled to identify differential correlates depending upon type of violence perpetration and victimization. Findings indicated a significant relationship between family-of-origin violence and psychological perpetration and victimization. The acceptance of violence in relationships significantly correlated with physical violence perpetration. Finally, being married, older, and employed protected against several forms of interpersonal violence. Policy implications are discussed.  相似文献

Over the next decade an increasing number of new "pharmacotherapy" medications will become available with the potential to tremendously impact the use and abuse of illegal drugs and the overall direction of national and international drug policy. These pharmacotherapy medications are designed to block or significantly reduce the "highs" elicited by illegal drugs. Used as part of a drug treatment program, pharmacotherapy medications may provide valuable assistance for people voluntarily seeking a chemical aid in limiting or eliminating problem drug use. However, the tremendously politicized nature of the "drug war" raises substantial concerns that, in addition to those who voluntarily choose to use such medications, some people will be compelled to use them. This article concludes that in the absence of extraordinary circumstances, governmental action forcing or coercing a person to use a pharmacotherapy drug would violate a number of important legal rights. Among the rights implicated by compulsory use of pharmacotherapy drugs are the right to informed consent, the right to bodily integrity and privacy, the protection against cruel and unusual punishment, and the right to freedom of thought.  相似文献

Identification of 6-acetylmorphine, a specific metabolite of heroin, is considered to be definitive evidence of heroin use. Although 6-acetylmorphine has been identified in oral fluid following controlled heroin administration, no prevalence data is available for oral fluid specimens collected in the workplace. We evaluated the prevalence of positive test results for 6-acetylmorphine in 77,218 oral fluid specimens collected over a 10-month period (January-October 2001) from private workplace testing programs. Specimens were analyzed by Intercept immunoassay (cutoff concentration=30 ng/ml) and confirmed by GC-MS-MS (cutoff concentrations=30 ng/ml for morphine and codeine, and 3 ng/ml for 6-acetylmorphine). Only morphine-positive oral fluid specimens were tested by GC-MS-MS for 6-acetylmorphine. A total of 48 confirmed positive morphine results were identified. An additional 107 specimens were confirmed for codeine only. Of the 48 morphine-positive specimens, 32 (66.7%) specimens were positive for 6-acetylmorphine. Mean concentrations (+/-S.E.M.) of morphine, 6-acetylmorphine and codeine in the 32 specimens were 755+/-201, 416+/-168 and 196+/-36 ng/ml, respectively. Concentrations of 6-acetylmorphine in oral fluid ranged from 3 to 4095 ng/ml. The mean ratio (+/-S.E.M.) of 6-acetylmorphine/morphine was 0.33+/-0.06. It is suggested that, based on controlled dose studies of heroin administration, ratios >1 of 6-acetylmorphine/morphine in oral fluid are consistent with heroin use within the last hour before specimen collection. The confirmation of 6-acetylmorphine in 66.7% of morphine-positive oral fluid specimens indicates that oral fluid testing for opioids may offer advantages over urine in workplace drug testing programs and in testing drugged drivers for recent heroin use.  相似文献

Multidimensional scaling analysis was used to examine adolescents' cognitive appraisals of major and daily stressful events. The desirability of events was the only salient feature for early adolescents. Middle and late adolescents also appraised events in terms of their desirability; in addition, the amount of impact that events exerted on their lives and the generality of the causes of events were salient dimensions for these age groups. These results suggest increased complexity with age in adolescents' cognitive appraisals of stressful events. Correlational analyses also indicated that perceptions of personal coping capabilities are associated with high event desirability, high frequency of occurrence, stable causes, and decreased ability of others to be of help in coping.This research served as a portion of the doctoral dissertation of the first author and was supported in part by an institutional award from the University of Vermont to the second author.Received his Ph.D. from University of Vermont. Combining local council election data with fiscal data on grant allocations in a German state, we study partisan favoritism in the allocation of intergovernmental transfers within a quasi-experimental framework. We hypothesize that state governments pursue two distinct goals when allocating grants to local governments: (1) helping aligned local parties win the next election and (2) buying off unaligned municipalities that may obstruct the state government’s policy agenda. We argue furthermore that the relative importance of these two goals depends on local political conditions. In line with this argument, we show empirically that the effect of political alignment on grant receipts varies depending on the degree of local support for the state government. While previous contributions find that aligned local governments always tend to receive larger transfers, our results imply that the political economy of intergovernmental transfers is more intricate.  相似文献

Abstract: Air navigation service providers (ANSPs) play a critical role in the operation of a modern economy. Many countries have restructured their ANSPs by granting managerial and financial autonomy and creating new mechanisms for regulation and accountability to major consumers of air navigation services. These reforms have exemplified principles often associated with the New Public Management. The United States is the major exception to this international trend. Commercialization has allowed significant improvements in service quality without substantial increases in cost of service or erosion of safety standards. Other public interest considerations have also been protected. These performance benefits can be attributed to key decisions on the governance of new air navigation service organizations. Sommaire: Les fournisseurs de services de navigation aérienne jouent un rôle crucial dans le fonctionnement d'une économie moderne. De nombreux pays ont restructuré leurs fournisseurs de services en leur accordant une autonomie financière et de gestion, et en créant de nouveaux mécanismes de réglementation et d'imputabilité envers les principaux consommateurs de services de navigation aérienne. Ces réformes ont illustré les principes souvent associés à la Nouvelle gestion publique. Les États‐Unis sont la principale exception à cette tendance internationale. La commercialisation a permis d'apporter d'importantes améliorations à la qualité des services sans entraîner pour autant d'augmentations substantielles des coûts de services, ou une érosion des normes de sécurité. D'autres considérations d'intérêt public ont également été protégées. Ces avantages en termes de performance peuvent être attribués à des décisions clés en matière de gouvernance des nouveaux organismes de services de navigation aérienne.  相似文献
