Medical underservice: The source and the potential of the special district solution

This paper is concerned with the appropriateness of various structural approaches t o the problem of medically underserved areas (MUAS). The failure of the private sector in this regard and the experience with public sector responses to the problem are noted. The paper compares the merits of applying the device of the special district to MUAs with arguments favoring a general government solution. The experience of the state of Arizona with Health Service District legislation is observed.

The logic of the special district and the nature of medical underservice offerslim hope that this device might materially ameliorate the problem. A t the same time, the past initiatives of general purpose government have not served t o erase medical underservice. The point is made t h a t empirical evidence is needed t o fully test the assumptions underlying the arguments of the respective approaches.     

Women's employment and industrial organisation: commercial lace embroidery in early nineteenth-century Ireland and England

Abstract

The early nineteenth century saw expanding work opportunities for women in commercial lace embroidery in Britain. This article traces the connection between the development of commercial lace embroidery in several locations – Nottingham, Essex and Limerick. Despite the fame of the Irish industry, it has received almost no academic attention. The differing structures of the Irish and English industries are examined. Aspects of lace manufacture highlight the increasing emphasis on cleanliness and the respectability of women's work in the nineteenth century. The authors suggest that to appreciate fully the impact of the Industrial Revolution on women's employment opportunities, we must look to the periphery of the national economy, as well as the centre.     

Corruption and Popular Support for Democracy and Government in Transitional Contexts: The Case of Kyrgyzstan

Abstract

Many studies have argued that corruption undermines perceived democratic legitimacy, trust in state institutions, and government support. However, few prior studies have included transitional or hybrid regimes in their analyses, or simultaneously examined multiple aspects of this relationship. Using focus groups and a nationally representative survey conducted throughout a tumultuous transitional period in Kyrgyzstan, we find evidence for our hypotheses that citizens with greater corruption concerns will be more pro-democratic when evaluating democracy or its institutional components in the abstract; yet, they distrust existing government institutions. Even in a hybrid regime lacking democratic experience, citizens exhibit attributes of distrusting democrats.     

Our kids: Unaccompanied minors and the question of care

The question of care and what it means both conceptually and practically in the designated arena of child and youth care is a vexing one in the 21st century. Without a doubt, there are increasing numbers of young people who are either demonstrably in need of care or perceive themselves to be either lacking adequate care or simply existing outside those social spaces where care is available. Perhapsthere is no better exemplar of the contested set of relations that is contemporary childhood than those young people traversing national borders. Some of these young people travel with family members, but an ever-increasing number travel unaccompanied by parents or any other form of adult relation. Of these, a group particularly at risk are those without legal documentation. We would argue that this group of young people is urgently in need of our attention as child and youth care workers and scholars.
