The rate for the sudden infant death syndrome (SIDS) in Cape Town, South Africa, is estimated to be among the highest in the world (3.41/1000 live births). In several of these areas, including those of extreme poverty, only sporadic, nonstandardized infant autopsy, and death scene investigation (DSI) occurred. In this report, we detail a feasibility project comprising 18 autopsied infants with sudden and unexpected death whose causes of death were adjudicated according to the 1991 NICHD definitions (SIDS, n = 7; known cause of death, n = 7; and unclassified, n = 4). We instituted a standardized autopsy and infant DSI through a collaborative effort of local forensic pathology officers and clinical providers. The high standard of forensic investigation met international standards, identified preventable disease, and allowed for incorporation of research. We conclude that an effective infant autopsy and DSI protocol can be established in areas with both high sudden unexpected infant death, and elsewhere. Postmortem diagnosis of early myocardial infarctions is an ever recurrent problem in pathology. In the present study we determined the troponin I expression in 46 autopsy hearts using an immunohistochemical technique. Troponin I has, as a specific cardiac muscle protein, become a widespread used marker in testing patients with acute chest pain. The hearts were divided into three groups based on the macroscopical findings: definite signs of infarction, possible signs of infarction and no signs of infarction. All 14 cases of definite myocardial infarction showed a well-defined area with loss of troponin I. Twenty-three of 24 cases of possible myocardial infarction also showed a well-defined area with loss of troponin I. None of the eight non-cardiac death controls showed loss of troponin I expression. The results suggest troponin I expression as a sensitive test in diagnosis of early myocardial infarction.  相似文献

With a two-dimensional concept of 'New Public Management' as its point of departure, the article points to the development of a specific Danish model of NPM at the local level of government. In the municipalities the market-oriented NPM dimension has been almost absent and the managerial dimension has been interpreted and translated into a 'governance'-oriented model that combines decentralized self- and user-governance from below with centralized goal-steering from above. This combined model institutes new governing roles including a new leadership role for elected councillors as central goal-steering decision and policy makers. Rather than strengthening the local councillors, the new leadership role has turned out to be problematic for the elected councillors. The problems inherent in the new institutional role as goal-steering decision makers are discussed and arguments are put forward in favour of a more 'governance' and less NPM and government-oriented role for elected councillors. What seems to be needed is another new role that stresses local councillors as co-governors and guardians of an inclusive and democratic form of local governance.  相似文献

Presumably, anger is a common experience of parenting. Although practitioners and researchers recognize the role of anger in various parenting situations, objective and standardized measures of parental anger have been notably lacking in the field. This study examined the Parental Anger Inventory (PAI), a measure developed specifically to assess parental anger in response to child misbehavior. A diverse sample of 98 parents participated in the study, including (a) physically abusive or neglectful parents, or both, n = 44; (b) nonmaltreating clinic parents seeking assistance for child behavior problems, n = 24; and (c) nonmaltreating, non-help-seeking community parents, n = 30. Results support the internal consistency, temporal stability, and convergent validity of the PAI. Findings also demonstrate the PAI's potential utility when working with maltreating and help-seeking parents. Results highlight the importance of assessing parental anger toward children and support the use of the PAI in assessment, treatment, and research.  相似文献

We challenge the civic–ethnic dichotomy drawn by previous authors and propose a four-category typology of identities based on out-group tolerance and in-group attachment. Drawing from work on national identity formation and nation-building, we test hypotheses about the processes that cause individuals to adopt one identity over others using survey data based on representative samples of five ethnic groups in Ukraine. We find that the effects of socialisation processes vary greatly depending upon ethnic group. Our results challenge some long-held assumptions about the potential destabilising effects of ‘ethnic’ identities and the degree to which ‘civic’ identities correspond to values and behaviours supportive of democracy.  相似文献

Employees’ organizational commitment is associated with a number of positive outcomes, such as employee motivation, job satisfaction, productivity, and retention. But previous research has suggested that there may be important sector differences in both commitment and its explanations, yet results are very mixed. We study sector differences in affective organizational commitment among 1,043 public and private office workers who perform very similar tasks across sectors, testing the influence of factors thought to explain commitment and expected to vary between the sectors. We propose that the mixed results are due to different variables mediating the relationship between sector and organizational commitment. Conducting a multiple mediation analysis, we find that more hierarchy negatively mediates the relationship between public sector employment and organizational commitment. On the other hand, the relationship between public sector employment and organizational commitment is positively mediated by public service motivation and participatory management. This highlights the importance of not only studying simple sector differences, but also including relevant individual and organizational-level variables that can explain the mechanisms behind such differences.  相似文献

This article investigates how election information such as opinion polls can influence voting intention. The bandwagon effect claims that voters ‘float along’: a party experiencing increased support receives more support, and vice versa. Through a large national survey experiment, evidence is found of a bandwagon effect among Danish voters. When voters are exposed to a news story describing either an upwards or downwards movement for either a small or large party, they tend to move their voting intentions in the according direction. The effect is strongest in the positive direction – that is, when a party experiences increased support, more follows. Consistent effects are found across two different parties for a diverse national sample in a political context very different from earlier research on the bandwagon effects. Considering previous research and the fact that evidence is not found that suggests that the effect of polls vary across sociodemographic groups, the results imply that bandwagon behaviour is based not on social or political contingencies, such as media or political institution, but on fundamentals of political cognition.  相似文献

Interest in public values has grown considerably over the past two decades. Much of this attention reflects a growing awareness that public values hold considerable significance for citizens and public employees. Yet, despite the rapid expansion of research on public values, we still know little about the role of race in shaping and determining public employees’ values preferences. To begin remedying this gap, this article examines whether minority and white public managers in large US local governments exhibit the same value preferences when making departmental decisions. Results from a multiple group confirmatory factor analysis indicate that minority and white managers express similar preferences for traditional public administration values; however, minority managers report a stronger preference for both traditional public administration (e.g., efficiency and effectiveness) and social equity‐oriented (e.g., equity, representation) values.  相似文献

Abstract

African poverty statistics depend on household-level measurements from survey data, making the definition of household of critical importance. Detailed case studies from Tanzania and Burkina Faso explore (1) understandings of household membership and ambiguities, and (2) how well survey definitions capture households as economic units, and the implications for household size and responses to and mitigation of poverty. We develop an analytic framework of ‘open’ and ‘closed’ households. ‘Open’ households cope with poverty using flexibility, movement and extra-household networks, but are poorly represented by survey data. Closed households are likely to be better described by survey data.  相似文献
