Spontaneous Coronary Artery Dissection: Case Report and Literature Review

Spontaneous coronary artery dissection (SCAD) is a rare cause of acute myocardial infarction and sudden cardiac death. It occurs most commonly in otherwise healthy women during pregnancy or the postpartum period. The true incidence of SCAD is underestimated, as most cases are diagnosed at autopsy. The pathophysiology of SCAD is still not fully understood, and its management can be challenging. This report describes a 35‐year‐old pregnant female who presented with an acute antero‐lateral ST elevation secondary to spontaneous dissection of the left anterior descending artery and the circumflex artery. The diagnosis was established by coronary artery angiography. However, the patient died following cardiac tamponade. The examination of this case represented a starting point for the reviewing of the diagnosis, clinical course, and management of SCAD, and for the placing of this in context with the existing literature. This study highlights the importance of prompt diagnosis and subsequent lifesaving treatment.     

Adolescent Marital Expectations and Romantic Experiences: Associations With Perceptions About Parental Conflict and Adolescent Attachment Security

This study tested associations between adolescent perceptions of interparental conflict, adolescent attachment security with parents, and adolescent marital expectations and romantic experiences. Participants were 96 early adolescent females from 2 parent families. Insecurity was examined as a mediator of the association between negative perceptions of parental conflict and romantic outcomes. Results supported the mediation model in which adolescents' negative perceptions of parental conflict was associated with insecure attachment with parents, which was in turn associated with negative marital expectations and romantic experiences. Implications for understanding how parent-adolescent and interparental variables influence adolescent marital expectations and romantic experiences are discussed.

Invited Commentary: National Safe Place: Meeting the Immediate Needs of Runaway and Homeless Youth

An estimated 1.6 million youth run away from home each year. While on the run, these youth are vulnerable to exploitation, victimization, increased dangers and perpetration of criminal behavior. Runaway and homeless youth are far more likely to engage in substance use and delinquent behavior, drop out of school and suffer from sexually transmitted diseases and mental illness at greater rates than the norm. Timely and direct intervention in runaway and throwaway cases is imperative to protect youth from the high risks of living on the streets. National Safe Place is an outreach and prevention program that is uniquely designed to provide immediate safety and access to services for any youth in need. In partnership with over 360 youth serving agencies and over 10,000 businesses and community organizations across the United States, the Safe Place program educates youth about alternatives to running away and homelessness and provides easily accessible links to service providers. Ongoing data collection indicates that National Safe Place has been successful in reaching endangered youth at risk of abuse, neglect or serious family problems but that expanded program models remain needed. The challenges and successes of current programming and the future of National Safe Place program expansion are discussed.
