Fournier Gangrene and Unexpected Death

Abstract: Fournier gangrene represents a rare but progressive perineal infection that may result in rapid death. A 70‐year‐old man with poorly controlled diabetes mellitus and alcohol abuse is reported who was found unexpectedly dead. He had last been contacted the night before his death. At autopsy, the most striking finding was deep necrotic ulceration of the scrotum with exposure of underlying deep muscles and testicles, with blood cultures positive for Escherichia coli. Death was, therefore, attributed to necrotic ulceration/gangrene of the perineum (Fournier gangrene) that was due to E. coli sepsis with underlying contributing factors of diabetes mellitus and alcoholism. In addition there was morbid obesity (body mass index 46.9), cirrhosis of the liver, and marked focal coronary artery atherosclerosis with significant cardiomegaly. Fournier gangrene may be an extremely aggressive condition that can result in rapid death, as was demonstrated by the rapid progression in the reported case.     

The High Representative and directoires in European foreign policy: the case of the nuclear negotiations with Iran

ABSTRACT

The negotiations with Iran about its nuclear programme have seen the most protracted involvement of the High Representatives of the European Union in a high-profile policy case. This article traces the evolution of the High Representatives’ participation in the negotiations, from the first contacts with the E3 (Germany, France and the United Kingdom) in 2003 to the adoption of the Joint Comprehensive Plan of Action in 2015. It focuses on the institutional role the High Representatives played in relation with the directoires leading the talks with Iran – first the E3 and, since 2006, E3/EU. In this context, it examines the personal and organisational factors that affected the influence each of the three High Representatives (Javier Solana, Catherine Ashton and Federica Mogherini) had in regard to the directoires. The analysis distinguishes specifically between the phases before and after the 2009 Lisbon Treaty. The article shows how similar personal qualities of the three High Representatives in terms of problem-solving and trust-building gave them political capital that enabled them to adopt a fairly constant role as bridge-builders within the directoires and between the directoires and other actors. The reforms of the Lisbon Treaty had only a minor impact.     

西尼罗病毒E蛋白的原核表达及其多克隆抗体的制备

利用PCR技术扩增西尼罗病毒(WNV)E蛋白基因并将其插入原核表达载体pET-30a(+),构建重组质粒pET-30a(+)-WNV-E,并转化至大肠杆菌BL21(DE3)感受态细胞中,经IPTG诱导目的蛋白表达,对诱导条件进行优化,利用His镍柱亲和层析法纯化目的蛋白,纯化后的蛋白经SDS-PAGE与Westernblot鉴定正确后,免疫BALB/c小鼠制备多克隆抗体并进行间接免疫荧光鉴定。结果显示:WNV E蛋白在大肠杆菌中以包涵体形式被成功表达,蛋白最佳诱导条件为37℃,0.8 mmol/L IPTG诱导5 h,经间接免疫荧光鉴定多克隆抗体能特异性识别WNV E蛋白。本研究成功表达并纯化了WNV E蛋白,证明该蛋白具有良好的免疫原性,为WNV抗原、抗体检测方法的建立和新型疫苗的研发及相关研究奠定了基础。     

Mary Wollstonecraft’s divergent attitudes towards trust

ABSTRACT

Mary Wollstonecraft is increasingly being recognized as a philosopher who made a noteworthy contribution to moral and political philosophy. Her work not only encompassed political treatises, such as the now well-known A Vindication of the Rights of Woman, but also fiction. This article demonstrates how Wollstonecraft’s work comprised three main ways of engaging with trust, namely distrust, virtuous trust, and open trust. It puts these three forms of trust into context with Wollstonecraft’s ambivalent relation to the 18^th-century culture of sensibility. Moreover, Wollstonecraft’s open form of trust is compared with the 20^th-century Danish theologian and philosopher K. E. Løgstrup’s conception of trust. Løgstrup regarded genuine trust as a spontaneous, basic phenomenon that was not rooted in moral reasoning. While there are some similarities between Wollstonecraft’s open trust and Løgstrup’s understanding of trust, Wollstonecraft ultimately reinforced the value of ‘an educated heart’, namely the idea that the feelings of the heart should be cultivated by reason. Accordingly, this article offers some insight into how we may perceive Wollstonecraft’s strong rejection of Edmund Burke’s ‘inbred sentiments’, that is, Burke’s belief in innate, benign moral instincts.

鸡柔嫩艾美耳球虫哈尔滨株3-1E基因的重组表达及其表达产物的免疫保护性

应用RT-PCR技术扩增柔嫩艾美耳球虫哈尔滨株子孢子表面抗原3-1E基因,再克隆至质粒载体pMD18-T中,获得重组质粒pMD18-T-3-1E,采用EcoR I+Hind III双酶切法及PCR扩增鉴定正确后,将3-1E基因cDNA目的片段亚克隆到原核表达载体pET-32a(+)中,获得重组质粒pET-32a-3-1E,用EcoR I+Hind III双酶切法及DNA测序证明cDNA序列完全正确.表达蛋白的SDS-PAGE分析表明,表达产物与预期大小(36.47 ku)一致,为可溶性表达.对诱导时间以及IPTG浓度的优化结果表明,当诱导6 h且IPTG浓度在0.8 mmol/L时表达量最大.Western-blot分析表明,表达的蛋白可被感染柔嫩艾美耳球虫鸡的阳性血清特异性识别.将重组3-1E蛋白纯化后分为肌注蛋白组和肌注蛋白加弗氏完全佐剂组免疫AA鸡,通过计算抗球虫指数检测其保护力.结果显示,肌注重组蛋白组与肌注重组蛋白加弗氏完全佐剂组均能刺激鸡体产生一定的免疫反应,对柔嫩艾美耳球虫感染产生有力的保护,其中后者优于前者,表明该重组3-1E蛋白具有研制成亚单位疫苗的潜力.     

A Comparison of Four Presumptive Tests for the Detection of Blood on Dark Materials

Detection of blood on dark materials is difficult for crime scene examiners so presumptive tests are used to assist. This study compared the ability of luminol, leuko crystal violet, tetramethylbenzidine, and Combur Test®E to detect whole, diluted blood (1:100) and a key‐shaped blood transfer stain (1:10), on dark cotton sheeting, tea towel, socks, synthetic carpet, and car mats. Powdered bleach was used to evaluate specificity of the blood detection tests. Sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and overall misclassification rate (OMR) assessed the quality of the blood tests. Luminol was the preferred test for diluted blood having the highest sensitivity (79%–96%), NPV (66%–93%), and the lowest OMR (3%–15%). Luminol was also found to be most efficient with a testing time on 25 items of 2 h 50 min compared with up to 8 h. Overall, luminol was the most effective method, also providing information on bloodstain patterns.
