Fournier Gangrene and Unexpected Death

Abstract: Fournier gangrene represents a rare but progressive perineal infection that may result in rapid death. A 70‐year‐old man with poorly controlled diabetes mellitus and alcohol abuse is reported who was found unexpectedly dead. He had last been contacted the night before his death. At autopsy, the most striking finding was deep necrotic ulceration of the scrotum with exposure of underlying deep muscles and testicles, with blood cultures positive for Escherichia coli. Death was, therefore, attributed to necrotic ulceration/gangrene of the perineum (Fournier gangrene) that was due to E. coli sepsis with underlying contributing factors of diabetes mellitus and alcoholism. In addition there was morbid obesity (body mass index 46.9), cirrhosis of the liver, and marked focal coronary artery atherosclerosis with significant cardiomegaly. Fournier gangrene may be an extremely aggressive condition that can result in rapid death, as was demonstrated by the rapid progression in the reported case.     

Stereochemical Analysis of Methorphan Using (−)‐Menthyl Chloroformate

Abstract: Forensic laboratories around the world have seen an increase in the number of drug seizures containing methorphan. Levomethorphan is a narcotic and a controlled substance, and its enantiomer dextromethorphan is an antitussive agent used in over‐the‐counter medications. For the forensic analysis of seized drugs containing methorphan, it is important to report the stereochemical composition of the drug. Ideally, a method based on common forensic laboratory instrumentation is desirable. The use of the chiral derivatizing agent (?)‐menthyl chloroformate followed by routine gas chromatography–mass spectrometry analysis of the derivative was shown to successfully determine the stereochemical composition of methorphan. This approach was applied to a street seizure containing methorphan proving that it was pure dextromethorphan. The derivatives of dextro‐ and levomethorphan were subjected to mass spectroscopic and nuclear magnetic resonance analysis, which confirmed that the structure of the derivatives remained unchanged as a result of the derivatization process.     

重型颅脑损伤数字计算ERP特征及与智商的相关性

目的 根据ERP理论和韦氏智力量表内容建立数字计算ERP范式,探讨正常人与重型颅脑损伤患者数字计算ERP特征及与智商的相关性.方法 采用Neuroscan-ERP仪和韦氏智力量表对29名重型颅脑损伤患者(外伤组)和46名正常人(对照组)进行ERP及智商检测,比较引出的ERP成分特征及与智商的相关性.结果 数字计算范式的题目、正确、错误答案3类刺激引出的ERP波形包括外源性成分N1c、P2c及内源性成分N2c和P3c;对照组引出的N1c和P2c波幅和潜伏期相近,但N2c和P3c有明显差异,其中正确答案的P3c波幅最高,错误答案的P3c潜伏期最长;外伤组引出的各ERP成分波幅降低、潜伏期延长,N2c和P3c具有显著性差异(P＜0.05);对照组错误答案引出的P3c潜伏期与言语智商和总智商呈中度负相关,高于题目和正确答案;外伤组P3c潜伏期与言语智商、操作智商和总智商的相关系数分别为-0.64、-0.46和-0.60(P ＜0.05),明显高于对照组.结论 数字计算ERP范式可引出特征性ERP波形,其P3c潜伏期与智商有一定相关性,可作为评价重型颅脑损伤后数学能力损害程度的参考指标.

骨骺愈合程度作为推断青少年活体年龄的重要手段之一,在相关鉴定中得到了广泛的应用.随着技术发展,国内外众多学者利用骨骺愈合程度推断年龄的研究取得了很多成果,本文针对根据骨骺愈合程度推断年龄的方法的产生、发展、研究成果以及有待解决的问题等进行综述,以期为相关研究和实践提供参考.     

A proposed dissection procedure for vertebral arteries in forensic pathology

Vertebral artery removing constitutes a significant forensic pathology challenge. Dissection techniques during head-neck autopsy are based on anterior approach, a difficult method, which is unable to assess the transverse part of the artery. This work presents an original and simple method for dissecting vertebral arteries by a posterior approach, opening the vertebroarterial canal through the spinal canal without any special equipment. Once the spinal cord is removed, the transversarium foramens are opened by an internal cut at the pedicle and an external cut at the transverse process. This enables us to visualize vertebral arteries in its entirety. The method improves both the examination of the upper extracranial segment of the vertebral artery and the neuropathological study when arterial injury is suspected. Applying this method routinely is both feasible and useful in suspected cases of vertebral artery trauma and could contribute to assess more precisely the actual incidence of this injury.     

大鼠闭合性脑损伤后MAP-2的时序性表达

目的探讨大鼠闭合性颅脑损伤后脑组织微管相关蛋门－2（MAP－2）表达变化规律。方法建立大鼠闭合性颅脯挫伤模型,64只Wistar大鼠随机分为损伤后0．5h、6h、12h、ld、3d、7d、14d7个损伤组及对照组,每组8只。应用免疫组化和蛋h印记Western blot法及图像分析技术,检测脑挫伤后各设定时间点挫伤脯组织中MAP－2的变化。结果免疫纽化染色显示：对照组大鼠脑组织中可见MAP－2阳性表达;实验组伤后0．5h,挫伤灶及周同MAP－2阳性表达下降,伤后6h阳性表达降至最低,伤后12h,阳性表达逐渐增多,伤后14d,仍保持较高水平;蛋白印记结果与免疫组化结果一致。统计学分析显爪：MAP－2的表达各损伤组与对照组比较,差异均有统计学意义（P〈0．05）,伤后12h、3d及14d与上…组比较,差异有统计学意义（P〈0．05）。结论大鼠脑挫伤后14d内,MAP－2存脑组织挫伤灶及其周同呈现先减少后逐渐增多的表达变化,可作为推断脑挫伤经过时间的参考指标之一。     

MAP-2在大鼠脑损伤后表达变化的时间规律性研究

目的探讨MAP-2在大鼠脑损伤后表达变化的时间规律性。方法以355.09kPa冲击应力致大鼠局灶性脑损伤后,在不同时间应用原位杂交、免疫组化、免疫组化双染色技术检测MAP-2 mRNA和蛋白表达情况。结果 MAP-2mRNA在对照组及假手术组均有表达,伤后30 min表达增强,伤后2d,达到峰值,并维持高水平表达至伤后7d。MAP-2蛋白在对照组及假手术组均有表达,伤后3h开始呈下降趋势,伤后1d,达到最低值,伤后2d起,表达逐渐恢复,伤后15d,恢复至对照组水平。结论脑损伤后MAP-2 mRNA及蛋白表达具有时间规律,可用于法医学推断脑损伤形成时间。     

物证鉴定证据应用层级论

用物证鉴定结论证明犯罪需要经过推理过程,将物证来源鉴定结论转化为嫌疑人行为推论,后者再作为法律事实证明嫌疑人是否有罪。物证鉴定证据应用的递进证明关系,可以用由依次推理关系连接的物证来源结论层、行为推论层和定罪层构成的物证鉴定层级模型描述。该模型可直观表达和评估物证鉴定结论证据应用效力。     

Multiplex short tandem repeat amplification of low template DNA samples with the addition of proofreading enzymes

Abstract:  With <100 pg of template DNA, routine short tandem repeat (STR) analysis often fails, resulting in no or partial profiles and increased stochastic effects. To overcome this, some have investigated preamplification methods that include the addition of proofreading enzymes to the PCR cocktail. This project sought to determine whether adding proofreading polymerases directly in the STR amplification mixture would improve the reaction when little template DNA is available. Platinum Taq High Fidelity and GeneAmp High Fidelity were tested in Profiler Plus? STR reactions alone and in combination with AmpliTaq^® Gold. All reactions included the additional step of a post‐PCR purification step. With both pristine low template DNA and casework samples, the addition of these polymerases resulted in comparable or no improvement in the STR amplification signal. Further, stochastic effects and artifacts were observed equally across all enzyme conditions. Based on these studies, the addition of these proofreading enzymes to a multiplex STR amplification is not recommended for low template DNA work.
