The purpose of this article is to provide an example of recent public administration reform in Afghanistan. In 2003, the Afghan Ministry of Public Health (MoPH) established the Basic Package of Health Services (BPHS) and other health services. To date, service delivery has been conducted largely by NGOs, funded by the main international donors, and managed by the Grant and Contracts Management Unit (GCMU) of the MoPH. Despite these advancements, Afghanistan continues to face significant challenges in health service provision and policy. In 2009, the GCMU was restructured into the Health Economics and Financing Directorate (HEFD), which broadened the scope of the Unit to conduct important economic analyses. This article examines the MoPH's role in health financing as a developing institution engaged in applied health economics and policy analysis. This development will allow the MoPH to build evidence for policy-making and further establish its stewardship role in the health sector. 相似文献

This study examines the implementation of a hospital cost savings program in a Swedish county hospital to understand how and why a downsizing intervention works and what outcomes it produces. Data were gathered from interviews and documents. Substantial differences were found among the healthcare professionals’ perceptions of the downsizing process. These diverse perceptions likely limited the scope of staffs’ participation and innovations of new savings strategies. Our findings stress the need for impact analyses addressing both intended aims and unintended side effects. Full attention should be paid to communication processes required to establish a trustful and committed change climate. 相似文献

Citizens in democracies are expected to make better decisions if they understand policy tradeoffs. However, politicians rarely have incentives to communicate them; citizens are uncomfortable choosing among valued outcomes; and devising a common metric is difficult. It is not surprising that in the United States the environment provides relatively little cuing or priming of tradeoffs in television news. Russian citizens, on the other hand, face a media environment in which tradeoff cuing is intentionally suppressed by owners' agendas, yet viewers detect concealed tradeoffs even in the absence of tradeoff priming and viewpoint diversity. Analysis of discourse among ordinary Russians in 16 focus groups convened in four cities, differentiated by political reform and media market environments, showed that when watching news in which tradeoffs are thoroughly concealed, viewers challenge stories by offering a broad spectrum of uncued tradeoffs. Tradeoffs come from diverse policy domains and represent a range of cognitive strategies, some of which are considerably more abstract than others and link elements of their observations and assumptions (together with what they can extract from the stories) into complex reasoning outcomes. 相似文献

Abstract Uncontrolled outbursts of violence, of both an individual and group nature, are distinctive elements of social and economic changes occurring in Poland. They affect all social environments and institutions in this country, and unfortunately many people believe in violent solutions of problems (at least as represented in the media). In 2010, the Conservative‐Liberal Democrat Coalition placed a moratorium on airport expansion in the south‐east of England. In office, however, it has faced a sustained political campaign from supporters of the aviation industry and expansion, leading to the appointment in September 2012 of the Davies Commission on airport capacity. This paper critically evaluates this nascent policy reversal in aviation policy, analysing the political backlash in favour of expansion and the political mediation of such demands by the Coalition. It argues that while the shifting political context has placed new pressures on the coalition, its current difficulties cannot be divorced from the continued resonance of the logic of aviation expansion embedded in British institutions at the end of the Second World War. The paper concludes with an assessment of the challenges facing the Davies Commission, the coalition and campaigners, when set against the continued ‘grip’ of aviation on our collective consciousness. 相似文献

ABSTRACTWhen environmental NGOs in Australia successfully sued the nation’s environment minister in August 2015 to temporarily withhold environmental approval for Australia’s largest coal mine, the ruling Coalition government accused environmentalists of waging “lawfare.” Through a critical discourse analysis of Parliamentary debate and media coverage, this article explores the lawfare battles fought in Australia in 2015, arguing that these were a site of depoliticization, in Mouffe’s (2005, Mouffe, Chantal. 2005. On the Political. Abingdon: Routledge) sense of the term. By exploring how the question of legal regulation of coal mining was rationalized, moralized, and stripped of significant political or ideological differences, this seeks to add to our understanding of processes of depoliticization by considering metadiscourses concerning “the law.” 相似文献

Abstract This paper interrogates influential contemporary accounts of interdisciplinarity, in which it is portrayed as offering new ways of rendering science accountable to society and/or of forging closer relations between scientific research and innovation. The basis of the paper is an eighteen-month empirical study of three interdisciplinary fields that cross the boundaries between the natural sciences or engineering, on the one hand, and the social sciences or arts, on the other. The fields are: 1) environmental and climate change research, 2) ethnography in the IT industry and 3) art-science. In the first part of the paper, in contrast to existing accounts, we question the idea that interdisciplinarity should be understood in terms of the synthesis of two or more disciplines. We stress the forms of agonism and antagonism that often characterize relations between disciplinary and interdisciplinary research, and distinguish between three modes of interdisciplinarity. In the second part we outline three distinctive logics or rationales that guide interdisciplinary research. In addition to the logics of accountability and innovation, we identify the logic of ontology, that is, an orientation apparent in diverse interdisciplinary practices in each of our three fields towards effecting ontological transformation in the objects and relations of research. While the three logics are interdependent, they are not reducible to each other and are differently entangled in each of the fields. We point to the potential for invention in such interdisciplinary practices and, against the equation of disciplinary research with autonomy, to the possibility of forms of interdisciplinary autonomy. 相似文献
