滋阴潜阳法对肾性高血压大鼠神经免疫调节的影响

目的：观察滋阴潜阳法对二肾一夹肾性高血压模型大鼠(2K1C—RHR)的治疗作用及神经免疫调节，从神经免疫调节角度探讨滋阴潜阳法的作用机制。方法：复制2K1C—RHR模型，用滋阴潜阳中药治疗5周后，用放射免疫法测定血管紧张素Ⅱ(AngⅡ)、神经肽(NPY)、神经降压素(NT)及白细胞介素(IL4)含量。结果：滋阴潜阳法明显降低模型大鼠血中AngⅡ、NPY、IL-4含量，而升高NT含量。结论：滋阴潜阳法能明显调节肾性高血压大鼠AngⅡ、NPY、IL-4、NT含量，其神经免疫调节可能是其发挥治疗作用的机制之一。

芪白平肺胶囊治疗慢性阻塞性肺疾病

目的 探讨芪白平肺胶囊治疗慢性阻塞性肺疾病（chronic obstructive pulmonary disease,COPD）合并抑郁障碍的疗效。方法 将58例COPD合并抑郁障碍患者随机分为治疗组（29例）与对照组（29例）,在常规治疗基础上,治疗组与对照组分别加用芪白平肺胶囊与氟哌噻吨美利曲辛,观察两组患者血液流变性、T淋巴细胞亚群及汉密尔顿抑郁量表（Hamilton depression scale,HAMD）评分的变化。结果 治疗组在降低COPD合并抑郁患者的HAMD评分,降低全血高切、低切黏度及血浆比黏度,升高CD4+细胞百分比,降低CD8+细胞百分比方面显著优于对照组（P<0.05,或P<0.01）。结论 芪白平肺胶囊能改善COPD抑郁障碍患者血液流变性,调节T淋巴细胞亚群,降低HAMD评分,对COPD患者有较好疗效。     

车辆部件损坏与事故关系的技术鉴定

目的通过一起交通事故车辆转向器总成损坏与事敞关系鉴定的案例,介绍如何对车辆部件损坏情况与事故发生是否有关进行技术鉴定。方法从痕迹学、车辆工程学等方面,对提供的检材进行检验,根据检验结果,结合转向器总成构造及工作原理,对事故车辆转向器损坏与事故关系进行综合分析。结果排除了事故车辆因转向器故障而诱发事故的可能性。结论道路交通事故车辆部件损坏与事故关系鉴定需要多学科、多专业的技术支撑。     

North Korea’s parallel economies: Systemic disaggregation following the Soviet collapse

This article aims to introduce the concept of parallel economies to explain the transformation of North Korea’s command economy during the 1990s. The article summarises North Korea’s pre-1991 command economy, before identifying the collapse of the Soviet Union and the great famine (1994-1998) as causes for the splintering of the old command system into parallel economies—the official, military, illicit, court and entrepreneurial economies—separated from the central planning matrix. It concludes that the existence of parallel economies makes system-wide economic reform unlikely and increases the importance of foreign aid in maintaining the viability of the regime’s political architecture.     

Electoral contexts that impede voter coordination

An electorate can effectively coordinate on the viable parties in its district, or a relatively large proportion of its voters may “waste” their votes on parties that fail to obtain office. Avoiding wasted votes can be more or less difficult depending on several characteristics of the electoral context. Unfortunately, many of the features of electoral contexts hypothesized to cause vote wastage have been tested piecemeal on only a handful of (non-random) cases and/or with data inappropriately aggregated to the national, rather than the district, level. Based on results from 2007 districts in 183 lower chamber elections across 21 countries, we find evidence that new electoral rules, the entry of new parties, past electoral volatility, and high district magnitudes are all likely to make coordination a challenge, with entry by new parties having the largest, most consistent effect.     

Ochronosis Presenting as Methemoglobinemia

Ochronosis is the blue‐gray discoloration of collagen‐containing tissues due to homogentisic acid (HGA) deposition, secondary to endogenous alkaptonuria or exogenous enzyme inhibition. In renal disease, accumulation of HGA in serum can cause methemoglobinemia. A 60‐year‐old woman with renal disease and anemia presented with 3 days of weakness and months of gray skin discoloration. Her hemoglobin was 8.1g/dl with 24.5% methemoglobin. Despite treatment with methylene blue, exchange transfusion, and continuous renal replacement therapy, the patient died. Autopsy revealed gray discoloration and ochronotic pigment in the ribs and cartilage. Based on these findings, the patient was diagnosed with ochronosis, suggestive of alkaptonuria, complicated by methemoglobinemia. The differential diagnosis for blue‐gray skin discoloration includes argyria, methemoglobinemia, and ochronosis. This patient's clinical and autopsy findings suggested alkaptonuria complicated by methemoglobinemia due to progressive renal dysfunction. Development of methemoglobinemia in the setting of chronic skin discoloration and renal failure should prompt consideration of alkaptonuria.
