The paper deals with the recentdevelopments in New York City's social policy. Itanalyses the present system of welfare benefits in thecity as well as the local effects of the 1996 federalPersonal Responsibility and Work Opportunity Reconciliation Act. Itcan be shown that the impressive reductions of thewelfare caseload are the result of a conservativephilosophy that aims at the political utilization ofthe welfare clientele.     

The Vocal Euro‐outsider: The UK in a Two‐speed Europe

The EU is divided between member states that have adopted the euro and those that have not. This article looks at the issue of differentiated integration with particular reference to eurozone integration and the euro‐outsiders. I explore the recent public debate in the UK on euro‐outsiderness, comparing this with debates in Denmark. The article highlights some striking differences between the UK and Denmark when it comes to the actual management of euro‐outsiderness in Brussels as well as some of the dilemmas facing euro‐outsiders such as the UK, Denmark, Sweden and Poland as the EU struggles to exit its crises. Finally, I discuss the future of two‐speed European integration and the UK's possible exit from the EU. The UK cannot escape the dilemma of favouring either influence or autonomy; whether the UK remains in or leaves the EU, it will need to allow the eurozone to proceed in order to prevent further eurozone crises.     

Troublesome Peace Making: How American Views on Terrorism Affected Norwegian Mediation in Sri Lanka, 2000–2009

Weak mediators normally need to borrow leverage from more powerful players to provide efficient mediation. This situation requires strong co-operation between the weak mediator and more powerful actors or coalitions involved in a peace process. But what if this co-operation fails? This analysis demonstrates how lack of productive co-operation with the United States negatively affected Norway’s 9-year-long attempt to mediate in the conflict between the Sri Lankan government and the Liberation Tigers of Tamil Eelam between 2000 and 2009. More specifically, it explores how the United States views on terrorism in the aftermath of the terrorist attacks on 11 September 2001 limited Norway’s room for diplomatic manoeuvre. Based on a unique set of classified sources from the Norwegian Ministry of Foreign Affairs and interviews with key actors, the analysis demonstrates how diverging perceptions of a conflict can make mediation painfully challenging.     

Sudden cardiac death in young adults: environmental risk factors and genetic aspects of premature atherosclerosis

Familial hypercholesterolemia (FH) is a genetic disorder that may lead to premature coronary heart disease (CHD) and sudden cardiac death (SCD). Mutations in the LDLR or APOB genes cause FH. We have screened the LDLR and the ligand-binding region of APOB genes in 52 cases of SCD. Deceased patients were younger than 40 years of age and were suspected of having FH. The LDLR and APOB genes were examined via PCR, high-resolution melting, and DNA sequencing. Therein, it was observed that 7.7% of the screened patients exhibited a rare sequence variant in the LDLR gene, with 5.7% suspected of being pathogenic mutations. Lipid profiles and genetic testing for FH could be considered when autopsy reveals significant atherosclerosis of the coronary arteries in young adults. First-degree family members are advised to seek medical advice and testing to determine their own risks of atherosclerosis to prevent premature CHD and SCD.     

An exploration of citizens' professionalism in coproducing social care services

Citizens' involvement in public service delivery challenges the principles of professionalism as such citizens are amateurs. However, there is little evidence of the (non)integration of these principles by citizen coproducers and how this affects professionalism in public service delivery. This article focuses on two principles of professionalism: expertise and accountability. The theoretical framework further reviews the coproduction literature on what can be expected of citizens with regard to these principles and elaborates on the concept of amateurism. The concepts of professionalism and amateurism form the framework for the analysis of citizen coproducers' identity. Empirically, this paper presents two case studies of social services in the European context. The results show that these citizen coproducers to a certain extent create a professional identity, tend to stay away from integrating accountability, and introduce elements of amateurism. The presence and guidance of public servants in coproduction can ensure accountability and streamline amateurism.     

Physical Restraint and Near Death of a Psychiatric Patient

Physical restraint is used as a last resort emergency measure to calm and safeguard agitated and/or aggressive psychiatric patients. This can sometimes cause injuries, and rare fatalities have occurred. One mechanism of injury and death while in physical restraint is that of severe asphyxiation. We present the case of a hospitalized man in his mid‐30s, suffering from schizophrenia. The patient was obese. He became aggressive and had to be manually restrained with a “takedown.” After having been put in the prone position on the floor with a significant weight load on his body, he lost respiration and consciousness. Subsequently, he was given CPR. He regained consciousness and respiration, while the cyanosis receded in 1–2 min. Psychiatrists and pathologists should be aware that physically restraining a patient in the prone position with a significant weight load on the torso can, in rare cases, lead to asphyxiation.
