Misdiagnosis of child abuse and neglect can delay early treatment. Some authors have pointed out that nurses can miss child abuse and neglect diagnoses due to a lack of knowledge. It is unclear whether the lack of knowledge is due to students' insufficient preparation in nursing school and/or a deficiency in continuing education. An 18-item questionnaire was administered to final-year nursing students to assess their degree of knowledge on child abuse and neglect and to evaluate if the lack of knowledge was due to insufficient teaching/training during nursing school. The students were also asked to evaluate themselves by assigning a score to their knowledge. A statistical comparison was performed to define whether sufficient/insufficient results were associated with the following variables: sex, pediatric or general nursing student, attending pediatric lectures, training in pediatric wards/ambulatories, and attending specific lectures on child abuse and neglect. The study population comprised 175 students (154 females, 20 males, 1 unknown). Exactly 66.3% of the participants had ≤9/18 correct answers. Of all students, 77.7% self-evaluated their level of knowledge as ≤5/10. The comparisons yielded statistically significant differences between the groups with sufficient objective knowledge and those unrelated to training in pediatric wards/ambulatories or pediatric nursing students. Overall, there was little objective knowledge on the subject, which may be related to insufficient teaching/training in nursing schools. Useful corrective strategies include further teaching on child abuse and neglect, preferably using a practical approach. Further, common teaching/training programs should be conducted by both pediatric and general nursing schools.  相似文献

Efforts to significantly reduce the deficit must focus on the largest componenet of federal spending, entitlements such as social security, federal retirements, and medicare. Entitlements account for 45 percent of federal spending. About two-thirds of the growth in these programs arises from automatic cost of living adjustments (COLAs). Each one percent COLA increase adds $23 billion to the deficit over the next two decades.

It is debated whether COLA restraint should be included in deficit reduction efforts. Advocatesargue that COLAs are a major cause of deficits , tha trestraint is fair after a decade in which benefits grewfaster than in flation and wages, and that restraint improves the solvency of major programs. COLA restraint also reduces the deficit quickly, spread s the burden of deficit reduction over a very large group, and allows nominal benefits to remain at current levels, or even to grow, albeit more slowly. Opponents of restraint argue that COLAs are a fundamental program feature, that COLAs are needed to maintain beneficiaries purchasing power, and that recipients would experience economic hardship if COLAS were reduced.

Common approaches to restrain COLAs emphasize either short-term budget savings or long-term program reform. Past successful efforts to restrain COLAs have been related to ensuring program solvency or ensuring equal treatment of individuals in different programs.Proposals to slow COLA growth to achieve budgetary savings haven ot been successful.

Three types of COLA options seem viable under current economic and political conditions: A one-year COLA freeze, a cap on the COLA at two-thirds the rate of

inflation, and a cap on the COLA at two percentage points less than the rate of inflation. The one-year COLA freeze reduces the deficit most quickly, but produces less savings in the long-run than the other options.

A major attraction of formula reductions in the COLA is that the concept may be extended to other areas of the federal budget and greatly reduce the federal deficit. A plan to limit growth to two percentage points less than in flation on COLAs, other entitlements,national defense, discretionary domestic programs, and income tax indexing would reduce the deficit by one-half by 1990. An overall budget reduction plan based on CCLA restraint may be the only approach left for policy-makers who have exhausted other approaches to reducing the deficit.  相似文献

A palynological study was carried out on 28 corpses brought in one year (June 2003-May 2004) to the morgue of the Institute of Legal Medicine of Parma (Northern Italy). This preliminary research focuses on the date of death, which was known for all corpses examined. Pollen sampling and analyses were made with the first aim of comparing the pollen grains found on corpses with those diffused in the atmosphere in the region in the same season as the known date of death. Eyebrows, hair-line near the forehead, facial skin and nasal cavities were sampled. Most of the corpses had trapped pollen grains, with the exception of two December corpses. All pollen grains were found with cytoplasm and in a good state of preservation. In this way, a series of reference data was collected for the area where the deaths occurred, and we examined whether pollen grains on corpses could be an index of the season of death. To verify this hypothesis, the pollen analyses were compared with data reported in the airborne pollen calendars of Parma and the region around. Pollen calendars record pollen types and their concentrations in the air, month by month. The quantity of pollen recorded on corpses did not prove to be directly related to the quantity of pollen in the air. But qualitatively, many pollen types which are seasonal markers were found on corpses. Main corpse/air discrepancies were also observed due to the great influence that the local environmental conditions of the death scene have in determining the pollen trapped by a corpse. Qualitative plus quantitative pollen data from corpses appeared helpful in indicating the season of death. A preliminary sketch of a "crime pollen calendar" in a synthetic graphic form was made by grouping the corpse pollen records into three main seasons: A, winter/spring; B, spring/summer; C, summer/autumn. Trends match the general seasonal trend of pollen types in the air.  相似文献

We present results of an evaluation of Reading for Life (RFL), a diversion program for nonviolent juvenile offenders in a medium‐sized Midwestern county. The unique program uses philosophical virtue theory, works of literature, and small mentoring groups to foster moral development in juvenile offenders. Participants were randomly assigned to RFL treatment or a comparison program of community service. The RFL program generated large and statistically significant drops in future arrests. The program was particularly successful at reducing the recidivism of more serious offenses and for those groups with the highest propensity for future offenses.  相似文献
