A kinetic model describing the pharmacokinetics of ethyl glucuronide in humans

The glucuronide conjugation is a minor pathway of ethanol metabolism. The determination of ethyl glucuronide (EG) in serum and urine has gained importance in forensic and other legal decisions. To prospectively calculate the serum concentration of this non-oxidative ethanol metabolite, the computer program developed includes a parameter fitting routine. Multiple ethanol doses can be handled.The mathematical modeling was based on the following assumptions and simplifications, respectively. A single enzyme system is responsible for ethanol conjugation at one distinct site; the distribution of EG into the systemic circulation is delayed; the elimination of EG follows first-order kinetics.The concentration of EG was calculated using three kinetic parameters: a rate constant for the first-order formation of EG from serum ethanol, a transfer constant for an obstructed transfer of EG from the formation site (FS) into the central compartment (CC) and an exponential elimination constant.The program was applied to the data collected from 21 drinking experiments. The fitting algorithm optimized the three kinetic parameters, until the sum of concentration error squares of the data points was minimized. The means+/-standard deviation of the rate constant for the first-order formation of EG from serum ethanol was 0.0011+/-0.0006 h(-1), the transfer constant for an obstructed transfer of EG from the FS into the CC was 0.43+/-0.1996 h(-1) and the exponential elimination constant was 3.0+/-1.45 h(-1).Using the range of these parameters, it is now possible to calculate minimum and maximum serum concentrations of EG based on ethanol doses and drinking times. The comparison of calculated and measured concentrations can prove the plausibility of an alleged ethanol consumption. This can be crucial when the serum ethanol concentration (SEC) itself is not meaningful.

Zusammenfassung Nur selten schafft es eine verwaltungsgerichtliche Entscheidung an die erste Stelle der ARD-Tagesschau um 20 Uhr. Dem Urteil des BVerwG vom 27.9.2007 ist das gelungen. Es liefert die Antwort auf eine Frage, die sich mancher TV-Zuschauer und Zeitungsleser seit der ersten H?lfte des Jahres 2005 gestellt haben mag. Einer aufgeregten Medienberichterstattung war damals zu entnehmen, dass der gerade verbindlich gewordene Feinstaubgrenzwert vielerorts überschritten werde und davon Betroffene Erkrankungen des Herz-Kreislauf-Systems und der Atemwege, ja sogar eine Verkürzung ihrer Lebenserwartung zu befürchten h?tten. Die Frage die sich stellte: Was kann ein Betroffener dagegen unternehmen?     

Industrial Relations and Justice Ideologies in the Firm: A Justice-Based Explanation of Works Council Effectiveness

This paper investigates why the effectiveness of works councils varies across firms. Drawing on a large body of literature on justice in organizations, and on the basis of the premise of methodological individualism, we present the groundwork for a justice-based theory of workplace codetermination. The theory focuses on workers' attitudes toward justice in organizations, the structural reasons behind these attitudes, and their consequences for the effectiveness of codetermination. Results from a qualitative employee survey in four German firms provide first empirical support for the theory presented.

Staying in the Game or Changing It: An Analysis of Moves and Turns in Negotiation

As parties bargain over the terms of an agreement, they are concurrently negotiating their relationship. In this parallel negotiation, parties seek to position themselves to advantage by using a variety of strategic moves. In so doing the other party can be put into a defensive position making it difficult to advocate effectively. Turns, such as interrupting, correcting, questioning, naming, and diverting, challenge these moves. Turns can be used restoratively to move out of a defensive position or participatively, to engage the other in collaboration. Anticipating strategic moves and having turns in mind is part of preparing to negotiate.     

Does immigration weaken natives’ support for?the?unemployed? Evidence from Germany

Using data from the 1997 and 2002 waves of the German Socio-Economic Panel and from official statistics, I study whether natives are less supportive of state help for the unemployed in regions where the share of foreigners among the unemployed is high. Unlike previous studies, I use individual-level panel data, which allows a more convincing identification of a causal effect. I find that the negative bivariate association is mainly driven by observed individual differences, such as East German origin or income. While there remains some evidence of a negative association even after adjusting for observed and unobserved individual differences, the association is relatively weak.

Recently, a growing number of researchers claim that income is only one dimension of social inequality. Neglecting wealth as a more profound measure of an entity??s financial situation can lead to a misleading or even wrong understanding of social inequalities. This article examines the relationship between income and wealth among elderly people in 13 European countries and tries to answer the following two research questions: Are the income-rich also the asset-rich? And what is the composition of wealth within different income groups? The analyses show that higher wealth often goes along with higher income, whereas a high income does not necessarily correlate with high wealth. Instead, in relation to the amount of wealth they possess, the asset-rich do not dispose of a high income. This may be an indicator that income is not the most important mechanism for the accumulation of wealth in the upper part of the wealth distribution. In general the wealth portfolio of asset-rich households is much broader and therefore less ??crises-prone?? than of the asset-poor. Homeownership is the central component in the wealth portfolio of households. For the lowest income quartiles it is often the only meaningful asset. Summing up, the previous income-based definition of poverty and wealth proves to be inadequate. Analyzing wealth in addition to income, it may turn out that the often proclaimed problem of old-age poverty has been either under- or overestimated.     

Forensic analysis of carbohydrate-deficient transferrin (CDT) by HPLC--statistics and extreme CDT values

Background

Carbohydrate-deficient transferrin (CDT) is the most specific serum marker of chronic alcohol abuse so far. There is little knowledge about extreme CDT values of >20% and the more >30% CDT.

Methods

Serum CDT/transferrin ratios from 19,236 serum samples sent to our laboratory for routine CDT analysis were determined by HPLC. About 75% of these serum samples were from traffic or employment medicine investigations. A CDT value frequency histogram was computed and extreme CDT values were clinically validated.

Results

Fourteen thousand four hundred and sixty-one CDT results were normal (≤1.7%) and 4775 increased (1.8–36.9% CDT). Most frequent normal and increased results were 0.9% CDT (n = 1964) and 1.8% CDT (n = 356). CA. 70% of the pathological results were between 1.8% and 5.0% CDT, ca. 88% between 1.8% and 10.0% CDT, and 98% between 1.8% and 20.0%. CDT values >20.0% appeared in 79 cases and results >30.0% in two cases (33.8% and 36.9%). In each case of CDT values >20%, chronic alcohol abuse was the underlying cause as confirmed by anamnestic exploration.

Conclusions

CDT/transferrin ratios are usually <20%. Higher values can appear in rare cases. CDT results >30% can be due to alcohol abuse but should be considered as remarkable single observations. Visualization of the transferrin isoform patterns by HPLC allows the detection of pathological transferrin isoform patterns and of genetic transferrin variants. This is essential for a reliable interpretation of (extreme) CDT values. CDT analysis by immunoassays without physico-chemical confirmatory analysis is no longer acceptable.
