The Affordable Care Act (ACA) aimed to achieve nearly universal health insurance coverage in the United States through a combination of insurance market reforms, mandates, subsidies, health insurance exchanges, and Medicaid expansions, most of which took effect in 2014. This paper estimates the causal effects of the ACA on health insurance coverage in 2014 using data from the American Community Survey. We utilize difference‐in‐difference‐in‐differences models that exploit cross‐sectional variation in the intensity of treatment arising from state participation in the Medicaid expansion and local area pre‐ACA uninsured rates. This strategy allows us to identify the effects of the ACA in both Medicaid expansion and non‐expansion states. Our preferred specification suggests that, at the average pre‐treatment uninsured rate, the full ACA increased the proportion of residents with insurance by 5.9 percentage points compared to 2.8 percentage points in states that did not expand Medicaid. Private insurance expansions from the ACA were due to increases in both employer‐provided and non‐group coverage. The coverage gains from the full ACA were largest for those without a college degree, non‐whites, young adults, unmarried individuals, and those without children in the home. We find no evidence that the Medicaid expansion crowded out private coverage.  相似文献

This article will examine the development and implementation of the new geography curriculum in secondary schools in Shanghai. Analysis of the processes and mechanisms which underlie curricular change in China reveals how embedded bureaucratic, social and cultural norms have profoundly influenced the degree to which reforms to the geography curriculum have achieved the desired results. The analysis begins with a detailed examination of the wider institutional hierarchies which initiated and managed curricular reforms in Shanghai. Interviews with teachers and extensive classroom observations, and a review of the new curriculum and geography textbooks are utilised to evaluate the practical outcomes of curricular change in Shanghai. The findings reveal a deep dichotomy between the intended goals of curricular reform, the processes of curriculum development and implementation, and an examination system which compels teachers to cling to traditional teaching methods of lecturing and rote learning. These findings are discussed in the context of the established educational hierarchies, the cultural politics of curricular reforms in Shanghai and the nature of geography teaching in China. A more balanced approach which addresses the provision of adequate resources and professional development for teachers, and which recognises the need for a fundamental restructuring of the examination system, is suggested.  相似文献

This article highlights the need for an inclusive and integrated policy‐making model by drawing on the experiences of the bushfire fighting efforts of community, business and government bodies during the bushfires that ravaged North‐East Victoria during January and February of 2003 and the delivery of recovery assistance since the fires. These experiences revealed some shortfalls which militated against delivering public value for the communities affected during and after the fires. A policy framework has been developed in light of these shortfalls and is used here to reveal the ways in which bushfire management policy and practice needs to be ‘modernised’ if arrangements in the future are to deliver public value.1  相似文献

With the industrial restructuring of former socialist countries of Eastern and Central Europe (ECE), the local R and D system experienced fundamental changes as well. Specialized R and D institutions that supplied technology to enterprises were downsized dramatically. With the privatization of the industry, multinational corporations became major players in the advanced sectors of the industry. Sourcing of technology within the multinational corporate system increased import of technology and locally performed R and D dropped sharply. The rapid decline of R and D staff both at specialized research institutions and at the enterprise level coincided with a sharp drop in local patent applications. Operation of multinational corporations has also resulted in vertical and horizontal spillover of technology. The evidence on the extent of such transfer has varied among ECE transition economies and industrial sectors.  相似文献

The article focuses on Central and Eastern European (CEE) countries' experiences related to Afghanistan, Iraq and Libya, three non-European theatres of Western military operations, in predominantly Muslim lands, in the decade between 2001 and 2011. CEE countries readily became involved in two of these foreign missions (Afghanistan and Iraq) because of their deep ties to Western politico-economic structures, without direct security interests compelling them to do so, but not without normative convictions regarding what were seen by them as virtues of the two missions. In Libya, however, they were reluctant to join the Western intervention. In light of this, the article is interested in examining how political elites within the region relate to the generally constrained security policy agency that they have. A key argument advanced is that such agency may be located in how external hegemony is mediated in elite discourses of threat and legitimacy construction. This as well as the three case studies outlined in the article show that the seeming changes in CEE countries' behaviour in fact boil down to a simple set of rules guiding their behaviour. Having identified this “algorithm” as an implicit pattern of CEE foreign policy behaviour, originating in the intra-alliance security dilemma within the North Atlantic Treaty Organisation (NATO), the article formulates its conclusions about the alliance policy of these countries largely within a neorealist framework.  相似文献

This paper examines the introduction of premiums into the SCHIP program in Kentucky. Kentucky introduced a $20 monthly premium for SCHIP coverage for children with family incomes between 151 percent and 200 percent of the federal poverty level in December 2003. Administrative data between 2001 and 2004 is used to estimate a Cox proportional hazard model that predicts enrollment duration in this premium-paying category. The results suggest that a premium reduces the length of enrollment, with the impact concentrated in the first three months after the introduction of the premium. Similar results are not found for the non-premium-paying category.  相似文献
