Maternal hepatic rupture is a rare complication of pregnancy that can be fatal to both mother and child. This phenomenon is most often associated with preeclampsia/eclampsia and/or HELLP syndrome, which is defined by a collection of clinical features including hemolysis (H), elevated liver enzymes (EL), and a low platelet count (LP). These disease processes are typically identified and treated during pregnancy, often in the last trimester. The described case is unusual in that the decedent had no known history of preeclampsia/eclampsia or HELLP syndrome during this pregnancy, and she died suddenly several days postpartum of liver rupture with massive intraperitoneal hemorrhage following a routine cesarean section delivery and an uneventful hospital course. Similar cases are infrequent in the literature, which is reviewed in this report.  相似文献

This paper analyses elements of the legal process of consent to the donation of 'spare' embryos to research, including stem-cell research, and makes a recommendation intended to enhance the quality of that process, including on occasion by guarding against the invalidity of such consent. This is important in its own right and also so as to maximise the reproductive treatment options of couples engaged in in vitro fertilisation (IVF) treatment and to avoid possible harms to them. In Part 1, with reference to qualitative data from three UK IVF clinics, we explore the often delicate and contingent nature of what comes to be, for legal purposes, a 'spare' embryo. The way in which an embryo becomes 'spare', with its implications for the process of consent to donation to research, is not addressed in the relevant reports relating to or codes of practice governing the donation of embryos to research, which assume an unproblematic notion of the 'spare' embryo. Significantly, our analysis demonstrates that there is an important and previously unrecognised first stage in the donation of a 'spare' embryo to research, namely: consent to an embryo being 'spare' and so, at the same time, to its disuse in treatment. This is not explicitly covered by the Human Fertilisation and Embryology (HFE) Act 1990, as amended by the HFE Act 2008. Having identified this important initial stage in the process of consent to the donation of a 'spare' embryo to research in conclusion to Part 1, in Part 2 we analyse the idea of consent to an embryo's disuse in treatment on the basis that it is 'spare' with reference to the legal elements of consent, namely information as to nature and purpose, capacity, and voluntariness. We argue that there are in fact three related consent processes in play, of which the principal one concerns consent to an embryo's disuse in treatment. If the quality of this first consent is compromised, in turn this will impact on the quality of the consent to the donation of that 'spare' embryo to research, followed by the quality of consent to future cycles of assisted reproduction treatment in the event that these are needed as a result of a donation decision. The analysis overall is of central relevance to the debate as to whether, and if so when, it should be permissible to request the donation of fresh embryos for research, as opposed to those that have been frozen and, for instance, have reached the end of their statutory storage term. This has a particular bearing on the donation of embryos to stem-cell research since there is a debate as to whether fresh embryos are most useful for this.  相似文献

This study was designed to compare drug-abusing adolescents and their families with nondrug-abusing adolescents and their families by (1) a comparison of the adolescents on measures of self-esteem and perceived parental behavior, (2) a comparison of the parents of drug-abusing adolescents with the parents of nondrug-abusing adolescents on child-rearing attitudes and on distortion in perceived parental behavior, and (3) an identification of the salient factors contributing to the prediction of drug-use in adolescents. The subjects in this study were 26 clinical inpatient drug-abusing adolescents and their parents and 26 nondrug-abusing adolescents and their parents. Testdata consisted of the adolescent's self-esteem score on the Self Esteem Inventory (SEI), the adolescent's factor score on the Child's Report of Parental Behavior Inventory—Revised (CRPBI-R), and the parents' scale score on each of the five factors of the Parent Attitude Survey (PAS). Findings indicate that the adolescent's self-esteem and perception of parental behavior, the ability of the parents to predict the child's parental perceptions, and the professed parental attitudes toward confidence and responsibility in child rearing all combine to suggest a set of factors differentiating the drug-abusing adolescent from the nondrug-abusing adolescent.  相似文献

In this paper, we discuss findings from two studies designed to access and analyse the beliefs, attitudes, and behaviours of health-care professionals and scientists working in morally contested fields of biomedicine that involve the embryo. We seek to support the view that the embryo typically 'matters' to the people we interviewed and whose work we observed, even though it is impossible for them to agree in terms of why that is, and even though their work is of the type to which the moral guardians of the embryo object. In the first part of this paper, we touch on the policy and legal position in relation to embryos, noting Margot Brazier's account of the development of the relevant regulation in the UK and the importance of her claim that the embryo is widely thought to have an important symbolic value. We then turn to explore some of the views, attitudes, and work practices of those whose work involves the embryo, whether that be in relation to fertility treatment services, including IVF and PGD, or research that uses embryos. Our discussion shows the extent to which the embryo typically matters, in various ways, to those working in these fields.  相似文献

This paper examines the concept of putting history “under the power of the public”. It discusses the role played by former employees in researching and publishing the histories of two former large industrial history/heritage sites in Perth, Western Australia. The projects originated not in academia but from past employees who sought the help of labour historians to record the histories. Former employees provided much of the research material and assisted as sources of information, interviewers and interviewees, site guides and critics of work put forward for publication. While academic authors were responsible for interpreting their own research, past employees had a voice in the ways in which the sites’ stories were presented to the public, and the veracity of that information. The article explores the subversive potential of subaltern histories such as these, as well as some principles for putting history under the power of the public. It concludes by examining the strengths and weaknesses of this method, including the extent to which the pressures of academia discourage historians from engaging with “popular” projects requiring “a different voice”.  相似文献
