Medicare is a large government health insurance program in the United States that covers about 60 million people. This paper analyzes the effects of Medicare insurance on health for a group of people in urgent need of medical care: people with cancer. We used a regression discontinuity design to assess impacts of near-universal Medicare insurance at age 65 on cancer detection and outcomes, using population-based cancer registries and vital statistics data. Our analysis focused on the three tumor sites for which screening is recommended both before and after age 65: breast, colorectal, and lung cancer. At age 65, cancer detection increased by 72 per 100,000 population among women and 33 per 100,000 population among men; cancer mortality also decreased by nine per 100,000 population for women but did not significantly change for men. In a placebo check, we found no comparable changes at age 65 in Canada. This study provides the first evidence to our knowledge that near-universal access to Medicare at age 65 is associated with improvements in population-level cancer mortality.  相似文献

As attention has shifted to the topic of change in organised economies, Finland represents a valuable case study. The nation's leap from paper producer to high technology leader represents a unique opportunity to study restructuring within the confines of an allegedly incremental organised economy. To date, Finland's transformation has been interpreted as a product of sweeping liberal reforms in finance and corporate governance. This article, however, identifies an enduring role for collectively organised economic actors in labour market and technology policies. Cooperation in these arenas did not inhibit restructuring. On the contrary, collaborative technology policies actively facilitated the rapid redistribution of resources to an emerging ICT industry. In exploring the opportunities and constraints associated with these policies, this article not only sheds light on Finland's shift into high technology markets, but also identifies a more flexible role for coordination in the new economy.  相似文献

This article examines how the Foreign Office’s Information Research Department (I.R.D.) worked with SAVAK, the Iranian intelligence service, between 1956 and 1968. It explores how a mutual fear of the Soviet Union compelled both departments to work together to produce and disseminate anti-communist propaganda. As well as the publication of books, prominent Iranian journalists would be given stories that praised the Shah’s regime and discredited the Soviet Union. SAVAK figures were also invited to Britain by the I.R.D. to attend training sessions on producing propaganda. The ties between both agencies, however, meant SAVAK was able to persuade the I.R.D. to assist in the conception of anti-Arab Nationalist propaganda, something that contravened wider British foreign policy. Moreover, such an agency-led approach was rendered short-lived. By 1968, many of the SAVAK figures friendly to the I.R.D. were promoted and replaced by individuals less keen on working with the British.  相似文献

This extensive statistical study focuses on fertility patterns during the postwar period in Taiwan. The analytical technique is economic, with socioeconomic variables generally considered the important determinants of fertility; on the other hand, female education and labor force participation were seen to exert a strong negative effect on fertility. Taiwan has reduced birth rates nearly 50% in the period from the 1950s-1970s. In 1972, Taiwan's birth rate/woman was 3.4, a 50% reduction from 1950, generally attributed to institution of a well-conceived family planning program in 1964. It is hypothesized that the socioeconomic forces (presented in 7 comprehensive tables) which influenced negatively the rate of births, worked primarily to reduce excess rather than desired fertility. The clear connection between women's participation in the labor force and reduction in desired fertility leads to the suggestion that stronger economic incentives must be presented to women. Given the already wide availability and low cost of birth control devices, further fertility reductions caused by expanded participation in the family planning program are not likely to reduce desired family size significantly. Instead, it is argued that such reductions tend to occur slowly and to be associated with more economically meaningful roles for women. Analyzed on a cost-benefit basis, the fertility control efforts of the Taiwanese government should be directed to achieving a synchronization between the skill levels demanded by the economy and those acquired in the system of higher education. Economic incentives for fewer births would then augment, rather than offset, the presently extant negative effects of slowly changing attitudinal variables and economic development. Lesbian, gay, bisexual, and transgender (LGBT) people constitute approximately five percent of the population, but workplace discrimination based on sexual orientation and gender identity remains troublingly commonplace. This article summarizes a large and growing body of social science research demonstrating the economic benefits of LGBT inclusion (and the costs of LGBT exclusion) in the workplace. It concludes with recommendations for (1) future research, stressing the need for more China-specific studies, which would inform policymaking and theoretical understanding of diversity and inclusion practices on business performance, (2) improving employer LGBT-inclusion policies, and (3) strengthening state protection of the equal employment rights of LGBT people.  相似文献
